Center For Family Services is seeking a motivated, flexible, outgoing Case Manager for the Kinship Wraparound Program. The Case Manager is responsible for case management services for the Kinship program including, but not limited to: strong child/home assessment skills, investigating, verifying &amp; documenting information gathered from home visits; keeping accurate records; providing home visits to all referred Kinship Care families; preparing reports; making appropriate referrals to community services; maintaining strong working relationship with the court system, DCP&amp;P and other community partners by acting as an inter-agency and community liaison. <p><b>Duties and Responsibilities:</b></p>

Provide home visits to all referred Kinship Care families, maintaining respect for clients at all times</li><li>
Provide direct service in assisting eligible kinship caregivers with application for legal guardianship and eligibility for the Kinship Care Subsidy Program</li><li>
Complete and submit court assessment(s) to the appropriate Superior Court, Family Part</li><li>
Secure proper documentation to verify relationship, residence, age, and income</li><li>
Complete a home study, to include but not limited to: an interview with the caregiver, and all adult members of the household; a tour of the caregiver's home, (including the outside grounds); and, an observation and interview with the relative children</li><li>
Arrange for criminal history, domestic violence, and child abuse record background checks of all adults living in the caregiver's home</li><li>
Prepare written reports to the State documenting findings from the investigation
</li><li>Provide case management including problem-solving, consultation, interviewing, coordination of services, appropriate community referral when indicated, advocacy and detailed record keeping of client progress, and preparing reports related to the provision of service</li><li>
Promote and recruit families to program</li><li>

Position Overview
<br><br>
Under the supervision of the Family Preservation Clinical Coordinator, the Case Manager provides comprehensive family assessments and delivers in-home, strengths-based services to families enrolled in the Family Preservation Program. This role partners closely with families and a multidisciplinary team to develop and implement individualized service plans that promote child safety, family stability, and overall well-being. All services are delivered in alignment with best practices and contractual requirements set forth by the Department of Children and Family Services (DCFS).

Duties

<li>Conducts in-home visits to assess family needs, build rapport, and support clients in achieving established case plan goals.</li>
<li>Collaborates with multidisciplinary team members and clinical supervisors to develop, implement, and monitor individualized service and treatment plans.</li>
<li>Co-facilitates case conferences to review client progress, address barriers, and adjust interventions as needed.</li>
<li>Connects families to community-based resources and supports that enhance family functioning and strengthen parenting capacity.</li>

The Case Manager is responsible for providing social services to children, youth and families that are consistent with the MCH Model of Care and ensuring a safe, caring and therapeutic environment for those served. This position requires the intellectual and emotional maturity to work with families who are experiencing difficulty in providing appropriate care for the children in their home and children who have behavioral issues, developmental delays and mental health needs. This position also requires respect and compassionate understanding for these children and their families, and the ability to form effective working relationships with children, parents and other caregivers. Case Managers must have a thorough understanding of and demonstrated competency in the Trust-Based Relational Intervention (TBRI) and other evidence-based programs offered in the assigned area. The Case Manager must maintain a healthy value system, including moral and ethical behavior consistent with the agency's mission and core values.
</p><h4 class="jobSectionHeader"><b>DUTIES AND RESPONSIBILITIES</b></h4>
<p></p>
<ul><li>Identify and assess eligible participants according to program criteria and obtain consent for services for those accepted into the program, and make referrals to other community resources for those not accepted into the program.
</li><li>Develop and implement written plans of service based on the assessed needs of the child or family in accordance with program guidelines. Revise and update the written plans of service as needed to address the changing needs of those served and to comply with program guidelines.</li><li>
Provide services in compliance with program guidelines and as outlined in the plan of service.</li><li>
Maintain a regular schedule of contacts and case management services on the assigned caseload in accordance with program guidelines and the needs of each case. Goals and methods related to these contacts are to be established in consultation with the supervisor, parents/managing conservators and, when possible, with the children and youth served.</li><li>
Coordinate, consult and collaborate with foster families and/or kinship caregivers and other service providers to provide needed services to children, youth and families.</li><li>

This is a professional position that may require more hours than the normal 40-hour workweek. This position involves providing services in the client's home and extensive work in the community. Responsibilities include work in the evenings and on weekends. This position requires the ability to serve in an on-call capacity and respond to emergencies after office hours. Out of town and overnight travel are also required. An automobile for in town and out of town travel is required. Mileage reimbursement is provided for use of personal automobile for approved business purposes. A technology stipend is provided to support the communication requirements of this position.</p>

A Bachelor's Degree in Social Work or related field is required with a Master's Degree preferred. Relevant experience in programs and services offered by MCH is preferred.

The <b>Case Manager</b> will report to the Clinical Supervisor. In collaboration with other members of the clinical team, the case manager is responsible for serving as a liaison for the treatment team, client, families and collateral service providers.</p>

Responsibilities:</b></p>
<ul><li>Maintains assigned caseload and ensures the proper implementation of in-home services in accordance with referrals made by DCYF.</li></ul>
<ul><li>Works collaboratively with DCYF to respond to referral requests, create treatment plans, participate in treatment meetings, and provide regular service specific documentation.</li></ul>
<ul><li>Develops and collaborates on treatment plan formation with measurable and attainable goals in partnership with families and DCYF.</li></ul>
<ul><li>Adheres to all documentation requirements of service delivery including assessment writing and report writing.</li></ul>
<ul><li>Participates in reporting progress to the court system through in person appearances and written reporting as requested.</li></ul>
<ul><li>Provides parent education and supervised visitation to children, adults, and families in home and community-based settings.</li></ul>
<ul><li>Offers parent coaching and support including provision of effective parenting strategies, meeting healthcare and educational needs of children, understanding developmental milestones, meal preparation and budgeting.</li></ul>
<ul><li>Enforces supervised visitation expectations as identified by DCYF to ensure child safety.</li></ul>
<ul><li>Interfaces with DCYF, the NH court system, schools, behavioral health providers, and any other involved members of the treatment team as necessary to provide quality service delivery.</li></ul>
<ul><li>Successfully assesses client risk on an ongoing basis and consult with clinical supervisor regarding any safety concerns.</li></ul>
<ul><li>Transportation of clients, including children and adults, requiring car seat and booster seat installation.</li></ul>
<ul><li><b>On-call rotation</b>: responsible for responding to phone to provide support to families as utilized.</li></ul>
<ul><li><b><i>Early evening availability required for working hours. Most visits to clients are in the afternoon or early evenings. Must have flexible availability to work due to this. Schedule/hours are subject to our clients needs.</i></b></li></ul>
<ul><li>Ability to successfully document all clinical contacts and documentation through Electronic Health System.</li></ul>

Qualifications:</b></p>

<li>Bachelor&rsquo;s Degree with a major in social work, counseling, psychology or related human services field</li>
<li>Two years minimum experience working with children, youth and families in a home or community setting preferred</li>
<li>Valid driver&rsquo;s license with reliable and consistent access to a vehicle</li>
