<div>We are seeking a passionate, driven certified physical therapist assistant to deliver therapy to adults with a variety of medical conditions including orthopedic, neurological, and cardiopulmonary. In this position, you will provide and direct physical therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication and functions under physicians&rsquo; orders. We welcome seasoned, experienced PTAs to apply for this position, as well as new grads.

This short and long-term care community, which is privately owned by a local family, has a friendly team of therapists across all disciplines. The community features 93 beds, which are primarily long-term care. The team upholds a strong commitment to quality care, evidence-based practice, and outstanding outcomes.

Infinity Rehab assists healthcare providers with the oversight and execution of their in-house therapy services. We provide our customers with the systems, compliance oversight, staff training, MDS optimization, and management they need to run a successful program. Responsibilities:

As a Home Health PTA, You will:</div><ul><li>
Provides physical therapy services planned, delegated and supervised by the qualified Physical Therapist in accordance with the patient&rsquo;s Plan of Treatment</li><li>
Confers with Physical Therapist regularly regarding patient&rsquo;s plan of treatment, status of progress toward goals, questions and concerns</li><li>
Communicates patient&rsquo;s needs and progress to the Physical Therapist of record and assists in securing special adaptive equipment for patient to improve function. Incorporates the use of adaptive equipment to enhance the rehabilitation program</li><li>
Maintains and submits patient treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations</li></ul>
Qualifications:
<ul><li>Current unrestricted license as a Physical Therapist Assistant by state in which practicing</li><li>
Current Basic Life Support or cardiopulmonary resuscitation certification, in accordance state regulations</li><li>
A minimum of one year&rsquo;s work experience as a Physical Therapist Assistant preferred</li><li>
Demonstrates knowledge of rehabilitation techniques related to complex neurological injury</li></ul>

$49,500 - $109,300 annual total cash target pay

Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of &quot;direct&quot; and &quot;indirect&quot; patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable.
