This includes gathering and documenting patient information and history and conducting routine and ancillary patient testing. You&rsquo;ll play a vital role in the patients experience with OCLI and in helping our Dr&rsquo;s to diagnose and treat eye disorders.

WayPoint Center Coordinator, Various Locations (Bilingual - Spanish)

Community Hospice and Palliative Care

The Board and executive leadership of Community Hospice & Palliative Care formed Alivia Care Inc. in 2020 with the strategic intent to provide a structure in support of the expanded mission and vision of Community Hospice & Palliative Care and prepare it to respond to changes in care delivery and reimbursement for patients living with advanced or chronic illnesses. The structure of Alivia Care was also designed to support additional programs and services beyond hospice care that will fulfill its goal of serving more people in more places, in order to help them live better with advancing illness.

Under the direction of the Care Navigation Center Manager, the Care Navigation Coordinator provides support guiding incoming Community Hospice and Palliative Care, PACE and Home health referrals to the appropriate personnel or other healthcare resource.
<br>S/he completes the referral intake process via telephonic and non-telephonic communications, including but not limited to areas such as insurance verification, scheduling patient and personnel assessments, providing patient and affiliated family members with requested information, and conducting routine follow up check-ins with patients and referral sources ensuring seamless patient admissions to services.

Position Responsibilities

<li>Serves as primary contact for inbound/outbound customer service.</li>
<li>Provides customer support by phone, fax, or email, to internal and external customers.</li>
<li>Provides backup support for incoming calls from all lines of business for Alivia Care.</li>
<li>Appropriately documents phone calls, faxes, portals, medical records and e-mails.</li>
<li>Facilitates referrals with a high level of customer service and maximizing productivity.</li>
<li>Responds to various types of service inquiries including: hospice, palliative, PACE and home health services and follows up with potential patients/families in a polite and courteous manner.</li>
<li>Coordinates the initiation of Hospice services with peers in the Admissions Team, as well as Physicians, Community Agencies, Assisted Living Facilities, Long Term Care Facilities and hospitals, in addition to patients/families/caregivers.</li>
<li>Provides excellent customer service and maintains an amicable and professional working relationship with referral sources and partners.</li>
<li>Approaches every referral with the goal of setting an assessment on the same day, schedules assessment visits in a timely and cost-effective manner.</li>
<li>Inputs data into the appropriate Electronic Medical Record (EMR) and system. Also relays accurate referral information to appropriate personnel.</li>
<li>Takes ownership of all aspects for referrals and follows up on pending referrals to ensure speed to care.</li>
<li>Provides Explanation of Services to patients and families either over the phone or face to face.</li>
<li>Processes referrals with accuracy and in a timely manner.</li>
<li>Seeks direction from a licensed Nurse when handling complex patient cases that require nursing knowledge of hospice eligibility, including, but not limited to, patients transferring to Community Hospice from out of our service area.</li>

ABOUT THE CALL CENTER SCHEDULER</b></p>

Upperline Health is looking for a professional and engaging Call Center Scheduler to answer a high volume of incoming phone calls and schedule new &amp; existing appointments for all Upperline Health clinic locations.</p>

This position is remote based and available to candidates located in Alabama.</b></p><br>

WHAT YOU&rsquo;LL DO</b></p>
<ul>
<li>Answer, orient, and greet patients over the phone in an enthusiastic manner - 100% of work time spent is answering calls and scheduling appointments</li>
<li>Determine appropriate clinic location and schedule patients into Vision system accordingly</li>
<li>Obtain required demographic, insurance, and eligibility information and accurately document in Vision system to ensure proper filing of claims</li>
<li>Verify if the patient&rsquo;s insurance requires a pre-certification and begin the referral process</li>
<li>Maintain professional working relationship with all contacts, both internal and external</li>
<li>Provide timely responses to all verbal and written inquiries regarding patient accounts</li>
<li>Respond to voicemails and online requests within 1 hour of the patients&rsquo; messages</li>
<li>Exhibit a patient demeanor and strong customer service attitude at all times</li>
</ul>
<p><b>QUALIFICATIONS OF THE CALL CENTER SCHEDULER</b></p>
<ul>
<li>High School graduate and formal training from a vocational school or equivalent</li>
<li>1+ years&rsquo; experience in a similar call center or centralized scheduling setting</li>
<li>Strong written and verbal communication skills</li>
<li>Ability to multi-task and work under deadlines with urgency</li>
<li>Knowledge of general medical terminology and managed care functions</li>
<li>Working knowledge of electronic health records (EHR), Vision system, &amp; MS Office (word, outlook, teams</li>
