Safety and Quality. </b>You will use appropriate safety measures, including personal protective equipment. Working under the supervision of a Charge Nurse, you will adhere to all company policies, procedures, and state/federal laws and regulations. This position requires extensive travel between assigned patient&rsquo;s homes and may be up to long distances depending on the patient&rsquo;s needs. Travel to scheduled staff meeting may be required. PCTs spend considerable time walking, bending, stretching, and standing; assisting in patient transfers. PCTs must follow proper body mechanics and procedures for lifting/ repositioning patients without injury. PCTs may face hazards from exposure to chemicals and infectious/ contagious diseases. The work environment is characteristic of a dialysis facility with air temperature control and moderate noise.</p><p></p><p><br>
Ability to lift at least 30 pounds. Moving, lifting or transferring of patients may involve lifting of up to fifty (50) pounds.</p><p>

Primary Duties and Responsibilities</b></p><br>

Implements patients' plan of care as directed by the RN reporting information and findings to the RN and offering suggestions for revision of the plan of care as appropriate.
</li><li>Provides direct bedside care to patients as assigned inclusive of but not limited to: performs the activities of daily living bathing skin care toileting mouth care feeding and range of motion; assisting with ambulation mobilization and rehabilitation; assistance with crutches and walkers; and transporting of patients in wheelchairs and stretchers.</li><li>
Performs routine sterile and non-sterile treatments and other treatments identified by Unit/Hospital policy inclusive of but not limited to: tube feedings; ostomy care; Foley Catheter care; IV insertion/ monitoring/care; venipuncture; EKG; simple dressing changes and wound care; maintenance of drainage tubes with/without pump or suction; point of care testing; care of oxygen equipment ; and assistance with the application maintenance and removal of traction casts splints and exercise equipment.</li><li>
Participates in patient/family teaching as directed.</li><li>
Reports/records/documents information in designated areas of the medical record or via computerized systems according to established guidelines.
</li><li>Maintains a safe and orderly patient/unit environment.</li><li>
Contributes to the development and implementation of unit objectives.</li><li>
Participates in quality improvement initiatives.</li><li>
