<div>POSITION SUMMARY:
Work directly with the Supervisor and Manager to provide additional resources for the case management staff. This position does not provide any clinical review or decisions.
This individual is responsible for monitoring incoming correspondence received. The coordinator will ensure the referrals are set up in the case management system and promptly assigned to the case manager. The Referral Coordinator will answer incoming calls, screen, respond and route the calls. This position requires the ability to work independently to meet deadlines, as well as an excellent phone demeanor and a desire to provide exceptional customer service. The position requires knowledge of medical terminology, the understanding of referral criteria by client and/or jurisdictions. The referrals may be for WC Utilization Review and or Case Management services.
Responsibilities of the position include, but are not limited to:<ul><li>

<div>The Case Management Assistant in Bell&rsquo;s Next-Door program is the first contact for potential program participants and as such must always provide excellent customer service. The Assistant must have or develop an understanding of community resources available to assist individuals and families in need and direct people as appropriate to those services. Summary
<ul><li>The Case Management assistant provides daily support to the Clinical resource Management Department in the process of quality and utilization data necessary for the day-to-day operations of the dept.</li><li>
Facilitates appropriate follow up in administering The Centers for Medicare &amp; Medicaid Services forms (Medicare outpatient Observation notices and Important Message forms to enhance the quality of the services being provided to our patients across the system.</li></ul> 
 
Responsibilities
<ul><li>Provides the Centers for Medicare &amp; Medicaid Services CMS forms (MOONS, IMs) to patients in a safe, timely, and effective manner.</li><li>
Verifies and assures that documentation entered within the patients' medical records are entered accurately and kept confidential.</li><li>
Responds and verifies inquiries from CRM leadership team and/or responsible parties. Interacts with patients to keep track of their progress and to ensure satisfaction.</li><li>
Adhere to professional standards as outlined by protocols, rules and regulations.</li><li>
Develops effective working relationships with CRM team.</li><li>
Facilitates multiple care aspects as requested by CRM leadership team for DC planning (case coordination, information sharing, etc).</li><li>
Under direction of CRM leadership, assist with the daily referrals, per standard protocols, to Primary Care Physician and JHS specialty clinics, scheduling diagnostic studies and other clinics appointment and procedures.</li><li>
Coordinates with outpatient studies as needed.</li><li>

Performs all other related job duties as assigned.</li><li>

Experience

Education
