Scheduling Coordinator

Join our dedicated team as a Scheduling Coordinator, where you will play a vital role in coordinating care schedules and providing compassionate support to individuals in need. This dynamic position combines organizational expertise with hands-on caregiving, ensuring that clients receive personalized, timely, and high-quality care. Duties

  • Develop and manage daily care schedules for clients, ensuring all needs are met promptly and efficiently
  • Communicate effectively with clients, families, healthcare providers, and team members to coordinate care plans and updates
  • Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patient health by observing vital signs, medication administration, and overall well-being; document observations accurately
  • Support clients with meal preparation, light cleaning, and maintaining a safe living environment in accordance with care plans
  • Utilize specialized equipment such as Hoyer lifts or wheelchairs to assist with mobility and transfers safely
  • Maintain strict confidentiality of client information in compliance with HIPAA regulations and uphold privacy standards at all times
  • On call 1 night per week and rotating holidays

Requirements

  • Proven experience in home & community care, assisted living, nursing homes, or group home settings
  • Knowledge of patient observation techniques, medication administration, and vital signs monitoring
  • Hands-on caregiving experience working with individuals with developmental disabilities, autism, dementia (including Alzheimer’s), or memory care needs
  • Familiarity with care plans, HIPAA compliance, and documentation procedures
  • Ability to perform heavy lifting safely and use assistive devices like Hoyer lifts effectively
  • Strong communication skills to coordinate seamlessly with clients’ families and healthcare teams
  • Compassionate attitude combined with excellent organizational skills to manage multiple schedules efficiently
