Responsibilities 
<ul><li>
Performs all clinical practitioner service activities in the designated specialty area, including obtaining histories, performing physical exams, making assessments, and ordering tests to adequately assess, determine diagnoses, and plan care.</li><li>
Provides optimal medical decision making and patient care management, consistent with the PHHS mission, to ensure good health outcomes.</li><li>
Uses written medical protocols and consultants appropriately.</li><li>
Records health history, findings, treatments, and recommendations accurately, clearly, and concisely.</li><li>
Manages time effectively to ensure optimal use of patient and provider resources. Consults appropriately with supervising physicians and refers patients as necessary to ensure optimal patient outcomes.</li><li>
Includes patients and/or families in determining the plan of care, when appropriate. Educates patients and their families to ensure self-management, follow-up, and adherence to established treatment.</li><li>
Arranges appropriate follow-up appointments.</li><li>
Develops appropriate patient education materials and may participate in community outreach activities to promote education and PHHS as a patient-centered/patient-valued healthcare provider.</li><li>
Trains, supervises, and evaluates APP students and contributes to the education of residents, interns, and other healthcare trainees.</li><li>
Serves as a clinical resource to other APPs and other allied health professionals. Serves as a role model for junior staff and APP students.</li><li>
Stays abreast of the latest developments, advancements, and trends in the given practice field by attending seminars/workshops, reading professional journals, and actively participating in professional organizations.</li><li>
Participates in PHHS education activities, attends, and provides in-services. Educates peers and staff both formally and informally.</li><li>
Seeks information to ensure maintenance of a collaborative learning environment.</li><li>
Integrates new knowledge into their practice, takes initiative to learn new techniques and procedures, and shares skills and information with the healthcare team.</li><li>

Duties &amp; Responsibilities<ul><li>
Provide primary and acute health care to individuals aged 13 and older who are present at the mobile health unit, according to the regulations as indicated on Scope of Practice signed by the Medical Director.</li><li>
Document clinic visits and all telephone or verbal contacts in patient's charts.</li><li>
Reviews initial and follow-up of labs and diagnostic tests.</li><li>
Participate in the training of students assigned to the Health Center.</li><li>
Provide health care based on clinical protocols, which have been reviewed and approved annually by the Medical Director.</li><li>
Collaborate with outreach teams to identify, locate, and engage vulnerable, underserved individuals and populations.</li><li>
Meets every three months with the Chief Medical Officer and reviews ten random charts in which medication was prescribed. An ongoing list is to be maintained by the ANP. Completion of this review will be filed with the clinic nurse manager.</li><li>
Attend Health Center staff and Mobile Health Team meetings.</li><li>
Consult and refer to Health Center doctors and other doctors as needed to provide optimal care.</li><li>
Attend conferences and trainings as approved by the clinic manager that reflect clinic</li><li>
Other duties as assigned.</li></ul>
Hourly rate: $55 - $70/hour 
 
Experience and Skills:
Education &amp; Experience<ul><li>
Current license to practice as a registered nurse in Massachusetts.</li><li>
National certification as an Adult Nurse Practitioner.</li><li>
Massachusetts license to practice in the expanded nursing role.</li><li>
Possess or obtain prescription privileges in Massachusetts.</li><li>
Certified in Basic Life Support (CPR)</li></ul>
