We are a group of 20 CRNAs (full time, part time, and per-diem) that work in the Anesthesia Care Team model in a wide variety of locations/specialties in our 47 ORs, as well as many out-of-OR locations, OB, (no pediatrics), and our surgi-center in Foxborough.<br>

Job Summary</b></p>
A Certified Registered Nurse Anesthetist (CRNA) is responsible for the pre-operative, intra-operative, and immediate post-operative care of the patient requiring anesthesia for surgical, medical, therapeutic, or diagnostic procedures. This care includes, but is not limited to, pre-operative assessments, anesthetic care plans, assembly and care of specialized equipment; application, insertion and interpretation of data from anesthetic monitors; airway management; delivery of general, regional anesthesia, and monitored anesthetic care; induction, maintenance, and emergence of the patient; care of the patient in the immediate post-operative setting; and post-operative assessments in collaboration with a physician member of the Department of Anesthesiology. These services are rendered in the main operating rooms, Labor and Delivery, and designated &quot;Out of OR&quot; anesthesia locations (i.e., Radiology, MRT, etc.) CRNAs also fulfill a teaching function for the Student Registered Nurse Anesthetists that rotate to the BWH from Northeastern University for their clinical experience. Clinical research may be available as an adjunct for the CRNA who is interested in that area of practice, depending on protocols available, but it is not a department requirement, nor is it usually sponsored by the anesthesia department. The Post Heart Transplant Nurse coordinator is a professional registered nurse licensed in the state of Massachusetts with cardiac surgery or thoracic surgery experience who is responsible for all aspects of ambulatory and in-hospital care of the pre-transplant and post-transplant patient. This position will work closely with all members of the transplant team, inpatient staff, and others as necessary to ensure efficacious care during the transition between pre-, peri-, and post-transplant phases in ambulatory and hospital settings.

Additional responsibilities will include collecting and entering data into EPIC and heart transplant databases as required by the hospital, and by local, regional and national agencies.<br>

Transplant patients are an extremely complex group of patients and as such the transplant coordinator must possess a strong clinical background. This nurse must be organized, they must pay attention to detail, demonstrate the ability to coordinate and follow through on all aspects of the patient&rsquo;s care and effectively communicate with all members of the transplant team, the patient / family and transplant unit.<br>

The transplant coordinator will need to demonstrate the ability to effectively manage patients by assuming full responsibility for assessment, planning, implementation and evaluation of patient care. The RN is directly responsible to a designated nurse director, or supervisor.<br>

</li><li>Acts as a liaison between members of the inpatient care team and outpatient management team, including heart transplant nurse practitioners.
</li><li>Communicates with referring physician team to keep them up-to-date regarding patients clinical status.
</li><li>Telephone contact with patients to assess clinical status, monitor response to therapy, adjust medications in consultation with clinical providers, and organize outpatient or inpatient follow-up as needed.
</li><li>Assists in urgent patient triage, including emergency room visits and transfer from outside institutions, and documents care provided.
</li><li>Provides education to transplant patients and family members regarding important aspects of care, including but not limited to activity, diet, medication adherence and healthcare follow-up. Helps to maintain current supply of patient/family education materials.
</li><li>Sets up consults with specialty providers, follows up consult recommendations and communicates results to patient and other providers.
</li><li>Assists with communication and correspondence to/from patients, employers, pharmacies and insurance providers with regards to medication prescriptions, cardiac rehabilitation, disability, family medical leave, and payment coverage. May also communicate and correspond with other providers (e.g., utility company, telephone company) as dictated by patient needs. These duties may require telephone calls, faxes, mail and/or e-mail.
</li><li>Meets regularly with medical program director and nurse practitioners to facilitate quality and process improvement initiatives. As requested, implements and assesses initiatives and summarizes results for team.
</li><li>Attends weekly clinical meeting and heart transplant clinics as needed.
</li><li>Works with financial counselor and social work to assist patients/families in need of financial support, insurance coverage, medications, housing and/or utilities.

<ul><li>Accountable for assessing, planning, implementing and evaluating a plan of care for a specific patient assignment.</li><li>Sets priorities when organizing care for patients with varying acuity.</li><li>Maintain continuity through clear and concise (verbal and written) communication.</li><li>Demonstrates appropriate knowledge of growth and development of the adult and geriatric patient. In areas, such as the NICU and Obstetrics the principals of growth and development are applicable to the neonate.

<ul><li>Documents patient care in a manner that is clear, complete, concise and in compliance with nursing documentation standards.</li><li>Develops a comprehensive plan of care based on data from an initial assessment of patient and family, information from other members of the health care team, intra-agency referral and previous medical records.</li><li>Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources and documents according to the department&rsquo;s documentation standards.</li><li>Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.</li><li>Evaluates the effectiveness of the plan of care, and documents progress in meeting stated goals. Revises plan of care as needed to achieve desired outcomes.</li><li>Participates actively in the unit-based and organizational quality management and/or quality improvement programs.</li><li>Provides a safe environment for patients, staff, family and visitors.</li><li>Administers medication safely according to established policies and procedures.
