Kings View is seeking a Care Manager (Peer or Paraprofessional) to support Enhanced Care Management (ECM) and Community Supports (CS) programs. This role works directly with members and their support systems to coordinate care, address gaps in services, and promote whole-person wellness. 
 
The Care Manager partners with a multidisciplinary care team to ensure members receive coordinated services across medical, behavioral health, and social support systems, including addressing social determinants of health. 
 
Key Responsibilities: 
 
Care Coordination &amp; Case Management: 
<ul><li>Conduct outreach, engagement, and care coordination for members</li><li>Complete assessments and develop individualized care plans</li><li>Implement and monitor care plans to ensure member needs are met</li><li>Coordinate with healthcare providers, social services, and community resources</li><li>Prevent duplication of services and ensure seamless care delivery</li></ul> 
Member Support &amp; Advocacy:
