The Treatment Coordinator role is a key position to ensure that evaluation, treatment planning, support services, collaboration, and discharge processes comply with OMHC standards, including evaluative services, treatment planning and documentation, support services, and discharge/continuity-of-care expectations. POSITION SUMMARY

The Treatment Coordinator is a licensed behavioral health professional responsible for:
<ul>
<li>Providing structured, weekly, face-to-face clinical sessions to an assigned caseload of approximately 20 clients</li>
<li>Leading and coordinating all aspects of those clients&rsquo; treatment, including referrals, collaboration, and care transitions</li>
<li>Conducting structured weekly audits of treatment and documentation for assigned clients and their treatment teams</li>

This is a full-time, in-office position with no remote work. The schedule is structured to support a consistent service model:

<li>Morning: See 4 of the 20 assigned clients each day (rotating so all are seen weekly) in 45-minute face-to-face sessions. These sessions collectively make up 15 hours per week of direct client intervention.</li>
<li>Afternoon: Provide care coordination, documentation, treatment planning, team leadership, and follow-up for the clients seen that morning and others on the caseload, totaling 25 hours per week of coordination and administrative clinical work.</li>

ESSENTIAL DUTIES &amp; RESPONSIBILITIES

1. Clinical Evaluation &amp; Assessment

<li>Complete, or ensure completion of, an initial face-to-face diagnostic evaluation and related documentation for each assigned client in accordance with OMHC policy and within required timelines (no later than the second visit).</li>
<li>Formulate and document: presenting problem(s), relevant psychosocial and family history, mental status, risk factors, strengths, functional impairments, and current needs.</li>
<li>When clinically indicated and with proper consent, maintain and document ongoing collaboration with the individual&rsquo;s primary care provider or other somatic providers to support integrated care. (MdRules)</li>
<li>Update evaluative information as clinical status changes or at required review intervals.</li></ul> 


2. Treatment Planning &amp; Documentation

<li>Ensure that an Initial Individual Treatment Plan (ITP) is developed no later than the fifth visit following enrollment, using the assessment as the foundation.</li>
<li>Develop ITPs in collaboration with the client, and when applicable, the client&rsquo;s parent/guardian/caregiver, and other members of the treatment team.</li>
<li>Ensure that each ITP includes at minimum:
<ul><li>Current diagnosis</li>
<li>Presenting needs, strengths, recovery goals, and treatment expectations</li>
<li>Specific services and interventions (modality, frequency, duration, and responsible staff)</li>
<li>How services will help the individual manage symptoms and support recovery</li>

3. Direct Client Intervention (15 Hours per Week)
<ul>
<li>Maintain a caseload of approximately 20 clients at any given time.</li>
<li>Follow the service model of seeing 4 clients each morning (Monday&ndash;Friday) for 45-minute face-to-face sessions, such that each client is seen weekly, barring cancellations or clinically indicated frequency changes.</li>
<li>During sessions:
<ul><li>Review treatment progress and the client&rsquo;s treatment experience</li>
<li>Address clinical and practical barriers to engagement</li>
<li>Intervene to support treatment compliance, including motivational enhancement, skill-building, and problem-solving</li>
<li>Educate clients about risks and implications of going against medical or clinical advice (AMA), and document such education</li>
<li>Begin or support safety planning when indicated</li>

4. Treatment Coordination &amp; Referral Management (25 Hours per Week)

Afternoon work is dedicated to coordination, oversight, and administrative clinical duties for clients on the caseload, especially those seen that day.

<li>Coordinate all aspects of services as described in the ITP, ensuring that services delivered match what is authorized, clinically indicated, and documented.</li>
<li>Communicate regularly with psychiatrists, nurse practitioners, therapists, PRP staff, case managers, and other providers on the treatment team.</li>
<li>Issue and track referrals to internal and external resources, within scope, such as:
<ul><li>Psychiatric rehabilitation / PRP services</li>
<li>Substance use/addictions treatment</li>

6. Intake, Orientation &amp; Discharge
<ul>
<li>Participate in or conduct intake evaluations for clients who will be part of the Treatment Coordinator&rsquo;s caseload, including orientation to services and review of rights and expectations.</li>
<li>Participate in discharge planning and continuity-of-care processes in accordance with COMAR expectations, including ensuring a documented discharge summary within required timelines that includes reason for admission, reason for discharge, services provided, progress made, diagnosis at discharge, current medications, and follow-up recommendations. <li>Promote smooth transitions to higher or lower levels of care, or to other providers/agencies, when medically necessary.</li></ul> 


7. Team Leadership &amp; Staff Support
<ul>
<li>Serve as the Treatment Team Lead for assigned clients.</li>
<li>Provide day-to-day clinical direction, coaching, and feedback to team members delivering services to those clients.</li>
<li>Identify and address performance issues related to clinical documentation, timeliness, or adherence to the ITP and COMAR standards.</li>
<li>Recognize and affirm staff accomplishments and effective care.</li>
<li>Promote a collaborative, recovery-oriented, and culturally responsive team culture.</li></ul> 


8. Meetings &amp; Supervision
<ul>
<li>Participate in weekly clinical supervision when required by license and agency policy.</li>
<li>Meet weekly with the Operations Manager for caseload review, program updates, and operational coordination.</li>
<li>Meet weekly with a Human Resources representative to support staffing, training, and compliance matters as they relate to assigned clients and staff.</li>
<li>Meet weekly with a Finance representative to review billing, productivity, authorization, and payer requirements as they relate to caseload and service delivery.</li>

EXPERIENCE &amp; SKILLS

General Summary:
Under general supervision independently works with individuals in the community who need case management services in developing, securing, and maintaining basic life needs and mental health treatment. Provides an array of services to assist individuals through assessment, service planning and delivery, and referral or linkage to other community resources. Interacts with all individuals in a respectful, dignified, therapeutic, adult, engaging, and culturally sensitive manner using identified counseling and communication skills. Conducts outreach with individuals off-site and at locations convenient to individuals needs. Schedules and arranges off-site services while remaining aware of various community safety issues. 
Principal Responsibilities and Tasks:
The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.<ul><li>
Makes initial contact/outreach with individuals referred/assigned. Conducts an assessment for case management services, completion of admission paperwork, and the initial development of a service plan involving needs, strengths, and goals.</li><li>
Independently develops individualized support plan and coordinates/implements services.</li><li>
Within the first 90 days must make a home visit to the individual&rsquo;s home to evaluate the living conditions. If the individual is homeless, must meet the individual in a community setting of individuals choosing.</li><li>
Performs interventions that assist the individual in meeting short and long term goals. This involves transportation of the individual to community resource appointments, ongoing coordination of service/acquisition of resources, work with the family and other service providers as needed as well as outreach provision these services involve.</li><li>
During the course of provision of services the service plan will be reviewed and updated according to MHP and the Public Mental Health System requirements and in compliance with State regulations.</li><li>
Works with individuals to develop budgets and assist with management of these through the representative payee system.</li><li>
Attends regular and emergency team meetings. Actively participates in team meetings to ensure efficient and effective service provision.
</li><li>Maintains a highly flexible schedule to respond to individual needs. Exercises considerable independent judgement in determining the appropriate response to emergency needs of individuals.</li><li>
Independently encounters a broad range of clinical/social/family dynamics and situations, many of an unpredictable nature. Secures support and presence of co-workers as needed when encountering suspect situations.</li><li>
Collaborates with other staff team members. Assists other agency teams and team members as needed.</li><li>
Provides crisis intervention and oversees the resolution of crisis. This may involve doing an emergency petition, working more closely with clinical treatment staff, transporting a patient to emergency mental health services</li><li>
Responsible for meeting billing/service goals, ensuring accurate documentation in compliance with state regulations and PMHS/MHP requirements, following policies and procedures, and submitting on schedule. This includes but not limited to the ongoing development of service plans, annual assessments, progress notes for each contact and monthly summary notes</li><li>
Attends all required and relevant training to remain current in best practices, including those required by state regulations and offered by BMHS or the Mental Hygiene Administration.</li></ul>
Knowledge, Skills and Abilities:<ul><li>
Knowledge of community resources is essential. Knowledge of relevant criteria and application process is desirable.</li><li>
Must have demonstrated interest in community outreach.</li><li>
Effective oral and written communication and counseling skills (i.e. active listening, building rapport, respecting boundaries, etc.) are necessary in order to establish and maintain effective working relationships with members, staff members and the general public.
</li><li>Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient populations(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.</li><li>
Must be computer literate.</li></ul>
Patient Safety:
Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.

Reports adverse events and near misses to appropriate management authority.</li><li>

Education and Experience:<ul><li>
Bachelor&rsquo;s Degree in social work or a related field is required.</li><li>
One (1) year professional experience performing mental health services is required.</li><li>
Experience with those having serious and persistent mental illness preferred.</li><li>
Current, valid, MD state driver&rsquo;s license is required.
</li><li>Use of employee&rsquo;s own personal vehicle is required.</li></ul>
