Position Summary

The Advocate provides crisis intervention, safety planning, education, advocacy, and referrals through telephone and digital contacts. This role requires maintaining a professional, non-judgmental, and empowerment-based approach to client services. The Advocate is responsible for accurate documentation, data collection, and participation in outreach and evaluation efforts. 
Key Responsibilities

Crisis Intervention &amp; Client Support

<li>Provide crisis intervention, safety planning, advocacy, and referrals through phone and digital platforms.</li>

Community Outreach &amp; Public Awareness

<li>Participate in community outreach by developing awareness of agency services.</li>
<li>Network with area service providers and support public education efforts about victimization.</li>

Administrative &amp; Program Support Responsibilities

<li>Adhere to all Family Crisis Centers&rsquo; policies, procedures, and confidentiality.</li>

Qualifications

<li>Education &amp; Experience: High school diploma or equivalent required.

Participation in an on-call rotation is required. Candidates must be available to respond to emergencies at any time, including evenings, weekends, and holidays. This role is highly mobile and requires exercising sound independent judgment and manage responsibilities with minimal supervision. 13. Compile, prepare, submit and maintain accurate records, files, forms, statistics and additional information in accordance with agency policies, licensing and/or contract requirements. 
14. Transport clients, when needed, according to program protocol. 
15. Dispatch within the required time frame to assigned locations, which may include hospitals, police stations, DFPS offices, shelters, community-based settings, or incident locations. 
16. Continuously evaluate, develop, and strengthen the quality of rapport with assigned clients.

Essential Functions: 
1. Independently coordinate referrals, service planning and documentation of services for assigned caseload. 
2. Provide comprehensive, trauma-informed service and safety planning, including immediate risk reduction and short-term therapeutic interventions. 
3. Act as a liaison with stakeholders, including legal providers, child protection, law enforcement, federal partners and others, and ensure both program and local protocol are followed. 
4. Accept crisis calls and provide support to families in crisis. 
5. Participate in weekly face to face or phone supervision with the supervisor. 
6. Travel approximately 75 percent of time throughout the region, including overnight stays. 
7. Conduct comprehensive assessments, develop treatment plans, and make independent decisions in the field regarding the safety and well-being of assigned children and families. 
8. Complete in-home family preservation services as assigned. 
9. Participate in community meetings and directly advocating with stakeholders for child services.</div><div>

The Agency: S.H.A.D.ESurvivors Healing, Advising and Dedicated to Empowerment (SHADE) has a mission to empower survivors of sexually exploitation, human trafficking and domestic violence and create economic opportunities, leadership development, provide a survivor voice missing from the mainstream and make systemic and community change. We provide several services, but our end goal is always to help other organizations develop safe spaces and guidance to help sexually exploited persons to transform their lives. We also work toward long-term solutions to the problem of domestic victims of human trafficking such as research, policy and training for professionals working with children.The Program: Worth It-Street OutreachWorth It -Street Outreach is a survivor-led program that engages with individuals involved in the sex industries and those being trafficked or sexually exploited. We support connections with services that are vital to survivor well-being and a safe exit from the commercial sex industry. Through collaboration with multiple community stakeholders, our Street Outreach Team coordinates with direct service providers engaged in ending homelessness and human trafficking, and local government, who often interface with survivors and the homeless community.Our program is designed as an innovative response to the problem of identifying and serving victims of human trafficking that are unhoused and who are not served under our existing protocols for working within the child welfare system and the justice system, and who do not self-identify as victims nor present themselves for supportive services assistance.Under supervision of the Director and Direct Services Coordinator, the Outreach Advocate will work in Oakland and along the blade/tracks identifying exploited and trafficked individuals. The Outreach Advocate will utilize a trauma informed, multidisciplinary approach to engage the hardest to serve individuals in an effort to connect them to services, including SHADE services, county diversion and coordinated entry services, with the ultimate goal of helping them exit or free themselves.This position requires an outgoing nature and the ability to work independently in the field. Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
 
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
 
Street to Home, Brooklyn
 
Breaking Ground’s Street to Home program makes regular contact with more than 2,000 street homeless individuals and connects more than 500 individuals to housing, medical and mental health services, substance abuse counseling, and other essential supports each year. The caring, individually tailored attention clients receive at each stage of their journey from street to home ensures that more than 95% of people who Breaking Ground places remain stably housed.
 
The Housing Advocate works with street homeless conducting street outreach with the objective of engaging individuals to determine their eligibility for Street to Home services and to help secure appropriate housing solutions for all people who are street homeless. If chronic, the Housing Advocate will build a relationship with clients utilizing motivational interviewing to encourage the client to work towards permanent housing. If not chronic, Housing Advocate will utilize available resources to secure other housing placements. The Housing Advocate will always work with a partner and will spend most of the shift in an automobile conducting street outreach. The incumbent will respond to 311 calls and community concerns; participate in Code Blue and Code Red alerts; identify new hotspots/encampments and work to eradicate existing hot spots/encampments; conduct a bi-annual vulnerability index on all clients in their catchment area; and enter all required data into outreach database(s).
 This position has a 35-hour work week.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:
