Summary
 
The Family Service Coordinator is responsible for monitoring ERSEA (Eligibility, Recruitment, Selection, Enrollment, Attendance) practices and coordinating family services across sites including the promotion of family wellness, building community partnerships and operating as a liaison between programs, classrooms and home settings in the areas of family engagement, child development and mental health services.
 
Essential Functions: 
<ul>
<li>Perform the functions of this position with a strong understanding of the Head Start Program Performance Standards in addition to local, state and other federal guidelines;</li>
<li>Supervise and provide support to family service staff at sites;</li>
<li>Monitor compliance using data management systems including PROMIS, WES, Pre-Kids;</li>
<li>Monitor the development and progress of Family Engagement Goal Setting;</li>
<li>Develop system of creating and submitting reports to management team that provide information related to ERSEA, family goal setting, services provided and any other information that may be requested;</li>
<li>Assist with coordination and facilitation of parent and staff workshops;</li>
<li>Advocate and collaborate with community agencies to support child, family and agency outcomes</li>
<li>Coordinate and facilitate Health Advisory Committee meetings in accordance with Head Start guidelines</li>
<li>Monitor and collect data for funding streams, monthly reports and annual PIR.</li>
<li>Perform other duties, as may be required.</li>

Minimum Qualifications: 
<ul>
<li>Bachelor's Degree in Human Services or a related field plus 5 years' experience working in Human Services, Psychology or related field that includes course work in principles of social work, child development, counseling and/or psychology;</li>
<li>Proficiency in Microsoft Office products;</li>
<li>Ability to plan and carry out assignments independently;</li>
<li>Ability to prioritize, adhere to timelines and to multi-task;</li>
<li>Willingness and skills to deal with sensitive family situations in a non-threatening and professional manner;</li>

The Program Coordinator- Child Health will join the pediatric primary care team to provide interventions, referrals, and follow-ups for families with patients between 0-5 years old. This role will report to the project director and provide Healthy Steps services to patients, parents, and caregivers. Additionally, this role will build strong relationships with families and providers to increase the efficiency of the medical system and support team-based comprehensive care.

This is a full-time temporary position, expected to last 6-7 months. Schedule: Monday-Friday, 8am-5pm. This position is onsite.

Responsibilities

Preventative And Responsive Care
<ul>
<li>Conduct consultations for needs regarding development and/or behavioral concerns such as sleep, positive parenting, picky eating, etc.</li>
<li>Maintains the child development support line, including responding and tracking call requests within the designated response time</li>
<li>Participates in reflective supervision meetings</li>
<li>Facilitates parent/caregiver groups to introduce program</li>
<li>Assists patients/parents to participate in their medical and/or behavioral healthcare by overcoming barriers to care</li>
<li>Deliver and assesses Modified Checklist for Autism in Toddlers (M-CHAT), Patient Health Questionnaire (PHQ9), and the Ages and Stages Questionnaires (ASQ2/3) tools recommended by the Healthy Steps program</li>
<li>Documents and share patient screening scores and barriers with primary care and/or behavioral health providers, including appropriate consult documentation is secured in patient medical records</li>

<li>Communicates and delivers easy to read information about health and wellness, self-help materials and community-based resources</li>
<li>Creates and maintains community resource directory and database</li>

<li>Works with the front office administrative staff to ensure the scheduling of the LCSW time is in coordination with provider visits, such as well-child visits</li>
<li>Engages in continuous quality improvement (CQI) initiatives to ensure the Healthy Steps model is delivered with fidelity</li>
<li>Collaborate with Healthy Steps implementation team to implement core components</li>
<li>Maintains open communication with members of the primary care team</li>
<li>Works with pediatric primary providers around care coordination, goal setting, coaching, and education about key aspects of a child&rsquo;s development</li>
<li>Facilitates training to medical professionals and practice staff regarding early childhood development, early relational health/infant early childhood mental health, and trauma-informed practice</li>
<li>Utilizes evidence-based approaches when engaging with patients to increase their sense of control over their health, such as motivational interviewing to promote collaboration between patients and their medical/behavioral care team</li>

Documentation &amp; Reporting
<ul>
<li>Documents all patient clinical activity and care coordination in electronic health records (EHR)</li>
<li>Maintains database required to meet Healthy Steps fidelity metrics, including internal and external database files and EHR reports.</li>
<li>Collaborates with Healthy Steps implementation team to complete required annual site reporting, including analysis of external database and EHR data reports</li>
<li>Tracks caseload to ensure capacity to deliver the Healthy Steps services within the risk stratified service delivery model, including ensuring exit criteria is met and families/children are elevated to higher levels of care or referred to community providers, as needed</li>
<li>Ensures documentation is accurate, helpful, and compliant with regulatory requirements and accreditation standards</li>

Qualifications

Education/Experience
<ul>
<li>High school diploma or GED required; Associate&rsquo;s degree or higher-level education in a related field preferred.</li>
<li>Current Basic Life Support (BLS) certification required upon hire and must be maintained as a condition of employment. These courses must follow AHA guidelines- may be completed through approved online providers such as ProMed or other equivalent programs that meet recognized BLS standards.</li>
<li>One year of early childhood growth and development, parent-child relational health, infant/early childhood mental health, or family systems experience required</li>
<li>Two years of experience working in a healthcare setting required</li>
<li>Experience working in outreach and customer service within a medical office or behavioral health environments preferred</li>
<li>Valid CA Driver&rsquo;s License and proof of auto insurance required</li>
<li>Bilingual (English/Spanish) highly preferred</li>

FunctionThe Front Desk Coordinator collaborates with the multidisciplinary child abuse investigative teams (MDTs) from Manitowoc, Ozaukee, Sheboygan, and Washington Counties to coordinate child abuse evaluations in the safe, neutral, and family-friendly facilities of Lakeshore Child Advocacy Center (CAC). This role is primarily stationed at the Sheboygan Child Advocacy Center, with occasional travel required to the Saukville Child Advocacy Center. The Front Desk Coordinator reports to the administrative team. Key responsibilities include collecting case information and scheduling cases at the center, providing clerical support to both staff and partners, handling bookkeeping tasks, and supervising the CAC waiting room.Responsibilities<ul><li>Carry out general clerical duties, including facilitating communication via phone, e-mail, and Microsoft Teams, as well as organizing both hardcopy and digital center materials.</li><li>Manage multiple calendars efficiently.</li><li>Collect intake information, schedule forensic interviews and medical exams, and ensure that all investigative partners are notified accordingly.</li><li>Maintain confidentiality in all matters concerning clients, staff, and agency information.</li><li>Collect, forward, and accurately record all financial records as directed by the administrative team.</li><li>Greet partners, families, and visitors, ensuring compliance with center policies.</li><li>Supervise the waiting room for children and family members.</li><li>Work collaboratively with the administrative team to recruit, interview, and manage volunteers for the center.</li><li>Ensure that the waiting room, kitchenette, and other common areas are clean, sanitized, and free from clutter.</li><li>Carry out other duties as assigned.</li></ul>Qualifications<ul><li>Graduation from a standard school, supplemented with business courses or other relevant training, or at least 2 years of related experience.</li><li>Background or education in child development and/or experience working directly with children and families.</li><li>Ability and comfort in interacting with children and adults receiving services at Lakeshore CAC.</li><li>Proficient computer skills with experience using Office 365 and Microsoft Teams.</li><li>Strong organizational skills for effective office management.</li><li>Excellent time management and multitasking abilities.</li></ul>Preferred Candidate Qualifications<ul><li>Experience collaborating with members of a multidisciplinary team, such as law enforcement, human services, victim advocacy, or the district attorney&rsquo;s office is strongly preferred.</li><li>Previous experience working in an agency providing services to children, caregivers, and professional groups.</li><li>Strong communication and interpersonal skills to form positive relationships with individuals of diverse backgrounds and experiences.</li><li>Ability and comfort in interacting with children and adults experiencing traumatic events.</li><li>Demonstration of competence in managing the potential impacts of vicarious/secondary trauma from ongoing exposure to oral, written, and evidence related to child abuse, child drug endangerment, family violence, and other child-related crimes.</li></ul>Position DetailsThis is a full-time position, requiring 40 hours per week.
