DEPARTMENT/BRANCH PROFILE
Case Management provides services to complex or high-risk patients receiving care at Sidra. Although case management is often initiated during the acute inpatient care phase, services are also provided in the outpatient settings to connect Sidra specialized clinics with primary care providers in the community. The case management team works collaboratively with the broader health care team to minimize inpatient length of stay by coordinating patient care across the continuum for patients and families with complex care transition needs. Clinical research focused on education, quality improvements and a culture of patient and family safety will be used to inform and guide best practice standards to assure safe, cost-effective and quality outcomes and contribute to Sidra&rsquo;s reputation as a high reliability organization.

 As part of Strive&rsquo;s interdisciplinary clinical care team, the Nurse Practitioner (NP) will utilize their advanced education, skills, and judgment to deliver advanced nursing care for CKD and ESKD patients. The NP will be responsible for managing patient care by working with the Strive interdisciplinary team of social workers, care coordinators, pharmacists, dietitians, and physicians to provide best in class patient care. The Strive NP will be an advocate, navigator, trusted clinician, and safety net for the patient while delivering exceptional clinical care, building empathy and trust with patients, and delivering a world class patient experience. This position reports to the Senior Manager, Nurse Practitioner.
 
 
 
 The Day To Day

 Accountable for patient outcomes and KPI&rsquo;s for a panel of CKD and ESKD patients. Collaborates with external providers to ensure goals, treatment, and care plan alignment.

 Obtains patient history, performs physical exam, orders and interprets diagnostic tests and formulates a plan for individual patient short-term and longitudinal needs.

 Presents patient cases and provides clinical support for clinical rounds and interdisciplinary team meetings.

 Ensures assessment and plan of care incorporate best practices for chronic kidney care including CKD clinical care (management of all stages of CKD, co-morbidities, ESKD and support of transitions to renal replacement therapy, transplant, or conservative care) as well as participation in shared decision-making and end-of-life/advanced care planning discussions.

 The Recruiting RN is generally responsible for the pre-employment physical review and processing for required positions.

 Performs physical reviews in a timely and professional manner

 Provides oversight of TB and Fit Testing
 </li>

 Assists in triaging and managing work-related injuries and illness with the Case Management Team

 Job Functions
 
 
 
 <ul>
 <li>
 Assesses physicals and provides recommendations on the hiring of new employees
 </li>
 <li>
 Clarifies and follows up with clinicians after reviewing physicals
 </li>
 <li>
 Obtains clearance from treating provider when warranted
 </li>
 <li>
 Performs complete and professional assessments in accordance with Lincare policies and procedures
 </li>
 <li>
 Maintains and establishes relationships with various Occupational Health Clinics throughout the US
 </li>
 <li>
 Follows applicable policies and procedures of the Safety &amp; Regulatory Manual regarding General Safety, OSHA (including the proper use of Personal Protective Equipment), DOT (driving safety), FDA, and Emergency Planning
 </li>
 <li>
 Facilitates and utilizes clinical portals when available
 </li>
 <li>
 Collaborates with multiple departments to streamline hiring process
 </li>
 <li>
 Maintains employee pre-employment physicals in complete and organized manner in compliance with accreditation standards
 </li>
 <li>
 Assists management with work related injuries
 </li>
 <li>
 Ability to remotely triage workers following an injury
 </li>
 <li>
 Evaluates all injuries and illnesses in an efficient and professional manner within the scope of practice and consistent with clinical nursing guidelines
 </li>
 <li>
 Ability to communicate treatment options with workers based on the severity of the injury
 </li>
 <li>
 Assists departments with identifying, evaluating, implementing, accommodating, and return-to-work options for workers with restricted duty
 </li>
 <li>
 Coordinates case management of work-related injuries by managing all cases to facilitate productivity, ensuring appropriate care and return to work
 </li>
 <li>
 Works low volume on-call evenings and weekends on an as scheduled basis
 </li>
 <ul>
 <li>
 Frequency is determined by the number of employees available to share in the responsibility
 </li>
 <li>
 Must respond in a timely manner
 </li>
