Role Summary</b></p><p>
The Remote Advanced Practice Provider (APP) plays a crucial role in ensuring the success of PocketRN&rsquo;s dementia care (GUIDE) program. Working closely with the Chief Medical Officer, provider liaisons, clinical teams, care navigators, and patients, the APP will review clinical documentation, complete CMS-required attestations, and support timely, high-quality dementia care. This role helps ensure that patients and caregivers have access to the services and support they qualify for.</p><p></p><p><b>

Review and sign off on documentation from care navigators and nurses in a timely manner.</p></li><li><p>
Enter relevant charges into the electronic medical record (Athena) for services provided.</p></li><li><p>

Telehealth &amp; Administrative Responsibilities</b></h3><ul><li><p>
Maintain an active, unencumbered APRN license in all states where care is delivered.</p></li><li><p>
Review and sign off on completed comprehensive assessments, medication reconciliations, and required attestations within 7 days of the patient visit, when serving as the billing provider. This applies to patient visits conducted by nursing staff that requires provider review for completion and billing purposes.</p></li><li><p>
Participate in patient, caregiver, and staff education and coaching when requested.</p></li><li><p>
Conduct remote calls and video visits on PocketRN, including urgent escalation visits when needed. Appropriate documentation must be completed in 24 hours.</p></li><li><p>
Complete all required training to stay current with PocketRN clinical protocols and workflows.</p></li><li><p>
Complete all required clinical competency assessments and orientation checkoffs within 30 days of the start date, demonstrating readiness to independently manage provider visits, and meet organizational clinical standards.</p></li><li><p>
Assist with the creation of clinical content and training materials as requested.</p></li><li><p>
Perform administrative tasks as needed to support program operations.</p></li><li><p>
Cooperate with billing, documentation, credentialing, payor enrollment, and compliance requirements.</p></li><li><p>
Participate in quality assurance, peer review, and performance improvement initiatives.</p></li><li><p>
Prepare accurate and timely electronic medical record entries.</p></li><li><p>

The Advanced Practice Provider diagnoses and treats acute, episodic, or chronic illness, independently or as part of a healthcare team. In addition, this position focuses on health promotion and disease prevention; order, perform, or interpret diagnostic tests such as laboratory work and x- rays; and prescribe medication.</div><div></div><div><b><br>

The Advanced Practice Provider provides medical treatment and services, consistent with the provision of basic health care services, in conjunction with a physician.</li><li>
Formulates comprehensive treatment plans and prognosis based on the patient&rsquo;s health conditions.</li><li>
The Advanced Practice Provider prescribes medication dosages, routes, and frequencies based on patients' diagnosis permitted under State and DEA licensure.</li><li>
Orders, performs, or interprets the results of diagnostic tests such as complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (x-rays) with physician oversight as needed.</li><li>

Working closely with the Chief Medical Officer, provider liaisons, clinical teams, care navigators, and patients, the APP will review clinical documentation, complete CMS-required attestations, and support timely, high-quality dementia care. Review and sign off on documentation from care navigators and nurses in a timely manner.</p></li><li><p>
Enter relevant charges into the electronic medical record (Athena) for services provided.</p></li><li><p>
Complete provider attestations as required for the dementia care program(s).</p></li><li><p>
Provide oversight to care navigators and nurses supporting program beneficiaries.</p></li><li><p>
Respond to escalations from clinical teams and care navigators as needed.</p></li><li><p>
When appropriate, evaluate and diagnose patients with dementia when extenuating circumstances prevent assessment through their existing care team.</p></li><li><p>
Communicate clearly and frequently with patients, caregivers, PocketRN staff, and partners.</p></li><li><p>

Telehealth &amp; Administrative Responsibilities</b></h3><ul><li><p>
Maintain an active, unencumbered APRN license in all states where care is delivered.</p></li><li><p>
Review and sign off on completed comprehensive assessments, medication reconciliations, and required attestations within 7 days of the patient visit, when serving as the billing provider. This applies to patient visits conducted by nursing staff that requires provider review for completion and billing purposes.</p></li><li><p>
Participate in patient, caregiver, and staff education and coaching when requested.</p></li><li><p>
Conduct remote calls and video visits on PocketRN, including urgent escalation visits when needed. Appropriate documentation must be completed in 24 hours.</p></li><li><p>
Complete all required training to stay current with PocketRN clinical protocols and workflows.</p></li><li><p>
Complete all required clinical competency assessments and orientation checkoffs within 30 days of the start date, demonstrating readiness to independently manage provider visits, and meet organizational clinical standards.</p></li><li><p>
Assist with the creation of clinical content and training materials as requested.</p></li><li><p>
Perform administrative tasks as needed to support program operations.</p></li><li><p>
