Under the Shelter Supervisor, provides individualized employment case management services, including short-term and long-term support and will work to expand current programming by developing and facilitating employment curriculum, including job skills and other life skill classes.
Schedule: Monday-Friday, 8am-5pm
Pay Range: $18-20/hour
Accountabilities:

Receives prospective clients and determines eligibility for services

Admits and exits clients through established procedures
-
Develops employment plan with clients so that they may return to a self-sufficient life. Advises client in employment goal setting to attain successful independent living

Works collaboratively with Shelter Staff so that employment plan complements clients housing plan
-
Facilitates employment and life skill workshops
-
Coordinates referrals to a spectrum of community resources with an emphasis on areas that support employment growth. Maintains professional contact with community resources and advocates client needs

Records assessments, case management notes, service plans, vouchers, CQI, release of information, exit summaries and referral services

Works with clients until the time of shelter discharge, assess progress and determine eligibility for enrollment into long-term supported services

Closes client files at conclusion of services

Enters and updates program demographic and service data in the Catholic Charities TIER computer system

Records Incident Reports and Work Orders in Catholic Charities TIER computer system, as required

Attends required trainings.

Serves as on-call supervisor on a rotating basis

Performs other related assigned duties
Requirements:

Education
-
Degree in social work, social sciences or other related fields preferred
-
A combination of education and experience will be taken into consideration and may be substituted for degree

Certification, Recertification, Registration, or License

Class C driver's license, motor vehicle, and personal motor insurance required

Experience

Minimum of two years professional experience successfully working with low-income, economically disadvantaged families required

Preference given to applicants who have work experience in job readiness, homelessness, addiction, and mental health services
