The Case Manager will coordinate services and resources for youth and families being served. The Case Manager provides ongoing support and training to foster parents as well as ensuring that foster parents maintain compliance with established standards. Principal Duties and Responsibilities</p><p>
 Case Management</p><ul><li>
Prepare reports and reviews in a timely, clear, and concise manner per Service Manual guidelines, including individual treatment plans or person-centered plans, contact logs, quarterly reviews, supervision notes, discharge summaries, Child and Family, and other assigned reports, utilizing computer-based clinical management software and other network software</li><li>
Review medical administration records (MARs) and grids at twice a month to ensure timeliness and accuracy.</li><li>
Complete all documentation within agency deadlines, according to regulatory guidelines</li><li>
Participate in the planning and delivery of services regarding admission, treatment, and discharge. This includes coordination and facilitation of Child and Family Team Meetings as well as other services as needed.</li><li>
Provide direct treatment services as needed, entering the rotation in the event of staff crisis (illness, injury, other absence) or team vacancy, when applicable</li><li>
Complete all preservice training in required time frames. Staff will annually complete 24 hours of training.</li></ul><p>
Supervision responsibilities:
</p><ul><li>Provide documented supervision and support to foster families by completing a minimum weekly visits to Therapeutic foster parents and 2 twice monthly to family Foster Care parents to continuously evaluate strengths and needs and provide ongoing training and support</li><li>
Will meet with the child individually and will provide documentation of the visits on the child's contact log</li><li>
Return all emergency phone calls and text messages within 30 minutes of call; return all general phone calls and text messages within four hours
</li><li>Ensure implementation of Teaching Family Model, by providing consultation in accordance with specified service delivery schedule</li><li>
Maintain contact with involved parents or relatives regarding the child&rsquo;s life, progress in treatment, and treatment plan - c0ntact to be maintained at weekly intervals or as determined by the Admission Agreement</li><li>
Assess safety, family strength, and needs, as well as access necessary community resources</li><li>
Assist foster family in providing a safe and nurturing environment and be knowledgeable of mental health issues and behavioral management supports</li><li>
Maintain contact with custodians, social workers, and other involved professionals, including periodic updates of the resident&rsquo;s progress and treatment plan as set forth in the Admission Agreement</li><li>
Represent the agency in activities occurring within the ecosystem of the child (treatment review meetings, court hearings, and other functions involving professionals external to the service team</li><li>

Responsibilities</b>

Qualifications:

Education:</b></div><ul><li>
Bachelor&rsquo;s degree in human services, criminology, juvenile justice, psychology, sociology, counseling, special education, education, human development, child development, family development, marriage and family therapy, and nursing <i>required</i></li><li>
Master's Degree in Counseling, Social Work or related field<i> preferred</i></li></ul><div><b>
Experience:</b></div><ul><li>
Must have full Florida Board Certification in Child Welfare Case Management</li><li>
Two years of experience working with children, adolescents and families is strongly preferred.</li><li><div>
Strong organizational skills, critical thinking, and the ability to manage complex cases are essential. The Case Manager must demonstrate professionalism, cultural competency, and a commitment to supporting children and families through a trauma-informed and strengths-based approach.</div></li><li><div>
On call is also needed. Transportation and supervising visits may also be necessary.</div></li></ul><div><b>

The <b>Case Manager</b> will report to the Clinical Supervisor. In collaboration with other members of the clinical team, the case manager is responsible for serving as a liaison for the treatment team, client, families and collateral service providers.</p>

Responsibilities:</b></p>
<ul><li>Maintains assigned caseload and ensures the proper implementation of in-home services in accordance with referrals made by DCYF.</li></ul>
<ul><li>Works collaboratively with DCYF to respond to referral requests, create treatment plans, participate in treatment meetings, and provide regular service specific documentation.</li></ul>
<ul><li>Develops and collaborates on treatment plan formation with measurable and attainable goals in partnership with families and DCYF.</li></ul>
<ul><li>Adheres to all documentation requirements of service delivery including assessment writing and report writing.</li></ul>
<ul><li>Participates in reporting progress to the court system through in person appearances and written reporting as requested.</li></ul>
<ul><li>Provides parent education and supervised visitation to children, adults, and families in home and community-based settings.</li></ul>
<ul><li>Offers parent coaching and support including provision of effective parenting strategies, meeting healthcare and educational needs of children, understanding developmental milestones, meal preparation and budgeting.</li></ul>
<ul><li>Enforces supervised visitation expectations as identified by DCYF to ensure child safety.</li></ul>
<ul><li>Interfaces with DCYF, the NH court system, schools, behavioral health providers, and any other involved members of the treatment team as necessary to provide quality service delivery.</li></ul>
<ul><li>Successfully assesses client risk on an ongoing basis and consult with clinical supervisor regarding any safety concerns.</li></ul>
<ul><li>Transportation of clients, including children and adults, requiring car seat and booster seat installation.</li></ul>
<ul><li><b>On-call rotation</b>: responsible for responding to phone to provide support to families as utilized.</li></ul>
<ul><li><b><i>Early evening availability required for working hours. Most visits to clients are in the afternoon or early evenings. Must have flexible availability to work due to this. Schedule/hours are subject to our clients needs.</i></b></li></ul>
<ul><li>Ability to successfully document all clinical contacts and documentation through Electronic Health System.</li></ul>

Required Experience:</b></p>

<li>Bachelor&rsquo;s Degree with a major in social work, counseling, psychology or related human services field</li>
<li>Two years minimum experience working with children, youth and families in a home or community setting preferred</li>
<li>Valid driver&rsquo;s license with reliable and consistent access to a vehicle</li>
