What would you do in this role?
The role serves as the central coordinator for surgical patients, guiding them through all phases of care while ensuring they receive clear education, timely instructions, and consistent support. Responsibilities include managing and scheduling all procedures within the assigned specialty, securing required imaging, diagnostics, and medical clearances, and overseeing ultrasound surveillance workflows. The position tracks outstanding tests to prevent delays, coordinates dialysis‑related needs for vascular access patients, and works closely with prior authorization and coding teams to secure approvals and streamline billing workflows. It requires frequent communication with surgeons, APPs, OR staff, hospitals, dialysis units, and imaging partners to align care. Additional duties include assisting with referral triage, documenting all interactions in the EMR, identifying workflow gaps, maintaining efficient clinic flow, monitoring physician schedules, and analyzing productivity and quality metrics. The role also supports the development of new clinical programs and fosters strong communication and collaboration across the organization. 
A Day in the Life<ul><li>Serve as the primary point of contact for surgery patients in the assigned specialty, providing education, instructions, and support through their pre-, peri- and post-operative care;</li><li>Coordinate all procedures and surgeries related to assigned specialty, ensuring required imaging, testing, and medical clearances are completed;</li><li>Schedule surgeries and procedures according to surgeon protocols and facility availability;</li><li>Coordinate ultrasound surveillance workflows, including AAA surveillance tracking and follow-ups;</li><li>Track outstanding diagnostics (ultrasounds, CTAs, ABIs, labs) and follow up to prevent delays in care;</li><li>Coordinate dialysis-related needs for vascular access patients, including communication with dialysis centers and surgical planning, if assigned to the vascular specialty;</li><li>Work closely with prior authorization and coding support staff to ensure timely approvals;</li><li>Collaborate with designated team members to align workflows for authorizations and billing support;</li><li>Communicate with surgeons, APPs, OR staff, hospitals, dialysis units, and imaging partners, etc. to coordinate patients&rsquo; care;</li><li>Support the screening and triage of incoming referrals in collaboration with providers to ensure timely evaluation;</li><li>Document all patient interactions, requirements, and scheduling details in the EMR;</li><li>Assist patients in navigating barriers, understanding next steps, and preparing for surgery;</li><li>Assist in identifying workflow gaps electronic health records system issues and opportunities for process improvement;</li><li>Maintain efficient clinic flow and cover work as needed;</li><li>Monitor physicians&rsquo; schedules to confirm accuracy in patient scheduling and plan staffing needs accordingly;</li><li>Analyze productivity, quality of patient care and adherence to safety regulations of area and provide recommendations as needed;</li><li>Support vascular physicians in planning, implementing and coordinating new clinical programs and patient care initiatives.</li><li>Use Electronic Health Records to pull reports and review data;</li><li>Develop and maintain effective communication within the organization;</li><li>Collaborate with other administrative level staff within the organization.</li></ul>

o One (1) year of experience working in a medical office to gain understanding of medical terminology and workflows;
o Six (6) months&rsquo; experience working in a back-office position in a medical setting (medical assisting, nursing assisting, casting, etc.);
o Six (6) months&rsquo; experience scheduling complex medical procedures (such as surgeries, MRI images, etc.). 
Preferred Qualifications<ul><li>Knowledge of medical terminology specific to vascular surgery.

OVERVIEW OF PRIMARY RESPONSIBILITES: 
The position of Care Navigator (CN) is responsible for working closely with individuals to promote effective connection and collaboration to services. Care Navigators are committed to assisting in removing the barriers to care by identifying critical resources, navigating through health care services, insurance plans, and systems. Care Navigators help individuals "navigate" the maze of insurances, administrative systems and support services. They focus on integrating services around the needs of the individual.
 Effective Care Navigators build working relationships, solve problems, direct to resources, and manage information.
 
PRIMARY FUNCTIONS: 
Primary functions include coordination to improve quality of care through the efficient use of resources and thereby enhancing quality, cost-effective outcomes.
 Core Responsibilities
 <ul>
<li>Act as an advocate and assist in coordination of care to minimize the fragmentation of health care delivery systems.</li>
<li>Gather insurance information.</li>
<li>Assist in navigating the complex healthcare and insurance systems.</li>
<li>Assist in securing health insurance.</li>
<li>Assist with completion of paperwork for the sliding scale fee.</li>
<li>Complete referrals to care management services and other internal and external services as needed.</li>
<li>For referrals to care management (internal) follow up with the individual until a warm hand-off to care management is complete.</li>
<li>Identify and effectively utilize community resources to meet the needs of the member/family. Facilitate access to community resources.</li>
<li>Care across the healthcare continuum and optimize clinical and financial outcomes.</li>
<li>Maintain a working knowledge of payer requirements. Negotiate on behalf of the member for cost-effective, high quality services and to maximize the efficient use of resources.</li>
<li>Serve as a liaison to providers, members and families for coordination of services.</li>
<li>Document all interventions using the Complex Care Management billable documentation.</li>
<li>Work collaboratively as part of a team.</li>

Community Engagement and Outreach

<li>Routinely engage with members and potential members in community settings, which may include homes, shelters, community centers, and other public locations, to address barriers to care and directly facilitate service access.</li>
<li>Conduct community-based resource identification and networking to establish and maintain strong partnerships with local organizations.</li>
<li>Represent the organization at community health fairs, meetings, and events to promote services and identify individuals needing care navigation support.</li>
<li>Provide safe and reliable transportation for clients to and from appointments, resource centers, and other essential services as required for care coordination.</li>

<li>NYS licensure and registration as a Registered Nurse and a bachelor's degree or</li>
<li>Bachelor's level education or higher in any field with five years of experience working directly with highly vulnerable populations (i.e. those with health, behavioral health, substance use issues), or a Credentialed Alcoholism and Substance Abuse Counselor (CASAC) or equivalent experience in behavioral health, human services or other.</li>

This role requires flexibility for occasional shift changes and availability to work holidays that coincide with scheduled shifts.

The required schedule for this role is Saturday-Wednesday: 11am-7pm EST, or the equivalent in your respective time zone (i.e., 10pm-6pm CST).

The target base hourly range for this position is $33.33 - $42.31 hourly, and is part of a competitive total rewards package including stock options and benefits. The Respiratory Care Practitioner (RCP) Navigator plays a vital role in coordinating the safe and efficient transition of pediatric patients with respiratory needs from hospital to home or other care environments. Working across interdisciplinary teams, the RCP Navigator ensures appropriate equipment is provided, families are fully educated and confident in managing care, and respiratory plans are aligned with the patient's clinical and palliative goals. 
With deep expertise in pediatric respiratory therapies and durable medical equipment (DME), the RCP Navigator will serve as a clinical bridge between providers, case management, bedside care, and families. This position also supports the hospital's palliative care model by helping patients and families navigate complex care needs with compassion, clinical precision, and clear communication.
Qualifications: 
Education 
<ul><li>
Associates Degree Respirator Care (required)</li><li>
Bachelors Degree Health Related Field (preferred)</li></ul> 
Licenses and Certifications<ul><li>
RCP - Respiratory Care Practitioner License - RCB - Respiratory Care Board of California (required)</li><li>
RRT - Registered Respiratory Therapist - NBRC - National Board for Respiratory Care (required)</li></ul> 
Work Experience<ul><li>
Minimum three (3) years Experience in pediatric acute care (preferred)</li></ul> 
Skills and Abilities<ul><li>
Strong understanding of respiratory-related DME</li><li>
Demonstrated ability to educate families and caregivers effectively</li><li>
Exceptional interpersonal and communication skills</li><li>
Familiarity with palliative care approaches is a plus</li></ul>

Sun River Health is seeking a Full-time Patient Navigator to support our Washington St. - Poughkeepsie, NY site.
The Patient Navigator identifies individuals and/or members of their families within the target population who are in need of preventive and primary health care services and links them to the services they need. Includes care for: acute; chronic and communicable diseases; dental services; prenatal care and family planning; WIC and other nutrition services; pediatric care and immunizations. Trains and mentors the Patient Representatives Perform other duties as assigned on as needed basis.
Essential Duties and Responsibilities
<ul><li>Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance and application assistance.
</li><li>Facilitates patient appointments within different departments of SRH and outside agencies. Follows SRH procedures for referring patients to different departments of SRH and outside agencies, utilizing SRH preferred provider lists.
</li><li>Assist patient in overcoming obstacles that would prevent them from receiving needed referrals.
</li><li>Responsible for facilitating patients to appropriate referred agencies, with specific focus on referrals to specialty care and public health insurance programs, as needed and appropriate; track progress in eCW and alert providers.
</li><li>Answer patient questions to clarify provider instructions, procedures and referral needs.
</li><li>Responsible for transmitting all necessary clinical information available to specialist for referral; obtains consultant reports; obtains consent for release of information.
</li><li>Review open referrals for upcoming medical appointments and provide to clinical team.
</li></ul>Minimum Education Requirement: High school diploma/ GED
 
Minimum Work-Related Experience: 0-2 years
Bilingual Spanish is preferred
