Candidates must maintain a permanent residence in California for the duration of their employment.</p><p><b>Primary Responsibilities:</b></p><ul><li>Perform comprehensive health assessments, including physical exams, medical histories, and medication reconciliation.</li><li>Develop and implement individualized care plans in collaboration with the interdisciplinary care team, tailored to each resident&rsquo;s needs and preferences.</li><li>Manage chronic and acute health conditions with timely interventions and appropriate referrals.</li><li>Order, review, and interpret diagnostic tests (e.g., labs, imaging) to support clinical decision-making.</li><li>Prescribe medications and treatments within licensure and in accordance with state laws and facility protocols.</li><li>Lead care initiation for residents enrolling in chronic care management services.</li><li>Educate and counsel residents and families on health maintenance, disease prevention, and self-care practices.</li><li>Work closely with physicians, nurses, therapists, social workers, and other healthcare staff to ensure integrated, high-quality care.</li><li>Participate in quality improvement efforts and interdisciplinary team meetings to optimize resident outcomes.</li><li>Document all patient assessments, treatments, and communications accurately and timely (within 72 hours) in the electronic medical record.</li><li>Support other duties as assigned in alignment with our products and services.</li></ul><p><b>Candidate Profile:</b></p><ul><li>Current, unrestricted Nurse Practitioner license in California; additional state licensure is a bonus.</li><li>Active 103 NP Certification, or eligibility and willingness to obtain it, to practice independently without Physician supervision.</li><li>Minimum of 3 years or 4600 hours of clinical experience in Family Medicine, Geriatric Care, and/or Internal Medicine</li><li>Board certification in at least one of the following subspecialities required: FNP, AGNP, or Internal Medicine preferred.</li><li>Strong clinical assessment, communication, and interpersonal skills.</li><li>Ability to work both independently and collaboratively in a team-based environment.</li><li>Competency in medication management and de-escalation techniques.</li><li>Proficient with electronic medical records and healthcare technology.</li><li>Willingness to travel occasionally within your assigned region to complete initial patient visits.</li></ul><p><b>Compensation &amp; Benefits:</b></p><ul><li>Competitive salary starting at $130,000, depending on experience</li><li>Health, dental, and vision insurance</li><li>Employer paid term life insurance and AD&amp;D</li><li>401k plan with employer match</li><li>PTO and paid holidays</li><li>Company-issued equipment provided</li></ul><p><b>Travel Requirements:</b> This role currently requires approximately 50% travel.
