Program Overview

The Advanced Care Partner Program creates a team-care structure to collaborate with PCPs who are managing complex patient panels.

Advanced Care Partner Role:

The Advanced Care Partner is a full-time, remote clinician who works business hours live online with a team of PCP peers, a clinical supervisor, and the Clinical Case Management team of Care Navigators and nurses, as needed, for highly complex cases. The ACP will partner with approximately 5 PCPs and their patient panels, owning these shared panels with longitudinal accountability over time.

The ACP is responsible for maintaining task category SLAs, including but not limited to:

<li>Answering clinical questions from patients and engaging in clinical dialogue to complete these conversations.</li>
<li>Clinical decision-making resulting from information coming from messages, lab results, and consults.</li>

What You&rsquo;ll Be Doing</h3>
As part of the inpatient team, you&rsquo;ll play a key role in delivering high-quality cardiovascular care within affiliated hospitals:<ul><li>
Perform inpatient consults and daily rounding on cardiology patients</li><li>
Evaluate and manage acute cardiac conditions including heart failure, arrhythmias, chest pain/ACS, and post-procedural patients</li><li>
Partner closely with cardiologists on diagnosis, treatment planning, and care progression</li><li>
Order and interpret diagnostic testing (labs, EKGs, imaging, stress testing as applicable)</li><li>
Manage medications, discharges, and transitions of care</li><li>
Document encounters, histories, and clinical decisions accurately and efficiently</li><li>
Educate patients and families on treatment plans and follow-up care</li><li>
Participate in a shared call rotation</li></ul>

Weekend Psychiatrist

Compensation: per hour plus per encounter rate (1099 or W2)
Location: telehealth 
Responsibilities:<ul><li>
Must have interstate compact license.</li><li>
The psychiatrist will conduct comprehensive psychiatric assessments to diagnose mental health disorders.</li><li>
Prescribe and monitor psychotropic medications ensuring that treatment plans are effective and adjusted as needed</li><li>
The psychiatrist will create individualized treatment plans that include medication management, psychotherapy, and other interventions.</li><li>
Chart audits, peer review, meetings, provider management, etc.</li><li>
The psychiatrist will maintain up to date medical records, ensuring compliance with facility policies, state regulations, and standards of care.</li><li>
Maintain and monitor confidentiality of patients and their files; audit files to determine that all services provided are documented; review status of patients to verify that all treatment and therapeutic programs are completed.</li><li>
The psychiatrist will work closely with psychologists, social workers, nurses, and staff to provide integrated and coordinated care.</li><li>
Provide immediate psychiatric care in crisis situations, such as suicide attempts or acute psychiatric episodes, and work to prevent incidents through proactive mental health management</li><li>
Endeavor to comply with the requirements of applicable regulations, laws, rules, procedures, policies, standards and/or contract.</li></ul>
Qualifications:<ul><li>
Psychiatrist is required to have interstate compact license</li><li>
2 years of inpatient experience (trauma experience required)</li><li>
Board Certified or Eligible (within 1 year of residency completion) in Psychiatry</li><li>
Valid and unrestricted DEA</li><li>
Must be proficient in using Telemedicine technologies to treat patients remotely</li><li>
Open to PMHNP collaboration (preferred)</li></ul></div>
