JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) provides intensive case management services to eligible clients, meeting participants at least weekly. The Case Manager is the primary point of contact to coordinate services and care for assigned clients. Each Case Manager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available.
DUTIES AND RESPONSIBILITIES
<ul><li>Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan.</li>
<li>Provide service linkages and support systems to ensure identified needs are met.</li>
<li>Coordinates individualized planning with clients to meet short- and long-term needs.</li>
<li>Coordinates case conferences with residents, consults with service team, monitors progress of clients on case plan, assists clients to meet the agreed upon goals.</li>
<li>Acts as advocate for client before judicial, community, social service, and administrative bodies as needed;</li>

<div>Anne Arundel County Government is looking to fill a Full-Time, Permanent, Emergency Essential Community Case Manager (Addiction Specialist) position in the Department of Health. 
 
The CCM Case Manager is responsible for professional work in coordinating and conducting a substance abuse community intervention program through the Anne Arundel County Health Department. 
 
The CCM Case Manager provides comprehensive case management services to clients enrolled in the program. These services are tailored to meet the individual needs of clients while complying with the requirements of the Drug Treatment Court Program. The primary goal is to help clients overcome their drug and alcohol use and to encourage them to develop a more productive lifestyle. 
 
Case managers assist individuals with complex substance use disorder (SUD) needs by addressing various aspects of their lives, including medical, mental health, family, employment, criminal justice, housing, and financial issues. These areas often pose barriers to achieving success in the recovery process. This position will facilitate care and streamline transitions with other entities within the county healthcare system, ensuring that participants receive appropriate ongoing care. Responsibilities also include helping individuals access public benefits and other supportive services. The case manager is required to complete all necessary documentation for the CCM Program.

JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) &ndash; Problem Solving Specialist provides intensive housing crisis response and case management services to eligible clients, meeting participants at least weekly. Using active listening techniques, the Case Manager &ndash; Problem Solving Specialist is the primary point of contact to coordinate creative resolutions to a housing crisis &ndash; locating both temporary and permanent housing. Located at a VOALA shelter, and working is the field as needed, the Case Manager (Specialist) &ndash; Problem Solving Specialist focuses on resolving the housing crisis, but can refer to other resources as needed.
DUTIES AND RESPONSIBILITIES
<ul><li>As a member of the program case management team, provides information referrals, crisis intervention, and assistance with permanent housing placement for participants</li>
<li>Assists case managers in conducting the culturally appropriate, comprehensive screenings and assessments to collect functional, environmental, financial, employment, housing, educational, and health information, as appropriate, to develop a Housing Stability Plan</li>

About the Role &ndash; Who You&rsquo;ll Work With
The Case Specialist Lead will work closely with the Case Specialist Senior Lead, Lead Project Director, Assistant Project Director, ORR federal field specialist, data analysts, other workers/staff, or stakeholders as applicable and in collaboration with the U.S. Department of Health &amp; Human Services (HHS), Office of Refugee Resettlement&rsquo;s (ORR) Reunification Management Unit and its care providers.
What You&rsquo;ll Do<ul><li>
Provide oversight and execution of daily operations to ensure work is completed on schedule, within budget and to specified quality standards to assigned Case Specialists (CS).</li><li>
Review all documentation submitted by CS to ensure they are aligned with policy and procedures for safe and efficient reunification.</li><li>
Regularly review documents submitted by CS and uploaded to government case management tracking system (UC Portal).</li><li>
Promote productivity and ensure staff meet their performance standards and metrics/goals.</li><li>
Hold daily sessions with CS team to check on caseload progress, discuss any policy changes from the Office of Refugee Resettlement (ORR), and answer questions.</li><li>

Work Schedule<ul><li>

<div>The Case Management Specialist works with providers, patients, and insurance companies to help ensure patients receive the highest level of care, assisting them with their chronic care and preventative care needs to help reduce long-term health concerns and hospitalizations. They educate patients on the benefits of recommended preventative care services and build rapport with them to improve patient engagement, self-care management, and adherence to their individualized plan of care. They track patients&rsquo; chronic care needs and work with teams within the organization to identify barriers for patients accessing care and link them to community resources. They act as a liaison, collaborating with our community partners and other healthcare organizations to ensure continuity of care and care coordination activity within the EMR. 
<ul><li>
LPN/RN license with clinical experience required, or Medical Assistant (MA) certification with a minimum of five years clinical experience in a medical setting.</li><li>
Clinical experience with an understanding of chronic disease management and knowledge of all life cycles required.</li><li>
Knowledge of Microsoft Excel required.</li><li>

Ensure all required elements are documented for CCM and related Annual Wellness Visits (AWV) component billing.</li><li>
In collaboration with the Health Education Team, schedule and complete Annual Wellness Visits (AWV) or Initial Preventative Physical Exam (IPPE).
