<p><b>Case Manager-Redirect</b></p><p><b>Job description</b></p><p>The Redirect addiction-related diversion program is a street-based, long-term, and unconventional approach to case management. Redirect case work is non-coercive, non-time delimited, street-based, and built on multiple pathways to recovery, including harm reduction, and steeped in Stages of Change theory and motivational interviewing practices.</p><p>This position will manage a caseload of approximately 15-25 participants and will work in conjunction with local police departments and FAN&rsquo;s Comeback Quick Response Team (QRT).
