Summary of Program and Position:
Family Peace Centers promote peace and safety for Hawaii&rsquo;s families and communities by providing support and education for those who use violence, survivors, and their children. Violent and abusive behavior is learned and can be unlearned. Accepting responsibility and being accountable for one&rsquo;s behavior are critical in the path to peace. The consistent use of non-violent skills assures safety for survivors and children, promotes offender accountability, and solidifies an end to the cycle of violence in families and communities. Family Peace Centers emphasize partnership, equality and respect among and between all family members.
The Counselor Advocate is responsible for providing advocacy, counseling, and support services to domestic violence survivors, offenders and/or children exposed to domestic violence participating in the FPC program. This position is under the direct supervision of Program Supervisor.
 Essential Duties:<ul><li>
Plans, conducts, and completes intake/assessments in accordance with agency and program procedures.</li><li>
Plans and facilitates a variety of domestic violence intervention groups, as designated.</li><li>

 As a Child and Family Advocate in the Kids- TALK Program, you will provide direct services to children and families seen at the Kids-TALK Children&rsquo;s Advocacy Center. This position contributes to the mission of the Children's Advocacy Center by providing advocacy, crisis support and referral services to child victims of abuse and neglect and their non-offending families and caretakers. The Advocate is required to ensure all aspects of the Kids-TALK program meet the program&rsquo;s mission while staying in compliance with required reporting agencies. The Advocate monitors that referrals are handled in a timely manner and that all required data is submitted accurately and timely. Regular working hours are Monday through Friday 8:30 a.m. until 5:00 p.m., although due to the needs of the CAC&rsquo;s clients and partners, additional hours including evenings and weekends may also be required. This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family&rsquo;s home and community&ndash;Intercept&reg; is not an office-based program. 
 
Intercept provides intensive, strength-based support tailored to each family&rsquo;s unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.

 Position Summary
 
 
 
 The Family Centered Treatment (FCT) Practitioner will provide structured, goal oriented and time limited Family Centered Treatment in the natural environment to families involved in a variety of systems such as child welfare, juvenile justice, mental health, and substance use. The ultimate goal of treatment is to maintain the family unit and avoid out of home placements.

ABOUT THE PROGRAM: East Hartford Outpatient

East Hartford Outpatient (EHOP) therapists have the unique opportunity to provide therapy to adults, children and families. Therapists see clients in person and through telehealth for diagnostic assessments, treatment planning, individual and family therapy sessions. Opportunities for case consultation, supervision, training and professional growth are provided.

ABOUT THE POSITION: Care Coordinator

Coordinate and monitor the implementation of plans that address the behavioral health needs of the client. To work in partnership with the client and treatment team to coordinate and triage daily tasks and care for clients. Communicate and function as a liaison between services and providers. Complete any necessary reporting in a timely manner. Advocate and support clients to address treatment barriers such as access, transportation, etc. in order to increase the independence and self-efficacy for individuals and families within our services.

Duties &amp; Responsibilities:
<ul>
<li>Participate as member of multi-disciplinary treatment team</li>
<li>Track and coordinate client intake, discharge, services and referrals as applicable</li>
<li>Provide assistance to team including coordination of treatment planning and providing daily triage.</li>
<li>Educate and link individuals to self-help groups, natural support and other resources as needed.</li>
<li>Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance, housing, treatment, representative payee, legal services, medical services, education and employment.</li>
<li>Maintain careful documentation of contacts and communication to maintain quality services. Complete all required documentation in compliance with agency policy.</li>
<li>Work as a liaison between agency and other relevant providers and teams to coordinate care for referred clients.</li>
<li>Assist with scheduling and providing transportation to and from appointments.</li>
<li>Maintains documentation in compliance with agency and program standards.</li>
<li>Participates in staff meetings, supervision, training and other meetings as assigned.</li>
<li>Establish knowledge of cultures and communication styles associated with various groups, particularly oppressed groups, to provide culturally sensitive and competent care.</li>
<li>Provides crisis intervention and support as clinically indicated.</li>
<li>Additional duties as required.</li>
</ul>
QUALIFICATIONS:

Education: Bachelor&rsquo;s degree in human service-related field required. Experience may be accepted in lieu of education for some positions.

Experience: A minimum of two years&rsquo; related experience preferred.

Licensure/Certification/Registration: Valid driver&rsquo;s license required.

Training: Complete any program specific training required. Complete required annual agency trainings. CPR/First-Aid Certification may be required within 6 months of hire

COMPENSATION*:

$19.28/hr min, and up commensurate upon experience.
<ul><li>Actual rates are determined at the time of offer and are based off relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)</li></ul>
