The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. The Social Work Supervisor in the outpatient child psychiatry clinic participates as part of the child and adolescent psychiatry leadership team to facilitate clinical and administrative oversight of clinic operations.
Responsibilities<ul><li>
Clinical and administrative supervision of clinic social work staff with the goal of ensuring quality service delivery using evidence based treatments, monitoring, tracking and supporting staff in meeting visit targets established by the division, identifying training needs of staff and supporting ongoing trainings in evidence based practices.</li><li>
Management of evaluation/intake for new clients and case assignment with goal of balancing clinical and administrative mandates to ensure compliance with state and departmental regulations.</li><li>
Provides leadership and management to ensure that clinical documentation, services and clearances of all service providers meets the regulatory standards established by outside accreditation sources such as Office of Mental Health (OMH), Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Administration for Children&rsquo;s Services (ACS), New York State Justice Center and Division of Ambulatory Psychiatry.</li><li>
Participates as part of Ambulatory leadership team in the Division of Child and Adolescent Psychiatry and Ambulatory Psychiatry.</li><li>
Supports educational mission of the Division through oversight of the social work interns placed in the outpatient clinic, provides didactic teaching for trainees in psychiatry on psychosocial treatment of children, adolescents and families.</li><li>
Maintains an individual caseload of children and families at the clinic, in addition to having supervisory responsibilities.</li></ul>
Qualifications<ul><li>
NYS LCSW.</li><li>
Extensive knowledge and experience in providing and supervising evidence-based treatments with children and adolescents in a psychiatric setting.</li><li>
Knowledge of OMH, JCAHO, ACS and Justice Center regulations and the implementation of such in an outpatient mental health setting.</li><li>
Fluency in DSM V diagnostic classification system and ability to provide supervision and training on the application of this system with staff and trainees.</li><li>
Experience in developing training curriculum in evidence based psychosocial treatments for interdisciplinary trainees.

<ul><li>CFS Hotline Specialists answer and receive all abuse and neglect hotline calls and reports coming in via mail, eFax, website, and email.
</li></ul><ul><li>Gathers necessary information from incoming callers using professional interviewing skills and knowledge to seek collateral information from others. Provide customer service to callers, collateral contacts and professionals, Law Enforcement and other stakeholders that hotline staff come in contact with on a daily basis.
</li></ul><ul><li>Using critical analysis, the hotline specialist will determine the urgency of the call and ensure proper LE and HHS response to immediate safety for children and vulnerable adults. Provides de-escalation and assessment over the phone and referrals to community agencies, resources or other information as needed.
</li></ul><ul><li>May be required to work non-traditional hours, shifts and holiday. Assesses and ensures child, youth and vulnerable adult safety, permanency and well-being while making reasonable efforts to solidify, preserve, strengthen and maintain family unity.
</li></ul><ul><li>Reviews all accessible information in NFOCUS and other data sources to assess the history and other risk factors involved with the abuse and neglect report. Analyzes all gathered information and apply the SDM Intake tool to make a screening and priority decision and determine any immediate safety action needed. Contacts law enforcement as needed. Distribute all intakes to proper law enforcement and the local DHHS office
</li></ul><ul><li>Make all contacts needed for priority 1 intakes and ensure LE is informed of community crime and immediate safety issues. Provides technical assistance and support.

Requirements/ Qualifications
Minimum Qualifications : A Bachelor's degree in social work, psychology, sociology, counseling, human development, mental health care, education, criminal justice or another closely related area. Consideration will be given to applicants who are enrolled in the final semester of their Bachelor's Degree. Promotion to the Child and Family Services Classification is contingent on successful completion of degree requirements and required training.
Preferred Qualifications : 2 Years of child support experience, SDM IA and Intake trained, knowledge of NFocus. At least one year experience working children and families. Experience in child welfare and/or case management experience, internships with human services/child welfare agencies will also be considered, demonstrated ability to handle high levels of stress and emotionally volatile situations, problem-solving skills, and resiliency to provide excellent written and verbal communication, and ability to effectively manage time and deadlines. Experience with SDM (Structured Decision Making) and SOP (Safety Organized Practice) a plus.
Other : Ability to provide independent travel to occasional training is required. Evening/ Overnight Hours This is a P/T position.
This will include some Weekends and Holidays.

JOB PURPOSE/SUMMARY

Summary of role of team:
<ul><li>Works within the Children's CSU at East Tennessee Children's Hospital (ETCH), as well as the larger Children's Crisis Continuum.
</li><li>This position requires certification in and adequate implementation of verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.</li><li>
This position requires utilizing a personal dependable vehicle to conduct Center business.
</li><li>Maintaining a dependable vehicle and certified driver status is a condition of employment.
</li><li>CPR and First Aid Certification required.
</li><li>Mandatory to remain awake and alert during shift.</li></ul>

Summary of position:
<ul><li>Serves as a clinical member of the multi-disciplinary team at the Children's CSU housed at East Tennessee Children's Hospital (ETCH), providing treatment, rehabilitation, and support for client population.
</li><li>The CSU RN must be adept at multi-tasking.
</li><li>Provides direct medical care to clients in a CSU setting including clients in varying levels of crisis, frequently working with SPMI and clients with co-occurring issues.
</li><li>Assists with crisis interventions and medical emergencies, obtains vital signs each shift or more frequently as indicated. Administers intramuscular injections, communicate with PCPs during recovery partnership and transition phases, and liaison with other agencies as indicated.
</li><li>RN is responsible for triaging referrals, completing RN intake/assessments on all clients admitted during their shift, obtaining urine drug screens, BAL and pregnancy tests on admissions, obtaining admission orders, dispensing routine and PRN medications, taking orders from prescriber and updating MAR, completing electronic and paper charting in a timely manner and participating in treatment team meetings.
</li><li>Additionally, RN may take calls from recently discharged clients requesting bridge prescriptions, follow-up calls on weekends, obtain prior authorizations and coordinate with pharmacies.
</li><li>RN collaborates with Mobile Crisis and Family Walk in Center assisting with clients being referred to higher level of care, completing S-BARs.
</li><li>Required to be certified in and adequately implement verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
</li><li>Schedule may require 12 hours shifts and rotating weekends.

TYPICAL WORKING CONDITIONS/ENVIRONMENT

This position will be housed at East Tennessee Children's Hospital (ETCH) where the Children's CSU will be operating.
</li><li>Staff will be expected to work in and create a therapeutic environment while meeting the medical and facility requirements for operating a CSU.</li></ul>

JOB DUTIES/RESPONSIBILITIES

1. Daily Staff Duties
<ul><li>
Performs the initial admission medical assessment and completes required admission paperwork including nursing assessment, nursing note and nursing admission orders.</li><li>
Update medication tabs.</li><li>
Update allergy lists.</li><li>
Complete paper MAR.</li><li>
Obtain urine drug screen, BAL and pregnancy tests upon admission.</li><li>
Follows all established protocols and guidelines for admission prior to admitting the client to the unit.</li><li>
Makes the determination of the appropriate admission protocols needed for each client (ex.-agitation protocol, diabetic protocol, HTN protocol).</li><li>
Obtains admission orders from the on-call prescriber in a timely manner.</li><li>
Completes charting for each client during every shift prior to the end of shift.</li><li>
Completes transition paperwork and discusses medication and aftercare instructions with client and/or family. Provides copy of paperwork for clients.</li><li>
Counts narcotics and documents at every shift change. Notify supervisor immediately of any discrepancies.</li><li>
Obtains and charts vital signs on every shift.</li><li>
Monitor for significant changes in client's condition/behavior every shift and make referrals to obtain new orders as needed.</li><li>
Administer scheduled, routine, and PRN medication in a timely manner.</li><li>
Obtains blood sugars as ordered.</li><li>
Triages potential referrals with Mobile Crisis and Walk in Center.</li><li>
Coordinates arrival times for potential clients with Mobile Crisis, Walk in Center and Transporters</li><li>
Complete C-SSRS on each client</li><li>
Attend to guest and/or client needs during medical emergencies (ie: seizures, falls, etc)</li><li>
Coordinate with Emergency Departments and EMS</li><li>
Obtain Prior Authorizations</li><li>
Answer phone calls from recently transitioned guests regarding medication issues</li><li>
Weekly medication counts of routine medications</li><li>
Update Admission/Discharge log each time a client is admitted or transitioned from unit.</li><li>
Complete follow-up calls on weekends in accordance to program requirements.</li></ul>

2. Team Building
<ul><li>
Must be punctual and maintain a good attendance record.</li><li>
Participates in daily treatment team meetings.</li><li>
Participate in shift reports.</li><li>
Coordinate transitions with other staff.</li><li>
Maintains a positive and professional working relationship with all staff members and supervisors.</li><li>

Starting salary for this position is approximately $31.51 /hr based on relevant experience and education.

Schedule:
<ul><li>The Children's Crisis Stabilization Unit (CSU) will operate 24/7, 365 days a year.
</li><li>The holder of this position will be expected to work shifts that meet the needs of the program operation, which may include day, night, weekend, and holiday hours.
</li><li>Regular attendance is an essential job function.
</li><li>Mandatory to remain awake and alert during shift.
</li><li>Schedule may require 12 hours shifts and rotating weekends.</li></ul>

Travel:
<ul><li>There is no travel required for this position during shifts, but may require coverage at other sites, as needed and requested.</li></ul>

QUALIFICATIONS - Children's CSU RN
Experience / Knowledge:
<ul><li>Experience in psychiatric nursing preferred.
</li><li>Experience in working with special populations including individuals with Severe and Persistent Mental Illness and Co-Occurring Disorders preferred.
</li><li>Computer experience is required.
</li><li>Experience working in a crisis setting preferred.</li></ul>

Education / License:
<ul><li>RN with current Tennessee license and experience in psychiatric nursing preferred.
</li><li>BSN or ANA certification as Psychiatric/Mental Health Nurse preferred.
</li><li>Should have sound knowledge of psychiatric medications, diagnoses, treatment modalities and interventions.
</li><li>Ability to use computer.
</li><li>Be familiar with Microsoft Office and EMR.</li></ul>

Physical/Emotional/Social - Skills/Abilities:
