The Case Management Assistant is responsible for providing accurate and timely administrative and technical support to the case management teams, helping ensure a positive patient experience and effective coordination of care and utilization review processes. Responsible for assisting in the discharge planning and utilization review process for all patients. Responsible for accurately entering patient data and documentation into records systems while interacting with doctors, nurses, insurance agencies, and patients to bring all aspects of a patient&rsquo;s care together and to match identified patient needs to the professionals who can meet them. <p><b>Job Summary</b><br/>Provides guidance and support to TLC reentrants, assists Case Manager, Program Director, and Executive Director as needed, maintains security facility, performs office and clerical duties, and assures compliance with TLC and DOC policies and with ACA Accreditation Standards.</p><p><b>Responsibilities</b></p><ul><li>Directly oversee resident behavior and maintain facility security.</li><li>Obtain relevant information by reviewing previous shift log reports and Activity Log book.</li><li>Update information and aid communication through daily shift reports and emails.</li><li>Utilize basic functions of computer programs.</li><li>Maintain inner cohesiveness of facility through communication between staff and following directives of case managers and supervisors.</li><li>Work cooperatively with Case Managers, Business Manager, Program Director, and Executive Director.</li><li>Maintain facility process through performance of clerical duties.</li><li>Maintain integrity of facility.</li><li>Enforce and promote appropriate boundaries between reentrants and staff.</li><li>Maintain cleanliness of facility through observation conducting during walk throughs, enforcing completion of details, etc.</li><li>Promote organization and efficiency through oversight of resident work schedules, school schedules, appointments, counseling, etc.</li><li>Conduct resident procedures upon releases</li><li>Comply with state mandated standards and DOC policies.</li></ul><p><b>Qualifications</b></p><ul><li>High school diploma or GED required</li><li>Degree in Criminal Justice/Human Services, preferred</li><li>Driver's License, preferred</li><li>Knowledge of computer and software programs</li><li>Skilled in oral and written communication</li><li>Observing confidentiality</li><li>Handling emergency situations</li><li>Ability to life up to 10lbs occasionally</li><li>Climb three flights of stairs each hour</li></ul><p>Job Type: Part-time</p><p>Pay: $15.00 - $15.50 per hour</p><p>Benefits:</p><ul><li>403(b)</li><li>Employee assistance program</li><li>Paid orientation</li></ul><p>Work Location: In person</p>
