<p><b>Summary</b>
</p><p>The Direct Support Professional works under the direct supervision of the Client Care Manager and under the general guidance of the individual as they provide general habilitation and companionship care as indicated in their Individual Support Plan (ISP). Direct Care Support Professional, have an important role in allowing our clients to maintain their independence with an Everyday Lives focused care.
</p><p>Direct Support Professionals (DSP&rsquo;s) report directly to Client Care Managers (CCM&rsquo;s) and are expected to communicate with their CCM regularly, as needed, and be available during their scheduled shift, as necessary. Announced and unannounced home visits will occur two times annually during a scheduled shift in the client&rsquo;s residence. DSP&rsquo;s are evaluated by their Client Care Manager on their performance at 90 days and annually thereafter. DSP&rsquo;s are expected to maintain the integrity of JEVS Care at Home, and are expected to comply with all JEVS Care at Home policies, as found in the Caregiver Manual. Electronic Visit Verification (EVV) is a requirement of the role, and training on how to properly document services and timekeeping will be provided to the DSP. DSP is responsible for communicating any EVV compliance issues with their Client Care Manager (CCM).</p>

Required Education and Experience</b></p>

<li>Must be 18 years of age or older.</li>
<li><b>Must have a clean driving record, reliable vehicle with updated insurance, registration, and inspection.</b></li>
<li>High school diploma or GED preferred.

TCM Care Manager - Charlotte

RESPONSIBILITIES:</div><ul><li>
Conducts comprehensive assessments and on-going re-assessments of the client including psychosocial, physical and mental health, environmental and spiritual needs.</li><li>
Writes comprehensive assessments. Based on assessment information with the client develops and initiates the Community Living Plan, which is client-centered, comprehensive and consistent with program guidelines and policies and procedures.</li><li>

Conducts client intake and eligibility assessments and reassessments to identify client support needs.</li><li>
Creates and maintains detailed client profiles in the electronic client database and in SharePoint.</li><li>
In consultation with agency staff, coordinate community groups across Boulder, organize and deploy caregiver support groups and grief groups at multiple community locations, often occurring simultaneously during a short time period.</li><li>
Communicates and maintains positive relationships with assisted living/residential facility/community center staff.</li><li>
Collects pre- and post-surveys for support groups.</li><li>
Communicates about groups, such as writing up group descriptions for social media and other promotional materials, coordinating with the JFS Marketing team.</li><li>
Works to ensure proper completion of paperwork and adherence to grant eligibility and data tracking.</li><li>
Represents JFS at various community outreach and educational events as needed.</li><li>
Answers questions and provides linkages to various benefit programs and/or options for services that best meet the needs of clients and/or family members.</li><li>
Attends professional trainings, seminars, in-services, and other staff development opportunities.</li><li>
Actively participates in department and agency meetings in both Boulder and Denver locations, as well as ad hoc committees, as requested by the director or CEO</li><li>
Oversee the Boulder mainline voicemails and provide information, referrals, and information and assistance, as appropriate.</li><li>
Expand outreach to additional referral sources, including physical therapists, gerontologists, Council on Aging, faith communities, and other community partners aligned with the target population.</li><li>
Work with the Director of Boulder and the Development and Community Engagement Manager to launch a community awareness and outreach plan that includes presentations, small gatherings, tabling events, and intentional word-of-mouth engagement.</li><li>

Requirements:<p><br>
</p><p><b>Education/Experience:</b></p><ul><li>
Required bachelor's degree in social work, related field, or equivalent work experience.</li><li>
Preferred three plus years&rsquo; experience as a Social Case Worker for a public or private agency.</li><li>
Preferred two or more years of experience coordinating programs or a customer service-related position.</li><li>
Preferred two or more years of experience working with older adults, adults?navigating chronic health challenges, and family members?impacted?by these populations.</li><li>
Preferred two or more years of group facilitation experience.</li></ul><p><b>
Other:</b></p><ul><li>
Ability to provide good client and customer service. This job requires being pleasant with others on the job and displaying a cooperative attitude while being careful about detail and thorough in completing work tasks.</li><li>
Substantial working knowledge of Microsoft Office.</li><li>
