JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) provides intensive case management services to eligible clients, meeting participants at least weekly. The Case Manager is the primary point of contact to coordinate services and care for assigned clients. Each Case Manager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available.
DUTIES AND RESPONSIBILITIES
<ul><li>Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate, to develop a case plan.</li>
<li>Provide service linkages and support systems to ensure identified needs are met.</li>
<li>Coordinates individualized planning with clients to meet short- and long-term needs.</li>
<li>Coordinates case conferences with residents, consults with service team, monitors progress of clients on case plan, assists clients to meet the agreed upon goals.</li>

The CCM Case Manager is responsible for professional work in coordinating and conducting a substance abuse community intervention program through the Anne Arundel County Health Department. 
 
The CCM Case Manager provides comprehensive case management services to clients enrolled in the program. These services are tailored to meet the individual needs of clients while complying with the requirements of the Drug Treatment Court Program. The primary goal is to help clients overcome their drug and alcohol use and to encourage them to develop a more productive lifestyle. 
 
Case managers assist individuals with complex substance use disorder (SUD) needs by addressing various aspects of their lives, including medical, mental health, family, employment, criminal justice, housing, and financial issues. These areas often pose barriers to achieving success in the recovery process. This position will facilitate care and streamline transitions with other entities within the county healthcare system, ensuring that participants receive appropriate ongoing care. JOB SUMMARY AND PURPOSE
The Case Manager (Specialist) &ndash; Problem Solving Specialist provides intensive housing crisis response and case management services to eligible clients, meeting participants at least weekly. Using active listening techniques, the Case Manager &ndash; Problem Solving Specialist is the primary point of contact to coordinate creative resolutions to a housing crisis &ndash; locating both temporary and permanent housing. Located at a VOALA shelter, and working is the field as needed, the Case Manager (Specialist) &ndash; Problem Solving Specialist focuses on resolving the housing crisis, but can refer to other resources as needed.
DUTIES AND RESPONSIBILITIES
<ul><li>As a member of the program case management team, provides information referrals, crisis intervention, and assistance with permanent housing placement for participants</li>

<div>The Case Management Specialist works with providers, patients, and insurance companies to help ensure patients receive the highest level of care, assisting them with their chronic care and preventative care needs to help reduce long-term health concerns and hospitalizations. They educate patients on the benefits of recommended preventative care services and build rapport with them to improve patient engagement, self-care management, and adherence to their individualized plan of care. Provide oversight and execution of daily operations to ensure work is completed on schedule, within budget and to specified quality standards to assigned Case Specialists (CS).</li><li>
Review all documentation submitted by CS to ensure they are aligned with policy and procedures for safe and efficient reunification.</li><li>
Regularly review documents submitted by CS and uploaded to government case management tracking system (UC Portal).</li><li>
Promote productivity and ensure staff meet their performance standards and metrics/goals.</li><li>
Hold daily sessions with CS team to check on caseload progress, discuss any policy changes from the Office of Refugee Resettlement (ORR), and answer questions.</li><li>
Promote positive environment with an emphasis on building high morale; offer words of encouragement to assigned US team and make them feel valued and appreciated; promote collaboration.</li><li>
Ensure CS staff always follow proper procedures and ORR policy.</li><li>
Report any issues with their assigned CS to the Case Specialist Senior Lead.</li><li>
Frequently and consistently interact with each assigned CS staff member throughout the shift to see if they need assistance, have any questions, or address roadblocks and issues on a case, etc.</li><li>
Provide recommendations for updates to SOPs and training to the Senior Lead Case Specialist.</li><li>
Assist with the development and refinement of policy interpretation, guidelines, and procedures that will improve or enhance service delivery of the program.</li><li>
Conduct orientation and on-the-job training for CS staff members.</li><li>
Monitor status of cases to comply with contract metrics and elevate issues to the Case Specialist Senior Lead.</li><li>
Assist in vetting sponsors nationwide.</li></ul>
Minimum Qualifications &amp; Skills<ul><li>
Bachelor&rsquo;s degree in social work or an equivalent degree in psychology, sociology, or other relevant behavioral science.</li><li>
Bilingual in Spanish and English.</li><li>
ORR experience, preferably doing case management work.</li><li>
Two (2) or more years&rsquo; supervisory experience.</li><li>
Two (2) or more years&rsquo; Case Specialist experience.</li><li>
Experience working with Government Clients, preferably ORR, FEMA, HHS, and/or DHS.</li><li>
Experience working with refugees under the age of 18.</li><li>
Demonstrated ability to service a diverse group of clients, including refugees or other minority families.</li><li>
Experience working with traumatized/exploited children and children with behavioral problems.</li><li>
Knowledge of state, community, and agency resources for victims of abuse preferred.</li><li>
Proficiency to easily listen to verbal communication and transcribe and translate that information into clear written error- free communication in English and/or Spanish.</li><li>
Ability to handle crisis intervention, conflict, confrontation or other complex situations with compassion and decisiveness.</li><li>
Aptitude to work cross functionally with TPG personnel and stakeholders to deliver the highest quality of work under extreme pressure and in a fast-paced environment.</li><li>
Proven experience and high level of comfortability operating technology and learning new software applications quickly to perform data entry.</li><li>

The Work Environment<ul><li>
You will be required to complete 2 weeks of mandatory live training, to include webinars, self-paced training, and in-person/web-based workshops.</li><li>
Travel assignments may be required based on project need, though program leadership will attempt to minimize travel by assigning tasks to those who reside closest to the site where a support need exists.</li><li>
Tasks/assignments are dynamic and will change based on client needs and resource availability, meaning the Case Specialist Lead is expected to provide additional administrative support, including the completion of data entry and intake paperwork, as required.</li><li>
Hybrid work schedule (onsite/remote):<ul><li>
When working within 30 to 50 miles of an office, you will be required to work in-office as agreed upon by the management team. If/when working remote, you must have a dedicated workspace conducive to full videoconferencing (camera and audio) for facilitating webinars and online discussions.</li></ul></li></ul>
Work Schedule<ul><li>
Due to the importance of this position, employees supporting this contract can be required to work extended hours to include evening work, on-call, and weekends/holidays to support time-sensitive or real-time complex services.</li><li>

The HLPS Case Manager (a &ldquo;CM&rdquo;) is an essential team member that develops and executes housing navigation and placement services including follow-up post placement for clients referred by NYC Health + Hospitals (H+H). The CM meets with the client in person to develop a housing plan and complete the housing packet and works to support the client to find and move into permanent housing. The CM checks in at consistent, regular intervals during and post move-in/placement and will help the client renew their lease or reapply for vouchers if necessary. The CM will document all interactions, activities, and status changes. The CM will also coordinate services with other contacts to support the client as needed, including shelter/safe haven housing specialist, H + H care team, etc. and as needed, referrals to external providers. The CM plays a key role in executing and updating housing plans, identifying needs, preferences and eligibilities, monitoring client behaviors and practices, and trouble-shooting incidents.</div>

Conduct intake process with new clients within 48 hours of assignment; includes but is not limited to explanation of HLPS client rights and responsibilities, and grievance procedures.</li><li>
Develop initial housing plans and conduct plan reviews as needed for an assigned caseload.</li><li>
Work with clients to assess eligible housing vouchers, identify housing preferences to search for and match with opportunities, coordinate with the client and the peer counselor/specialist for housing tours and/or interviews, collect necessary documents, and complete applications and paperwork.</li><li>
Regularly schedule follow-up after initial placement to identify further support needs, trouble-shoot incidents, confirm payment, and reapply for vouches and support in lease renewals as needed.</li><li>
Provide accurate, complete, and timely data entry and other documentation of the housing status, interactions, and communications with and on behalf of clients, and if relevant, provision of supportive social services, and client progression/regression toward the goals of securing permanent affordable housing.</li><li>
Attend staff meetings, case conferences, group supervision, and professional development activities such as in-service training(s), seminars, webinars, and conferences as needed. Work with other consortium partners as necessary.</li><li>
