DEPARTMENT OVERVIEW</h2>
The Refugee &amp; Asylee programs at LFSRM help people uprooted by persecution and violence work towards self-sufficiency through essential resettlement services including case management, employment, housing, school registration, cultural orientation, and referrals to internal and external services for English language acquisition, health and mental health needs, employment, and economic development. LFSRM works with national and local organizations to provide the services and resources needed for newly arrived Refugees to achieve rapid self-sufficiency, integrate into their new communities, and thrive.<h2 class="jobSectionHeader">
JOB SUMMARY &amp; RESPONSIBILITIES</h2>
The Case Manager coordinates with other members of the team to assist clients with stabilization, promote self-sufficiency, and facilitate long-term integration. Through case management and resource referral, this position helps clients who have been impacted by complex traumatic experiences meet immediate basic needs and achieve short- and long-term goals. Services are delivered in a culturally responsive manner and often in another language.<h2 class="jobSectionHeader">

Manage case file compliance: Maintain organized, up-to-date physical case files and digital records in accordance with program, agency, and funder requirements.</li><li>
Ensure data accuracy and integrity: Consistently enter, update, and review client data in online systems to ensure records are complete, accurate, and audit ready.</li><li>
Meet documentation timelines: Complete all required documentation within established deadlines, balancing client services with timely data entry, quality and attention to detail.</li></ul>

EXAMPLE ACTIVITIES<ul><li>
Coordinates with pre-arrival team to prepare for new arrivals and ensure housing is available.
</li><li>Greets clients at the airport, transports to housing, conducts intake and assessment and initiates service plans.</li><li>
Serves as an advocate and resource for clients and provides equal treatment to all client groups in accordance with the agency's commitment to the dignity of all persons.
</li><li>Works collaboratively with other members of the team to assist clients with applications for public benefits, enrolling children in school, obtaining identity documents, and other services and referrals, as needed.</li><li>
Conducts individual and group education and coaching on important life skills including but not limited to accessing transportation, budgeting, and managing household finances.</li><li>
Maintains ongoing communication with clients to assess client progress and provide support as needed throughout the life of the case.</li><li>
Maintains client case notes and documents all client services in a timely and accurate manner, maintaining high quality case file documentation.</li></ul><h2 class="jobSectionHeader">
TRANSPORTATION</h2>
Must maintain a valid driver's license and carry personal auto liability insurance at the level of $100,000/300,000/100,000.<ul><li>
Frequent regional travel. Required to transport clients in personal vehicle.</li><li>
Travel to conduct assigned airport pickups, home visits, or other tasks outside of standard business hours, including evenings, is required.</li></ul><h2 class="jobSectionHeader">
REQUIRED CERTIFICATIONS</h2><ul><li>
Associates degree OR two (2) years of experience in a related field such as social work, human services, humanitarian, mental health, or other related field. Equivalent combination of education and experience accepted.</li></ul><h2 class="jobSectionHeader">

Must be able to fluently speak, read, translate and write in the following language(s):

About the Opportunity</h2>

Pair Team is building a team of deeply passionate individuals ready to change primary care operations for those who need it most. We are looking for a highly motivated full-time Lead Care Manager who is willing to think creatively and empathically to help our team change the way people access healthcare.

We seek a full-time Lead Care Manager to play a critical role in our whole-person, interdisciplinary care model, responsible for directly outreaching and engaging with individuals living with Serious Mental Illness/ Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs. We believe in the power of trust and relationships to successfully engage those who may have never received the kind of whole-health care that Pair Team can provide. 

This position primarily allows for remote work; however, it includes 2-3 times a month on-site visits in the community alongside a fellow PairMate. You can expect to engage in these in-person activities 2-3 times per month, close to your city, while the majority of your duties, approximately 90%, will be performed from your home

What You'll Do</h3>
<ul><li>Maintain ongoing caseload of individuals through the utilization of evidence based approaches to promote engagement and achievement of health goals</li>
<li>Use relationship-based strategies to support members with social support navigation, understanding that many may have lived personal experiences causing them to be initially hesitant or distrusting of the health care system</li>
<li>Conducts periodic telephonic and SMS outreach to ensure timely follow-up to members</li>
<li>Work with member to identify health/wellness goals and incorporate goals into Health Action Plan/Shared Care Plan</li>
<li>Supports nurse care manager, behavioral health care manager, nurse practitioner and Community Engagement Specialist with delegated tasks</li>
<li>Collaborates on care issues with Enhanced Care Management team by participating in systematic case reviews and consulting with nurse care manager, behavioral health care manager, and nurse practitioner before taking clinical actions</li>
<li>Consistently meet monthly encounter metrics to ensure compliance with health plan regulations</li>
<li>Identify and break down barriers ensuring individuals' continuation with the program</li>
<li>Assists individuals in securing connection to community supports by scheduling appointments, managing referrals, and ensuring timely follow-ups</li>
<li>Coordinate physical care management appointments through collaboration with external and internal providers</li>
<li>Utilize external and internal online platforms to collaborate with team members and carry out daily tasks</li></ul>
