Major Responsibilities:<ul><li>
Evaluates, diagnoses, treats, and manages acute, critical and chronic patients illness and diseases within both an outpatient clinic and inpatient hospital setting.</li><li>
Obtains medical histories, conducts physical examinations and assesses and determines differential diagnosis. Prescribes medication, therapy and treatments accordingly.</li><li>
Interprets and evaluates patient data for the purpose of determining management and treatment of patients. Performs minor surgeries and routine procedures. Consults with physician as appropriate. Provides care coordination, which includes managing a variety of ancillary services, pre-operative plans, discharge planning, provider referrals and medical devices such as insulin pumps, defibrillators, and pacemakers.</li><li>
Prescribes, recommends and regulates pharmacologic and non-pharmacologic therapies across multiple care settings, including controlled medications that are warranted by patient's diagnosis or changing medical condition.</li><li>
Prepares, dictates and signs patient care plans and summaries of patient care. Maintains records according to established standards and guidelines.</li><li>
Provides clinical direction to nursing staff regarding the continued care of patients. Follows through with nursing staff to ensure patient receives recommended care.</li><li>
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. <li>Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care and length of stay.</li>
<li>Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with the collaborating physician.</li>
<li>Performs complete health history, physical examination and psychosocial assessment of patients within the designated practice or clinical unit.</li>
<li>Functions as a respectful member of the health care team.</li>
<li>Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies.</li>

Minimum Qualifications: 
To qualify you must have:

<li>Licensure: Current registration as Professional Nurse in New York and Registration as Nurse Practitioner in New York.</li>
<li>Education: Master's Degree in Nursing.</li>
<li>Certification required (as soon as feasible following graduation from NP graduate program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical area of practice.</li>

Previous experience as a nurse practitioner is preferred. Position Qualifications:

Collaborative Relationships:<ul><li>Build strong, collaborative working relationships with facility leadership, including administrators and nursing supervisors.</li><li>Engage with interdisciplinary teams (social workers, rehabilitation therapists, dietitians, etc.) to ensure holistic patient care.</li><li>Serve as a liaison between the facility, patients&rsquo; families, and external medical professionals to coordinate care and communicate patient progress.</li></ul>4. Compliance &amp; Documentation:<ul><li>Ensure compliance with all federal and state regulations, including OTLC, HIPAA, and OSHA guidelines.</li><li>Maintain accurate, detailed, and timely medical records.</li><li>Participate in quality improvement initiatives.</li><li>Remain current with continuing education and maintain all licensure and certifications required for practice.</li></ul>5. Health and Safety Oversight:<ul><li>Participate in infection prevention protocols and ensure that all medical interventions adhere to best practices in hygiene and patient safety.</li><li>Educate staff and patients on health promotion and disease prevention, especially concerning vulnerable populations in long-term care settings.</li></ul>6.
