SAFS - SEEDS Case Manager

Under the Shelter Supervisor, provides individualized employment case management services, including short-term and long-term support and will work to expand current programming by developing and facilitating employment curriculum, including job skills and other life skill classes.

Schedule: Monday-Friday, 8am-5pm

Pay Range: $18-20/hour

Accountabilities:

  • Receives prospective clients and determines eligibility for services
  • Admits and exits clients through established procedures
  • Develops employment plan with clients so that they may return to a self-sufficient life. Advises client in employment goal setting to attain successful independent living
  • Works collaboratively with Shelter Staff so that employment plan complements clients housing plan
  • Facilitates employment and life skill workshops
  • Coordinates referrals to a spectrum of community resources with an emphasis on areas that support employment growth. Maintains professional contact with community resources and advocates client needs
  • Records assessments, case management notes, service plans, vouchers, CQI, release of information, exit summaries and referral services
  • Works with clients until the time of shelter discharge, assess progress and determine eligibility for enrollment into long-term supported services
  • Closes client files at conclusion of services
  • Enters and updates program demographic and service data in the Catholic Charities TIER computer system
  • Records Incident Reports and Work Orders in Catholic Charities TIER computer system, as required
  • Attends required trainings.
  • Serves as on-call supervisor on a rotating basis
  • Performs other related assigned duties

Requirements:

  • Education
    • Degree in social work, social sciences or other related fields preferred
    • A combination of education and experience will be taken into consideration and may be substituted for degree
  • Certification, Recertification, Registration, or License
    • Class C driver's license, motor vehicle, and personal motor insurance required
  • Experience
    • Minimum of two years professional experience successfully working with low-income, economically disadvantaged families required
    • Preference given to applicants who have work experience in job readiness, homelessness, addiction, and mental health services

    • Efficient organizational and planning skills
    • Effective time management and ability to prioritize work
    • Ability to operate effectively in multidisciplinary professional environment and to work with others to build consensus in operating and setting realistic goals for the Agency
    • Ability to lead by example and to mento and guide others
    • Computer proficiency (including Word, Excel, Outlook, and databases)
    • Strong organizational skills and demonstrated capacity to develop and implement practical strategies and solutions to identified issues and problems
    • Ability to work independently
    • Provel skills in interpersonal communication and relationship building with co-workers, board members and external stakeholders
  • Exposures
    • Controlled inside work environment with noise/vibrations from general office equipment
    • Minimal exposures to fumes/dust/odors
  • Physical Demands
    • Work Type: Light, exerting up to 50 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects
    • Climbing/Balancing/Freedom to Move: May be assigned to multilevel building with some floors accessible by stairs only. Minimal required moving about the office. Sit about 25 percent of the time
    • Stooping/Bending: Regularly
    • Reaching: Occasional, overhead as well as horizontal and down
    • Vision: Adequate to perform essential functions
    • Color Vision: No particular color vision requirement
    • Hearing: Frequently perceive nature of sounds by ear
    • Speech: Frequently express ideas by means of spoken words
    • Eye/Hand/Foot Coordination: Frequently operates equipment requiring moderate ability
