Join our dedicated team as a Scheduling Coordinator, where you will play a vital role in coordinating care schedules and providing compassionate support to individuals in need. Duties
- Develop and manage daily care schedules for clients, ensuring all needs are met promptly and efficiently
- Communicate effectively with clients, families, healthcare providers, and team members to coordinate care plans and updates
- Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs, medication administration, and overall well-being; document observations accurately
- Support clients with meal preparation, light cleaning, and maintaining a safe living environment in accordance with care plans
- Utilize specialized equipment such as Hoyer lifts or wheelchairs to assist with mobility and transfers safely
- Maintain strict confidentiality of client information in compliance with HIPAA regulations and uphold privacy standards at all times
- On call 1 night per week and rotating holidays
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or group home settings
- Knowledge of patient observation techniques, medication administration, and vital signs monitoring
- Hands-on caregiving experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer’s), or memory care needs
- Familiarity with care plans, HIPAA compliance, and documentation procedures
- Ability to perform heavy lifting safely and use assistive devices like Hoyer lifts effectively
- Strong communication skills to coordinate seamlessly with clients’ families and healthcare teams
- Compassionate attitude combined with excellent organizational skills to manage multiple schedules efficiently
