JOB SUMMARY:
An child care provider provides care for children 6 weeks through 4 years of age under the supervision of the Child Care Center Director. The child care provider will help introduce and educate new and basic concepts to children by reading to, talking to, playing with, and nurturing them. The child care provider must be able to communicate well with the parents, which will include keeping daily records on the child.
GENERAL REQUIREMENTS:
Operate the classroom in compliance with all child care licensing standards, and Employee and Policy Handbooks. Must have a warm and caring attitude toward working with children and their families. Must be dependable and honest. Must be flexible and adapt to change throughout the program. Must be willing to accept supervision in order to improve work performance. Must be physically able to work with young children.
Requirements:

EDUCATION/EXPERIENCE:
High School Diploma or GED. Must be 18 years of age. Must have minimum of one year work experience working with children and meet additional training requirements.

MINIMUM QUALIFICATIONS: NP/PA

ESSENTIAL DUTIES AND RESPONSIBILITIES: 


Essential Clinical and Compliance Duties:
<ul>
<li>Work as a member of the primary care team to deliver comprehensive, patient-centered services.</li>
<li>Take appropriate health histories, perform physical exams, review vitals, labs, other studies, prescribe medications, perform procedures, review outside medical records and other forms of treatment as necessary and appropriate to the standard of care being provided in the setting for both acute and chronic health concerns.</li>
<li>Triage, diagnose, manage minor and/or acute illnesses and initiate treatment.</li>
<li>Along with the care team, support patients in achieving health resiliency and engagement by utilizing techniques such as shared decision making, motivational interviewing, continuity relationship building, health education, and preventive care appropriate to patient characteristics.</li>
<li>Timely completion of appropriate medical records.</li>
<li>Attend all mandatory CCC training in a timely manner.</li>
<li>Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners&rsquo; and outside providers&rsquo; patient confidentiality agreements, including privacy and security requirements for EMR access.</li>
<li>This position will provide after-hours phone coverage on scheduled rotation with colleagues. Calls will be addressed first by contracted nurse triage service, LMPs will be the second call.</li>
</ul>
Collaborative Care:
<ul>
<li>Support pharmacy-based collaborative drug therapy management programs and provide medical visits to support clients receiving treatment.</li>
<li>Coordinate with other CCC programs supporting patients in a continuum of services such as Hepatitis C treatment, withdrawal management, residential treatment for substance use disorders, specialty mental health, and overdose prevention programs</li>
</ul>
Quality Improvement and Practice-Based Learning:
<ul>
<li>Identify opportunities for improvement in medical workflows and logistics. Participate in review and collection of performance and quality Indicators; review and contribute to continuous improvement opportunities and plans.</li>
<li>Participate in and, at times, lead provider education sessions on a variety of clinical topics, including helping train and onboard other medical providers and staff.</li>
<li>Participate in compliance, quality assurance, and quality improvement activities as directed.</li>
<li>Collaborate with the care team to ensure optimal performance on team-based performance metrics.</li>
<li>Perform other duties as assigned, including work at other CCC clinical sites and/or off-site at community events.</li></ul>
