The Case Manager Supervisor, Community Corrections supervises case management services for inmates/residents and their families.<ul><li>

What The Case Manager Supervisor Does:<ul><li>
Maintain professional relationships with clients and client confidentiality.</li><li>
Communicate effectively with clients, direct reporting staff, peers, supervisors, and funders.</li><li>
Plan and organize program activities to maximize program contract's goals and performance targets as outlined in DHS&rsquo; Performance Incentive Plan.</li><li>

Ensure that all direct reporting staff completes all necessary case management documentations including housing applications, ILP, and referrals to providers as required.</li><li>
Pull reports from CARES and distribute to staff to ensure staff are up to date on their documentation</li><li>
Develop and maintain tracking systems in order for staff to maintain up to date documentation</li><li>
Ensure that all client charting documentation are up-to-date and reflect services provided such as service plans, sycho-social and psychiatric assessments.</li><li>
Assist with the development of program content to address residents needs related to housing, substance abuse treatment, mental health and employment.</li><li>
Provide assistance, interventions and follow-up in regards to critical incidents with clients.</li><li>

Attends all required meetings and completes all required administrative paperwork from the above agencies.</li><li>Networks with appropriate agencies, organizations and individuals for the purpose of sharing resources and engaging in collective advocacy for women and children who are homeless.</li><li>Facilitates communication with the core community regarding matters of mutual concern.</li><li>Responsible for the execution of agency policies and procedures in the residence.</li><li>Participates in administrative team meetings, and completes necessary follow-ups.</li><li>Collaborates with the Senior Director of Human Resources and Chief Operations Officer on all human resource activities.</li><li>Coordinates with the Director of Maintenance and Building Operations and Chief Operations Officer on all matters of concern regarding building and facility management.</li><li>Effectively use and practice Sanctuary Models tools; safety plans, community meetings, psycho-educational groups, team meetings, red flags reviews, and self-care in the program to create a non-violent and trauma sensitive culture.</li><li>Promote and model the core values of Providence House Mission (Compassion, Non-Violence, Hospitality, and Community) and the Seven Sanctuary Model Commitments to create a non-violent and trauma sensitive culture.</li><li>Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.</li><li>Meets for regular supervision and performs other tasks as assigned by the Senior Director of Programs.</li></ul>Team/Staff Relations<ul><li>Work as a Providence House team member with all staff and volunteers in order to implement the mission and core values of the agency.</li><li>Conduct regular internal and external agency meetings.</li><li>Work as part of team to plan and develop resident&rsquo;s programs.</li></ul>Administrative<ul><li>Ensures that clinical staff keeps accurate, up-to-date documentation on all clients. Including but not limited to, treatment plans, case notes, etc.</li><li>Ensures that clinical staff completes all paperwork in a timely and accurate manner that ensures payment to the agency from various sources.</li><li>Assists with any and all data collection systems.</li></ul>QUALIFICATIONS<ul><li>LMSW in Social Work or a related field required;</li><li>Minimum 5 years in program supervision and administration;</li><li>Excellent verbal, written, communication and interpersonal skills;</li><li>Computer Literate;</li><li>Experience in working with homeless families and knowledge of city/state/government funding programs;</li><li>Experience with client databases;</li><li>Ability to travel within the NYC and Westchester areas;</li><li>Compatible philosophy with the Providence House mission.</li></ul>WORK SCHEDULE<ul><li>The Program Director position is a full time, salaried position.</li><li>Hours worked will be weekdays at least 9:00a.m. - 5:00p.m, or at a mutually agreed-upon schedule. Evening and/or weekend work also may be required.</li><li>The office location will be in New Rochelle, New York.</li><li>Position requires travel throughout NYC and Westchester areas.</li></ul>SALARY AND BENEFITSThe starting salary range is $75K - $85K Annually based on qualifications and experience. Reimbursement for mileage expenses to locations beyond the initial worksite. Paid time off benefits include: 14 paid holidays, 3 weeks of vacation, 4 personal days, 12 sick days, early closure proceeding holidays.
