Manage product backorders, supply disruptions, and quality issues in collaboration with clinicians, sourcing, and suppliers to ensure continuity of patient care and minimize operational impact.</li><li>
Conduct financial modeling, cost-benefit analysis, and evaluation of reimbursement implications.</li><li>
Identify and execute cost reduction, cost avoidance, and efficiency opportunities using GPO contracts and supplier partnerships.</li><li>

GENERAL STATEMENT OF JOB</b></div><div>
The Care Manager Licensed Professional (&ldquo;Care Manager - LP&rdquo;) is responsible for providing proactive intervention and coordination of care to eligible Vaya Health members and recipients (&ldquo;members&rdquo;) to ensure that these individuals receive appropriate assessment and services. The Care Manager - LP works with the member and care team to identify and alleviate inappropriate levels of care or care gaps through assessment, multidisciplinary team care planning, linkage and/or coordination of services needed by the member across the MH, SU, intellectual/ developmental disability (&ldquo;I/DD&rdquo;), traumatic brain injury (&ldquo;TBI&rdquo;) physical health, pharmacy, long-term services and supports (&ldquo;LTSS&rdquo;) and unmet health-related resource needs networks. Care Manager - LP supports and may provide clinical transition planning assistance to state, and community hospitals and residential facilities and track individuals discharged from facility settings to ensure they follow up with aftercare services and receive needed assistance to prevent further hospitalization. Clinical Assessment, Care Planning, and Interdisciplinary Care Team:</b></div><div><ul><li>

Supports the care team in development of a person-centered care plan (&ldquo;Care Plan&rdquo;) to help define what is important to members for their health and prioritize goals that help them live the life they want in the community of their choice.<ul><li>
Ensure the Care Plan includes specific services to address mental health, substance use, medical and social needs as well as personal goals</li><li>
Ensure the Care Plan includes all elements required by NCDHHS</li><li>
Use information collected in the assessment process to learn about member's needs and assist in care planning</li><li>

Executes independent discretion and engages in business decisions for the Vaya Care Management Department that support initiatives to promote Vaya&rsquo;s integrated, whole-person care model for members.</li><li>
Serves as a collaborative partner in identifying system barriers through work with community stakeholders.</li><li>
Manages and facilitates Child/Adult High-Risk Team meetings in collaboration with DSS, DJJ, CCNC, school systems, and other community stakeholders as appropriate.</li><li>
Works in partnership with other Vaya departments to identify and address gaps in services/ access to care within Vaya&rsquo;s catchment.</li><li>
Participates in cross-functional clinical and non-clinical meetings and other projects as needed/ requested to support the department and organization.</li><li>

Participates in other high risk multidisciplinary complex case staffing as needed to include Vaya CMO/ Deputy CMO, Utilization Management, Provider Network, and Care Management leadership to address barriers, identify need for specialized services to meet client needs within or outside the current behavioral health system.</li><li>
Monitors provision of services to informally measure quality of care delivered by providers and identify potential non-compliance with standards.</li><li>
Ensures the health and safety of members receiving care management, recognize and report critical incidents, and escalate concerns about health and safety to care management leadership as needed.</li><li>

<p><b>Role Overview</b></p><p>The Care Manager (LPN) will work under the direct supervision of the Chief Nursing Officer</p><p>Under general supervision and medical oversight, the Care Manager is a licensed clinician responsible for their populations health management to improve individual overall health, reduce risk for significant events, and reduce claims utilization. This position is the first clinical contact for children, youth and families seeking referral and access to intensive level behavioral health services. The Clinical Access Specialist is responsible for determining eligibility and facilitating access to behavioral health services by using clinical and diagnostic assessment tools to gather child and family medical and behavioral health information, complete initial clinical assessment tool i.e. Child Adolescent Needs and Strengths and verify and determine eligibility for intensive behavioral health treatment services. Utilize Clinical assessment and engagement skills to meet with child, youth and family in the home, hospital or other community settings to gather demographic, clinical/situation information and assess for service need and intensity.</li><li>
Complete behavioral health clinical assessment tools including the CANS Assessment.</li><li>
Demonstrate familiarity with DSM criteria for clinical mental health diagnoses and formulate a clinical diagnosis based on client background and presenting behavioral health issues and concerns.</li><li>
Engage the child and family during the referral session, assessing for immediate safety needs and concerns and ensuring clinical support for the safety of the child throughout the referral determination process.</li><li>
Gather information from medical, behavioral health and other community providers for children/youth referred to support assessment for and access to services.</li><li>
Using an Electronic Health Record, Provider One, verify Medicaid eligibility for services.</li><li>
For referrals meeting access to care criteria, assess acuity level for response and coordinate referral assignment with identified clinical manager/supervisor.</li><li>
Communicate referral dispositions to Medicaid and BHO funders and obtain pre-authorization for those meeting service criteria. For those referrals not meeting criteria, report disposition information to funder in monthly reports.</li><li>
For referrals not meeting criteria for services, assist referent in resolving presenting concern, e.g. makes referrals to community resources, utilizing motivational interviewing and solution focused principles to resolve issue(s).</li></ul><div><b>
Manage access to services</b></div><ul><li>
Support and facilitate referral access in the community: represent Family Behavioral Health Services in the community by community outreach and collaboration. Develop and maintain strong and active relationships with funders, Behavioral health and Medical providers, Children&rsquo;s Administration, Juvenile Justice, community organizations, and other referral sources.</li><li>
Maintain on-going clinical contact with children/youth meeting access criteria for services pending clinical assignment, ensure appropriate and timely coordination, collaboration and support for behavioral health supports.</li><li>
Maintain contact with referents (community medical and behavioral health professionals) so that if youth is not immediately assigned, they are informed of the clinical intake process and authorization and approval for services. .</li><li>
Demonstrate flexibility and organization to respond to urgent referral requests in the home and community as needed, including flexibility to meet family and community needs after 5pm and on weekends.
</li><li>Maintain knowledge of community behavioral health and other support services to ensure all referents are linked to helping resources in their local communities from service referral to eligibility determination and disposition.</li><li>
Complete clinical assessment/referral documentation as required by funders and Medicaid, tracking client enrollment and referral documentation in clinical dashboard as needed for system wide behavioral health outcome reports.</li></ul><div><b>
