Job Summary</b>:</div><div>
The Case Manager is responsible for assessing the needs of unaccompanied children in care, developing Individual Service Plans, screening for human trafficking concerns, facilitating the safe and timely release or discharge of children and youth, and documenting the provision of services in the case files.</div><div></div><div><b>
Duties and Responsibilities:</b></div><ul><li>
Case Management services, as described below:</li><li>
Oversees all aspects of the UAC case</li><li>
Maintain case file documentation in accordance with ORR requirements.</li><li>
Maintain confidentiality when gathering, documenting, consulting or consolidating information necessary for the case management assessment and the plan of care.</li><li>
Maintain regular contact with each UC, UC&rsquo;s family when possible, and established service providers to communicate case planning and progress.</li><li>
Provides client services as needed, including but not limited to making medical appointments, finding resources, etc&hellip;</li><li>
Gather historical and current data relative to completion of a comprehensive case management assessment including, but not limited to, school records, medical records, mental health treatment, legal status and court involvement, social and family strengths and needs, support systems, and specific cultural issues.</li><li>
Presents case to federal stakeholders in weekly Case Staffing Meetings</li><li>
Participate in team meetings, staff in-service training and department meetings.</li><li>
Carries a case load of 8 clients, varying on program census.</li><li>
Completes Signification Incident Reports (SIRs), per federal policy requirements.</li><li>
Conducts internal chart reviews to monitor documentation compliance.
</li><li>Completes, timely and accurate timesheets and other personnel/administrative records as required.</li><li>
Other duties as assigned.</li><li><div>
Is aware of Morrison Compliance Program including Compliance Officer, Compliance Standards of Conduct, and Employee Compliance Hotline. Understands Compliance Standards of Conduct and signs attestation document. Completes all assigned compliance, HIPAA, client privacy, and fraud, waste, and abuse training. Knows how to report any client privacy or fraud, waste, or abuse concerns.</div></li><li>
Demonstrate a commitment to developing a thorough knowledge and application of the Sanctuary model and other organizational policies &amp; practices.</li><li>
Participate in staff development, in-services, and training related to workplace effectiveness; model professional behaviors and implement improvements to business practices with awareness of their impact in a trauma-informed behavioral healthcare organization.</li></ul><div>

Bi-lingual in Spanish and English, written, read and verbal. Must clear a language proficiency test for Spanish.</li><li>
Attained age 21 years or older before first day of employment in position (ORR)</li></ul><div>

<p>One Safe Place is seeking a full time Bilingual Advocate to provide direct services to victims of domestic violence. This is a grant-funded position and reports to the Director of Advocacy.</p><p><b>Responsibilities and Duties:</b></p><ul><li>Provides direct victims services including supportive guidance, advocacy, and in some cases mentoring to victims of domestic violence and their children.</li><li>Specific direct victim services include increase victim&rsquo;s knowledge of crime victims&rsquo; rights and increasing knowledge of community resources and services available to meet individual needs.</li><li>Advocate will assist with Crime Victim&rsquo;s Compensation, assist with Texas SAVNS, provide advocacy, crisis intervention and follow-up services to victims.</li><li>Ability to conduct presentations in an effort to provide education outreach to increase the knowledge of community resources and services and training to increase the knowledge of crime victims&rsquo; rights.</li><li>Provide direction and task supervision to interns and volunteers.</li><li>Attend and participate in High Risk Case Staffing, regular trainings and other meetings designated as necessary.</li><li>Work with other agencies both on and off site including, but not limited to, law enforcement, child protective services, local social service agencies, civil legal agencies, and county prosecutor&rsquo;s office.</li><li>Effectively input data into software programs for reporting purposes.</li><li>Work in a positive team/partnership environment.</li></ul><p><b>Qualifications:</b></p><ul><li><i><b>Bilingual in Spanish required</b></i></li><li>Bachelor&rsquo;s Degree in Social Work, Criminal Justice, Sociology, Psychology or related field from accredited university required.</li><li>Ability to pass a criminal background check.</li><li>Minimum 1-2 years&rsquo; experience working with domestic violence victims.</li><li>Applicant needs to have knowledge in domestic violence by understanding the causes, cycle of violence and impact on children who witness domestic violence.
