Duties include assessment, evaluation, service planning, and provision, as well as coordination of all other identified services.</p><p>1. Participate in the assessment and evaluation of all program referrals for CLTS and gather necessary documentation.</p><p>2. Complete CLTS functional screens and re-evaluate the functional level of care and eligibility at regular 6-month reviews and as needed with a client needs change.</p><p>3. Work with children and their families to develop an Individualized Service Plan (ISP) to address identified needs and ensure that services address person-centered outcomes.</p><p>4. Monitor ISP services and needs continually to ensure changes are made as needed.</p><p>5. Inform clients and/or families of their rights and responsibilities regarding services, privacy, and grievance procedures.</p><p>6. Coordinate services for all program participants, including coordination with therapists and other service agencies identified in the ISP with such services occurring in the child&rsquo;s natural environment.</p><p>7. Provide education, information, and resources to support clients and their families</p><p>8. Develop effective provider networks to ensure accessibility to all required services.</p><p>9. Assist with establishing, maintaining, and updating authorizations with service providers.</p><p>10. Calculate, invoice, and monitor cost-share payments required by the state for program eligibility.</p><p>11. Ensure all services provided meet program standard criteria.</p><p>12. With appropriate support and supervision, participate in collaborative case and treatment planning for children engaged in other programs such as child protective services or Comprehensive Community Services (CCS).</p><p>13. Maintain client records.</p><p>14. Complete all required paperwork and documentation of case notes for timely and accurate billing.</p><p>15. Coordinate services when a client is in crisis.</p><p>16. Develop programs and services to prevent abuse and neglect through supportive family services.</p><p>17. Inform all clients and families of how to report suspected abuse and neglect.</p><p>18. Prevent unnecessary separation of children from their families through intensive preservation services.</p><p>19. Help reduce service-related trauma to clients.</p><p>20. Assist with transitions out of an institutional setting; assist with relocation to community living settings.</p><p>21. Assist with completing financial eligibility forms for Medicaid as well as forms required for other programs, such as applying for disability determination and heating assistance.</p><p>22. Review billing and expenditures for accuracy and service plan alignment.</p><p>23. Complete annual CLTS foster care rates for billing.</p><p>24. Complete critical incident reports as needed.</p><p>25. Complete high-cost funding requests to DHS and act as a liaison between contractors and the state as needed.</p><p>26. Attend school IEP meetings and participate in the discussion of service needs; provide advocacy as needed.</p><p>27. Support and assist with transitional service coordination, including transition to high school and adult services.</p><p>28. Assist in coordinating work-related and vocational services for children when appropriate.</p><p>29. Preferred:</b> 60 hours college credit; 2 years as a Detective/Investigator with multidisciplinary teams on child abuse/neglect or similar. Experience using forensic investigation skills (interviewing suspects, collecting evidence, obtaining statements, report writing).</li></ul><p>
This position may be filled at a CPI Senior Investigator Trainee or a CPI Senior Investigator III. Factors such as education and experience may be considered when determining the classification to be used for hire.</p><p><b>
Additional Information:</b></p><div>
Applicants selected for this position are also eligible to receive a $416.66 monthly increase in addition to the base salary. Applicants hired into an investigative position will receive this increase the first of the month following 120 days of tenure. Once employed, the incumbent must remain in an investigative position to continue to receive the additional pay. If you leave Family Investigations or AR and return within the last 12-months, the INV stipend is given immediately. All other situations must wait the 120 days to receive the INV stipend.</div><div></div><div><b><br>
For APS/CPI/CPS Caseworker positions:
</b></div><p>This role requires frequent travel to homes and offices both within and outside your assigned region. Reliable transportation and a strong willingness to travel are essential, as you can expect to be on the road about 60% of the time. While much of your travel will be local, there will be instances&mdash;including last-minute situations&mdash;where you&rsquo;ll need to go outside your assigned counties, sometimes with little notice. Some overnight travel is also required, especially during initial training held outside your area and as cases demand. Flexibility is critical, as our mission to protect the most vulnerable means you must be ready to serve clients wherever and whenever you are needed.</p><p></p><p><br>

This position provides case management to families involved in the child welfare system due to identified child safety concerns. Drawing upon strength-based, trauma-informed practice and utilizing a family teaming model, works in partnership with parents and their family team to manage child safety while enhancing parents&rsquo; ability to keep their children safe, with a primary goal of keeping families intact and achieving safe reunification. Responsible for safety assessment, safety planning in in-home and out-of-home care placement settings, and case planning utilizing both formal service providers and informal supports. Utilizing critical thinking skills, case managers facilitate and monitor case progress towards permanency and safe case closure.</p>

Essential Functions:</p>

<li><p>Engages clients and focuses on establishing trusting relationships with parent clients and their children with the purpose of engaging and supporting parents through the case planning and behavioral change process.</p>

<li><p>Assess safety and determines plans in conjunction with case/permanency planning and service coordination to ensure child safety and child and family well-being.</p>
</li>
<li><p>Coordinates multi-disciplinary family team comprised of formal and informal supports, maintains ongoing communication with team members and facilitates family team meetings in order to monitor case progress.</p>
</li>
<li><p>Conducts in-home and community-based visits with children, families, and caregivers.</p>
</li>
<li><p>Facilitates placement into out-of-home care settings, establishes family interaction plans and supports caregivers in meeting the needs of children and youth in partnership with their families.</p>
</li>
<li><p>Documents case progress in the case record and completes court documentation.</p>
</li>
<li><p>Understands the role of Federal, State and tribal legislation as related to the Division of Milwaukee Child Protective Services in the day-to-day planning, protection and intervention for children and families. Collaborates with legal parties and represents child welfare perspective in court proceedings, providing testimony as needed.</p>
