The CCM Case Manager provides comprehensive case management services to clients enrolled in the program. Completes relapse prevention plan with patients who are in remission. Documents patient progress and treatment recommendations in the registry so they can be easily shared with PCPs (Primary Care Physicians), psychiatric consultants, and other treatment providers. Participates in regularly scheduled caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient&rsquo;s primary care provider. Provides brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving treatment, motivational interviewing, or other treatments appropriate for primary care settings.
