The Role

Patient Onboarding &amp; Chart Prep
<ul><li>Oversee AI-assisted onboarding workflows &mdash; welcome communications, intake forms, chart setup &mdash; and verify accuracy</li></ul>
<ul><li>Complete chart preparation in eCW: medications, allergies, history, vitals, care plan activation, and RPM device setup</li></ul>
<ul><li>Monitor device inventory and coordinate reorders; confirm RPM device linkage for new patients</li></ul>
What You Bring
<ul><li>Medical Assistant certification (CMA, RMA, or equivalent)</li></ul>
<ul><li>2+ years of MA experience in a clinical, telehealth, or care management setting</li></ul>
<ul><li>eClinicalWorks (eCW) proficiency &mdash; this is our system of record for all documentation, CCM time logging, and care plan management; prior experience is required</li></ul>
<ul><li>Working knowledge of CCM, RPM, or chronic disease management programs</li></ul>
<ul><li>Experience in cardiology, cardiac rehab, or a cardiac-focused telehealth setting is a strong plus</li></ul>
<ul><li>Familiarity with CCM/RPM CPT coding (99490, 99439, 99457, 99458, etc.) preferred</li></ul>
<ul><li>Bilingual a plus</li></ul>
