The Behavioral Health Case Manager works with individuals diagnosed with mental illness to promote recovery, independence, and successful community living. This is accomplished through the provision a variety of treatment services including crisis intervention under the guidance of a therapist (QMHP) and/or supervisor, individual and group skills training, and development; case management services; coordination and monitoring daily structured activities of clients. Must possess a valid Oregon Driver&rsquo;s License.</li><li>
Must be eligible to register as a QMHA through MHACBO.</li></ul>
Primary Responsibilities:<ul><li>
Provides case management, individual and group skills training, and development, mentoring and crisis intervention as directed by a QMHP and documented in the Individual Service and Support Plan (ISSP).</li><li>
Actively promotes the harmonious delivery of services through coordination of interventions in collaboration with treatment team members and foster providers achieved by ongoing communication and active problem solving around outcomes, barriers, and case observations.</li><li>
Maintains and completes required records, reports, documentation as required by departmental policy and state and federal law.</li><li>
Transports individuals, family members and other supports to/from services.</li><li>
Position supports crisis and specialized programs and works with law enforcement, the State and local hospital and CCO&rsquo;s to coordinate clients&rsquo; re-entry into communities.</li><li>
Respond to crisis calls within the community in coordination with the mobile crisis team, providing resources, safety planning and crisis interventions under the supervision of a QMHP.</li><li>
On-going interaction, effective communication, and active collaboration with individuals and families receiving treatment services through Adapt Integrated Health Care, natural and community support, agency partners, and other treatment providers.</li><li>
Emergency Response: This employee has been identified as a staff member expected to report for duty following activation of a public health emergency response plan.</li></ul>

The Behavioral Health Holding area in the Emergency Department provides a safe space for immediate psychiatric evaluation, medication management, and stabilization. The Behavioral Health Triage process ensures patients receive immediate care through timely assessments, medical clearance, and coordination with inpatient psychiatric services. The Behavioral Health Inpatient Unit delivers comprehensive treatment through therapy, education, and personalized care planning, helping patients stabilize, build coping skills, and prepare for safe discharge with ongoing support.
