The In Center Hemodialysis Patient Care Technician is responsible for providing exceptional hemodialysis care to patients under the supervision of the RN in the assigned In Center. 
 
PRIMARY DUTIES / ACTIVITIES:</div><ul><li>
Entering prescribed treatment into dialysis machine, setting up dialysis machines, dismantling dialysis machine after treatment is completed, performing chemical disinfection of dialysis machines and recording.</li><li>
Observing dialysis machines and RO water system for correct functioning, responding to visual and audible alarms and making appropriate adjustments throughout the dialysis treatment to ensure patient safety.</li><li>
Monitoring vital signs during treatment and taking patient vitals pro and post dialysis; blood pressure, temperature, weight and documenting it on the Hemodialysis Treatment Record</li><li>
Perform vascular access evaluation pre-treatment and report unusual findings to RN immediately</li><li>
Perform cannulation of patient&rsquo;s vascular access or performing catheter care, as allowed by state law.</li><li>

A Brief Overview 
The Patient Care Technician (PCT) on the Vascular Access &amp; Infusion Services Team provides essential clinical and administrative support to the Vascular Access RNs and the Infusion Services team. This position assists with preparation and support during Vascular Access procedures; manages infusion referrals; and performs patient care tasks within the approved PCT scope of practice. What you will do<ul><li>
Assist RNs with vascular access procedures by preparing patients and equipment for PICC and midline insertions, maintaining sterile fields in accordance with infection-prevention standards, and supporting dressing changes, securement, and site care under RN direction.</li><li>
Provide basic patient care and ensure procedural readiness by obtaining vital signs, assisting with positioning and comfort measures, setting up and maintaining vascular access equipment (including ultrasound machines, IV carts, and procedure trays), and restocking and organizing supply carts, procedure areas, and mobile kits.</li><li>
Coordinate intake and processing of infusion referrals by reviewing incoming orders for completeness (including diagnosis, labs, allergies, and treatment details), identifying and obtaining missing information from ordering offices, and accurately entering and routing referrals in the EMR to support timely scheduling.</li><li>
Support infusion workflow and patient readiness by tracking referral progress and maintaining daily workflow logs, assisting with appointment preparation (including confirming lab results, IV access needs, and pre-medication requirements), and communicating with provider offices to obtain necessary non-clinical documentation.</li><li>

Responsible for securing patient data based upon comprehensive data elements, financial education/counseling, insurance verification, provision of clinical information for pre-certification, authorization and third party claims management. Interviews patient for clinical, financial, and history information and arranges appropriate clinical care. A Clinical Pharmacy Technician will work independently gathering and submitted patient information with oversight of a pharmacist as needed. They will be responsible for performing pharmacy related functions in compliance with departmental policies and procedures that provide optimal pharmaceutical care.
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:<div><div><div>

4. Provides information and answers to patient/caregiver questions and refers to clinician when appropriate or requested.
5. Identifies all patients requiring pre-certification or pre-authorization at the time services are requested or when notified by another hospital or clinic department.
6. Contacts insurance company or employer to determine eligibility and benefits for requested services.
7. Maintains patient records in a timely and complete manner.
8. Assists the pharmacist with the process of filling and labeling the prescriptions.
9. Calculates initial measurements for compounds and maintains compounds logs.
10. Provides monographs and other required medication guides with each prescription.
11. Maintains proper storage of medications (i.e. security, temperature controlled items, etc.).
12. Demonstrates the knowledge and skills necessary to communicate to the infant, pediatric, adolescent, adult and/or geriatric patient per the established age-specific education/standard.
13. Works with coordinators and manager to develop and attain quarterly objectives.
14. Maintains confidentiality according to policy when interacting with patients, physicians, families, co-workers and the public regarding demographic/clinical/financial information.
15. Communicates problems hindering workflow to management in a timely manner.
16. Assists in the training of pharmacy technicians
17. Initiates charge anticipation calculations. Accurately identifies anticipated charges to assure identification of anticipated self-pay portions.
18. Certain duties cannot be assigned to Pharmacy Technicians because these duties can only be performed by pharmacists. These restrictions are regulated at state and federal levels and must be followed closely:<ul><li>Only pharmacists can receive oral prescriptions from prescribers or prescribers&rsquo; authorized designees. (This refers primarily to new telephone prescriptions).</li><li>Only pharmacists can consult with prescribers or prescribers&rsquo; designees about patients&rsquo; prescriptions.</li><li>Only pharmacists can provide confidential patient information to other health professionals or insurance companies.</li><li>Only pharmacists can check medications before dispensing them to patients. (In states where technicians can prepare or package prescriptions, they do so under the direct supervision of a pharmacist.)</li><li>Only pharmacists can consult with patients about prescribed medication use and other health issues.</li><li>Only pharmacist scan recommend over-the-counter medications.</li></ul>
