GENERAL SUMMARY: 
Provides consultative services to Bryan Health Connect (BHC) member clinics to support clinical processes and effective and efficient delivery of health care in order to improve quality and decrease health care costs. Works collaboratively with member clinics in measuring performance, identifying gaps in best practice and recommending improvement methods to transform patient care processes and improve cost and quality outcomes.
 
PRINCIPAL JOB FUNCTIONS:

2. *Acts as a subject matter expert in areas of Government, Insurance Payer and Health System regulatory, compliance and payer quality/cost efficiency programs.
 
3. *Develops, implements, and monitors the ongoing process of meeting clinical, operational, and governmental performance measures/objectives and value-based programs.
 
4. *Provides training and ongoing education of clinical users on features, operation, and usage of clinical systems. Selects appropriate training methodology, develops training plan, designs training materials, and trains end users to support the transition into new systems and processes for clinical staff and physicians.
 
5. *Coordinates the design of and generates routine and specialized reports that provide actionable data related to opportunities within clinical systems, quality of care, and clinical performance while assisting with interpretation and meaning of the data as needed.
 
6. *Presents complex data to physicians, administrators and other healthcare professionals.
 
7. *Maintains ongoing, positive, solution-driven relationships by frequently working with BHC clinical staff and physicians at the point of care to foster collaboration, engagement, improved adoption/consistency, and optimization of patient care delivery systems to advance the clinical priorities of BHC.
 
8. Collaborates with EHR departments at BHC sites on configuration to support efficient and effective clinician workflow, while enabling the capture of clinical data required for outcomes analysis, process improvement initiatives, regulatory reporting, and clinical operations management.
 
9. Provides coaching to BHC physicians and clinical support staff on clinical documentation improvement opportunities as well as opportunities involving coding and reimbursement.
 
10. Leverages clinical knowledge of processes, workflows, and evidence-based practice to improve the efficiency and effectiveness of BHC clinic systems and operations.
 
11. Proactively identifies and addresses knowledge deficits and serves as a facilitator of projects to solicit interdisciplinary end user input for analysis of current state, identification of process gaps, design of future state, incorporation of clinical best practices, and ongoing enhancement of clinical system processes.
 
12. *Works with appropriate leadership to implement and maintain the population health management strategy to develop and execute work plans to engage physicians, providers and community partners to improve healthcare quality and financial performance.
 
13. *Travels extensively between sites approximately 3-5 days per week, including evening and early morning meetings as needed. Travel is primarily during the business day, but some overnight travel may be required.
 
14. Utilizes data and evidence-based research to identify practice variability and define targeted and scalable initiatives for improvement.
 
15. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
 
16. Participates in meetings, committees and department projects as assigned.
 
17. Performs other related projects and duties as assigned.
 
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: 
1. Knowledge of government, clinical, and operational quality and performance measures/objectives and value-based programs.

3. Knowledge of certified electronic health record technology (CEHRT), third part data depositories, and government reporting applications.

EDUCATION AND EXPERIENCE: 
Bachelor’s degree in a related field, preferably nursing with three (3) years of work-related experience or Masters of Science degree with one (1) year of work-related experience required. Licensure in a medical or allied health area of study or professional certification preferred. Experience in nursing, allied health, healthcare administration, public health, clinical quality, or informatics preferred.
 
Experience in practice re-design work, practice transformation, quality improvement, population health management, ambulatory care setting, quality and efficiency metrics, and using data to set goals, influence stakeholders and measure progress are highly desired. Previous experience in electronic health/medical records preferred. Experience in performance improvement and project management preferred.
 
OTHER CREDENTIALS / CERTIFICATIONS: 
Valid driver’s license required. Verification and maintenance of responsible driving record per facility standards required.

The Martin Luther King Jr. Behavioral Health Center (MLK BHC) is a program of HealthRIGHT 360 and is
 contracted with the Department of Public Health's Substance Abuse Prevention and Control (SAPC)
 program. MLK BHC offers residential Drug Medi-Cal (DMC) services for a maximum of 99 adult individuals,
 comprising 33 beds each for judicially involved men, men, and women. MLK BHC collaborates closely with
 other Behavioral Health Center programs to ensure a comprehensive range of services for low-income
 and Medi-Cal-eligible individuals. Situated on the Martin Luther King Jr. Hospital campus in the
 Willowbrook area of South Los Angeles, the program serves residents of all Los Angeles County.
 
The SUD Counselor III is part of an interdisciplinary team that supports our clients through assessment of
 their needs and strengths in support of their health and recovery. This in-person role involves providing
 counseling in both individual and group settings, delivering curricula in accordance with program-specific
 requirements, and offering case management. The SUD Counselor III also assists clients in navigating
 systems of care while fostering a supportive treatment environment.
 
KEY RESPONSIBILITIES
 Treatment Setting

POSITION SUMMARY
The Behavioral Health Consultant (BHC) is responsible for the management of psycho-social aspects of chronic and acute diseases. The application of behavioral principles to address lifestyle and health risk issues. The Behavioral Health Consultant (BHC) will work to support the mission and vision of Neighborhood Healthcare by providing direct patient care for behavioral health services. This role will meet with individuals and families to provide brief behavioral psychotherapy services to patients and consultations primary care providers using an integrated care model. This is a hybrid position.

Responsibilities

Direct Patient Care
<ul>
<li>Provides short term targeted psychotherapy for problems identified by primary care providers</li>
<li>Performs initial and ongoing behavioral health assessment to develop patient care plans</li>
<li>Evaluates patients to identify social, emotional, and economic factors which may interfere with obtaining maximum benefits from medical care</li>
<li>Provides brief psychotherapy to patients and families</li>
<li>Addresses and factors co-occurring disorders into patient&rsquo;s care plan</li>
<li>Responds promptly and with caring actions to patients and employees</li>
<li>Provide crisis management and psychoeducation for patients and families</li>
<li>Conducts &ldquo;warm hand offs&rdquo; at medical provider requests</li>
<li>Acknowledges and honors patients&rsquo; psychosocial, spiritual, and cultural beliefs</li>
<li>Utilizes BHC staff protocols for referral, follow-up, and quality assurance</li>
</ul>
Provider &amp; Team Support
<ul>
<li>Consults with primary care providers about behavioral aspects of their patients&rsquo; medical treatment</li>
<li>Contributes to the success of the organization by participating in quality improvement activities</li>
<li>Maintains professional working relationships with all levels of staff, clients, and the public</li>
<li>Cooperates in accomplishing department goals and objectives</li>
<li>Participates in case conferences, trainings, and departmental or agency meetings, as directed</li>
</ul>
Regulatory Compliance
<ul>
<li>Follows BHC protocols, policies, and procedures</li>
<li>Provides personal information and participates in the credentialing process with managed care companies</li>
<li>Attends continuing education programs offered to upgrade professional skills and maintain licensing requirements</li>
<li>Documents legible and legally defensible medical records</li>
</ul>
Qualifications

Education/Experience
<ul>
<li>Education
<ul><li>Doctorate degree in psychology with a valid CA Psychology License, or</li>
<li>Master&rsquo;s degree in social work with a valid CA LCSW License, or</li>
<li>Master&rsquo;s degree in marriage and family therapy with a valid CA LMFT License, or</li>
<li>Master&rsquo;s in degree in counseling with a valid CA LPCC License</li>
</ul></li><li>Current Basic Life Support (BLS) certification though an American Heart Association (AHA)- Approved source is required upon hire and must be maintained as a condition of employment
<ul><li>AHA- approved courses include an in-person, hands-on skills check with a certified instructor using a manikin to demonstrate CPR and emergency response techniques</li>
<li>Online-only BLS courses without a live skills check do not meet this requirement</li>
</ul></li><li>Two year of pre/post licensure psychotherapy experience with children, adults, and/or adolescents required; child/adolescent experience required</li>
<li>Experience providing health psychotherapy services, such as smoking cessation, weight management, stress management, or work with chronically medically ill patients</li>
<li>Bilingual (English/Spanish) preferred</li>
</ul>
Additional Qualifications (Knowledge, Skills and Abilities)
<ul>
<li>Knowledgeable about and experience with displaying of normal signs of human development and ability to assess and provide age-appropriate care</li>
<li>Ability to work well in an interdisciplinary team within medical care.</li>
<li>Ability to operate one or more word-processing, spreadsheet, and/or electronic medical records systems</li>
<li>Ability to do some charting simultaneously while in session</li>
<li>Excellent verbal and written communication skills, including superior composition, typing, legible handwriting, and proofreading skills</li>
<li>Ability to interpret a variety of instructions in written, oral, diagram, or schedule form</li>
<li>Ability to successfully manage multiple tasks simultaneously</li>
<li>Excellent planning and organizational ability</li>
<li>Ability to work as part of a team as well as independently</li>
<li>Ability to work with highly confidential information in a professional and ethical manner</li>
</ul>
Physical Requirements
<ul>
<li>Ability to lift/carry 10 lbs/weight</li>
<li>Ability to stand for long periods of time</li>
</ul>
Salary Ranges
<ul>
<li>Psychologist: $113,600 - $172,800 per year, depending on experience.</li>
<li>LCSW/LPCC: $95,400 - $141,300 per year, depending on experience.</li> 
</ul>
Compensation Disclosure: The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate&rsquo;s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
