 Boulder County Community Services is seeking to hire a
 
 Client Case Manager.
 
 This position must be available to work non-traditional shifts and workdays. The case management role will be responsible for a caseload in all elements of one, or more, alternative sentencing programs currently utilized in Boulder County and the corresponding case management team. Current alternative sentencing programs include Electronic Home Monitoring, Day Reporting, Work Release and Community Corrections.
 
 
 Joe Pelle Center (JPC) Client Case Managers supports clients&rsquo; practical skills and tools needed to be successful in the program and upon reentry into the community. Emphasis is placed on treatment that specifically addresses criminogenic risk/needs by using social learning skill-building activities to assist with cognitive, social, emotional, and coping skill development, and utilizes Cognitive Behavioral Interventions.
 
 
 
 This position works four 10's or five 8's typically during the weekdays that very between early and last shifts. Would need to provide weekend coverage as needed.
 
 
 Two of the shifts need to be until at least 7pm each week.

 Exercises direct and indirect supervision of JPC clients and ensures compliance with program policies, procedures, rules, regulations, standards, local and state statutes, and best practices

 Coordinates in other areas of the JPC to ensure efficient and effective dissemination of information, scheduling of program related client activities inside and outside the facility and other program specific items that must be accounted for in daily facility operations

 Participates in off-site job/activity checks, substance use testing, random searches of property and person and documentation and conducts investigation of rule violations
 </li>

 Ensures that proper intake procedures, client evaluations and release from sentence reviews are computed in a timely manner and that related client updates are properly disseminated

 Responds to emergency situations involving clients both inside and outside of the facility and after standard work hours when necessary
 </li>

 Coordinates with courts, probation, and other law enforcement agencies regarding client assignment, participation, and compliance with sentencing programs, including DCJ standards

 Regularly participates in cross-program training to provide program coverage, on-call consultation, and technical assistance to staff.

 Position Summary
 
 
 
 The Case Manager are responsible for creating a positive and empathetic environment while assisting residents in accessing appropriate housing and services through one-on-one case management and coordination of services with partner agencies. The Case Manager develops individualized case plans that promote client progression towards securing housing, obtaining medical insurance, increasing income, and other needs to be self-sufficient and ultimately secure and maintain housing.

Clarkson Estates will provide supportive housing to 59 formerly homeless families, 35 formerly homeless young adults, 35 youth aging out of foster care, and 35 formerly incarcerated individuals. Programming will focus on economic development and workforce training, youth development and violence prevention, adult education and financial literacy, recreation, and health and wellness.

What The Case Manager Does:<ul><li>
Maintain professional relationships with clients and client confidentiality.</li><li>
Practice Universal Precautions/Standard Protocol &amp; Procedures.</li><li>
Comply with any and all Federal, State, City and CAMBA security and privacy polices intended to protect the security and privacy of individually identifiable health information.</li><li>
Review all documentation establishing tenants' eligibility for program and make file copies.</li><li>
Create and maintain tenant files.</li><li>
Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments.</li><li>
In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals.</li><li>
Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA.</li><li>
Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency.</li><li>
Recommend and implement strategies to persuade tenants to participate more fully in this process.</li><li>
Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes.</li><li>
Participate in case conferences.</li><li>
Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization.</li><li>
Provide all required information for weekly/monthly/quarterly/annual reports.</li><li>
May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building).</li><li>
Act as tenant liaison/tenant advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, etc.</li><li>
Schedule appointments for tenant with referral organizations.</li><li>
Escort tenants to appointments (educational, medical, social service, etc.)</li><li>
Assist tenants in completing applications for benefits and entitlements and may process applications on tenants' behalf.</li><li>
Follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability.</li><li>
Evaluate actual living conditions of tenants through home visits.</li><li>
May prepare marketing materials for the program.</li><li>
May prescreen tenants over the telephone for eligibility and may schedule intake appointments.</li><li>
