Requirements:<br>

Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted;</li><li>
May also correlate clinical data for the business office as required.</li><li>
Providing case management and care coordination services for patients in the clinic;</li><li>
Responsible for coordinating the continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;</li><li>
May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;</li><li>
Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team&rsquo;s relationship within the facility or treatment milieu; and,</li><li>
Perform other duties as assigned.</li></ul><div></div><div><br>
Compensation for roles at Fashion Valley CTC varies depending on a wide array of factors including but not limited to the specific location, role, skill set, and level of experience. As required by local law, Fashion Valley CTC provides a reasonable hourly range of compensation for roles that may be hired in California as set forth below.<br>

Qualifications:
<p><b>
Your Skills and Qualifications as a Case Manager:</b></p><ul><li>
Bachelor&rsquo;s degree in Social Work, Counseling or related Human Service field.</li><li>
1-2 years of experience working in behavioral health setting. Experience with the population served by the clinic preferred.</li><li>
Previous experience in case management and/or utilization review preferred.</li></ul><div><b>
Licenses/Certifications:</b></div><ul><li>
AOD Registration is required.</li><li>
AOD Certification is preferred.</li><li>
Registration/Certification must be obtained from a California-approved Counselor Certification Organization (CAADE, CADTP or CCAPP).</li></ul><div></div><div><i><br>
