<div>The Home Base Case Manager Supervisor works under the supervision and direction of their supervisor for the purpose of providing the highest quality of care to children and families. The Home Base Case Manager Supervisor provides in-home and community-based family services, including crisis intervention, counseling, skill building, and advocacy. Maintains a flexible work schedule of 40 hours a week Reads and replies to emails, directly liaisons with Department of Child Services, Probation and community resources. Case Manager Supervisor is responsible for Assignment of referrals as they are received, reviewing and approving progress notes, treatment plans and monthly reports for compliance, reviews program and policy needs, chart audits, and caseloads to maintain state audit requirements. The home-based Case manager provides direct supervision to up to 5 staff, covers needs due to illness, injury or failure of their staff to provide direct services.</div>

<div><b>Essential Functions</b>:</div>
<div><i>II. Conduct interventions that are consistent with the agency service model (values, beliefs, structure and practices) and with DCS Service Standards, including but not limited to</i>:</div>

<ul><li>Direct supervision of staff up to (5)</li>
<li>Assist with conducting weekly staff meetings</li>
<li>Reviews all case notes of employees assigned to them</li>
<li>Reviews and signs all treatment plans</li>
<li>Reviews all monthly reports</li>
<li>Reviews hours for time sheet reporting</li>
<li>Provides coverage in event of failure of employee to provide such</li>
<li>Assignment and tracking of incoming referrals to case load expectations within contract expectations.</li>
<li>Schedule and complete client intake with face-to-face contact with the family within 48 hours of referral.</li>
<li>Provide 24-hour crisis intake, intervention, and consultation seven days a week provided primarily in the family's home. Limited services may also be provided at a community site.</li>
<li>Provide ongoing risk assessment and monitoring family/parental progress.</li>
<li>Provide services that are time-limited and focused on limited objectives derived directly from the established DCS/Probation case plan or Informal Adjustment.</li>
<li>Provide services that include development of short and long-term family goals with measurable outcomes that are consistent with the DCS case plan.</li>
<li>Provide services that are family centered and child focused. They are to include intensive in-home skill building and must include after-care linkage.</li>
<li>Services include providing monthly progress reports; requested supportive documentation such as case notes, social summaries, etc.; and requested testimony and/or court appearances including hearings and/or appeals; case conferences/staffing. Monthly reports are due to your supervisor by the 1st of each month following the month. Completion of EON and NCFAS information within 2 weeks of Intake.</li>
<li>Services will be conducted with behavior and language that demonstrates respect for sociocultural values, personal goals, lifestyle choices, and complex family interactions and be delivered in a neutral-valued culturally competent manner.</li>
<li>Services will be provided within the context of the DCS practice model or Probation plan</li>
</ul> <div>with involvement in Child and Family Team (CFT) meetings if invited. A treatment plan will be developed based on assessment by the provider and agreements reached in the Child and Family Team meetings and/or documented in the authorizing referral.</div>
<ul><li>Conduct holistic, strength-focused assessment process.</li>
<li>Meet the current guidelines on face-to-face client hours-currently at a 5/2 ratio. This includes client face to face time of 5 billable hours and 2 non-billable hours for other duties such as writing emails, travel, phone calls, monthly reports, staffing etc.</li>
<li>Provide a range of clinical, concrete, and advocacy services to family members that are consistent with the family&rsquo;s values, learning styles, lifestyle, circumstances and culture.</li>
<li>Be aware of and respond appropriately to potential safety issues.</li>
<li>Utilize motivational interventions and cognitive/behavioral strategies.</li>
<li>Utilize a collaborative goal setting and teaching approach.</li>
<li>Use interventions with clients that are theoretically based.</li>
<li>Use effective strategies for engaging defusing and confronting clients.</li>
<li>Develop and facilitate services to community resources and informal supports for families.</li>
<li>Provide ongoing evaluation and modification of family goals and service plans.</li>
<li>Solicit feedback and input from the family about all aspects of the intervention.</li>
<li>Implement termination process at the end of intervention.</li>
<li>Participate in other program areas as it is requested by your supervisor to meet program needs.</li>

Youth receive services directly from Everstand, or via its national network of care providers comprised of other children and family-focused non-profits.</p><p><b>The Home Study and Post Release Services Program expands Everstand&rsquo;s continuum of care serving recently migrated youth through the provision of comprehensive, safety-focused home studies and holistic, client-centered, and trauma-informed Post Release case management services.</b></p><p><b>The Presbyterian Home for Children is currently recruiting for a PRS Case Manager Supervisor for the Caminos program. This position is based out of Birmingham, AL.</b></p><p>ob description:</p><p>Supervise Home Studies &amp; Post-Release Services (HS/PRS) Case Managers in the provision of Level One (1) and Level Two (2) PRS.</p><p>Essential Functions:</p><ul><li>Model and act in accordance with the Presbyterian Home for Children (PHFC)&rsquo;s core values.</li><li>Demonstrate and practice trauma-informed leadership.</li><li>Provide oversight and coordination for all aspects of Everstand&rsquo;s Home Study and Post Release Services Program, including direct supervision of the HS/PRS case management team.</li><li>Assign referrals to individual case managers within the required timeframe, considering case manager workload, location, and the complexity of case distribution.</li><li>Provide initial and continuous training and professional development for the HS/PRS case management team regarding responsibilities within the Caminos Program, including agency, state, and federal policy updates and changes, and best practice service provisions.</li><li>Provide ongoing and continuous identification of community-based resources, providers, and services available to meet the needs of communities served by the program, ensuring that HS/PRS case management team has easy access to current and up-to-date resources.</li><li>Facilitate regularly scheduled individual and group guidance sessions for all members of the HS/PRS case management team, including part-time and contract staff.</li><li>Actively participate in agency and community efforts to build culturally competent outreach, education, and interventions for the immigrant and refugee community.</li><li>Ensure adherence to Everstand-established performance metrics related to best practice contact requirements for youth and family, including phone and in-person face-to-face visitation requirements.</li><li>Provide oversight and support necessary to ensure that all case managers submit detailed and thorough documentation in compliance with Everstand and the Office of Refuge Resettlement (ORR) requirements and reporting deadlines, including but not limited to: case notes, federal and agency data collection, programmatic reporting, assessments, care plan documentation, transition/discharge planning, and post-discharge follow-up.</li><li>Provide effective communication across departments and programs to ensure consistency and clear expectations across the interdisciplinary team.</li><li>Responsible for interviewing and onboarding new HS/PRS Case Managers for the department. Remote duties require the ability to procure fast, reliable internet service (15 Mbps minimum download speed required) at the remote office location. Responsibilities include:</div>

<li>Prioritize work of subordinate supervisors and staff within the department and ensures that deadlines are met. This includes assessment of the risk to the child when complaint calls are received, and staff is not available to respond to all calls.</li>

Physical and Environmental Requirements:</b> 80% of the time in an office setting and up to 20% traveling via car or in the field. Includes visits to various environments such as shelters, day programs, other government agencies, and participants&rsquo; homes. Supervision and oversight of staff on call, which may include being available by phone 24 hours a day when scheduled. Infrequent evening or weekend hours (agency events). Must be able to lift up to 25 lbs.</p><p><b>

POSITION SUMMARY</b></p><p>
The Prevention Case Manager Supervisor (PCMS) provides direct supervision to the Prevention Case Manager team while also carrying a caseload of participants at risk of homelessness. This position ensures high-quality delivery of housing-focused case management services to families at risk of homelessness, primarily through partnerships with schools and churches. The PCMS role requires strong judgment, adaptability, and the ability to monitor program fidelity, and partners with internal and external stakeholders to promote housing stability and holistic support for individuals and families.</p><p><b>
RESPONSIBILITIES</b>
<br>
<i>General</i></p><ul><li>
Initiates and maintains effective working relationships with DRM departments and external partners.</li><li>
Represents DRM professionally in all interactions with clients, schools, churches, and other community contacts.</li><li>
Participates in required meetings, trainings, and organizational events.</li><li>
Monitor team caseloads, service plans, and documentation to ensure quality and compliance.</li><li>
Provides on-boarding and training for new Case Managers and Interns.</li><li>
Represent the Prevention team in multidisciplinary meetings, community collaborations, and case conferences.

Manage a dynamic caseload of participants at imminent risk of eviction or housing loss, including school and church-referred clients.</li><li>
Provide timely, housing-focused case management, including assessments, service planning, resource navigation, and eligibility determination for prevention services.</li><li>
Assist participants in accessing public benefits, rental assistance, and community-based supports that promote short-term housing stability.</li><li>
Collaborate with families to develop individualized plans focused on budgeting, employment retention, and long-term stability.</li><li>
Maintain accurate documentation in HMIS and internal systems, including case notes, eligibility assessments, and service outcomes.</li><li>
Provide culturally responsive, equitable support to families of diverse backgrounds, cultures, and beliefs.</li><li>
Participate in ongoing professional development and training activities.</li><li>
Perform other duties as assigned.</li></ul>
Requirements:<p></p><p><b><br>
MINIMUM QUALIFICATIONS</b></p><ul><li>
Bachelor&rsquo;s degree in Social Work, Human Services, or related field required; Master&rsquo;s degree preferred.</li><li>
Minimum 2 years of case management experience in housing or homelessness prevention services.
</li><li>At least 1 year of supervisory, lead, or mentorship experience.
</li><li>Experience in school, church, or family-centered service environments preferred.</li><li>
Bilingual in English and Spanish preferred.
</li><li>Must meet all background check requirements applicable to working within any Colorado school district.</li><li>
Must possess a valid driver&rsquo;s license and be able to travel frequently, typically between Denver and Northern Colorado on a weekly basis or more often, with occasional travel to other locations as required.</li><li>
