Your Job: 
The Care Coordinator will support MMG Primary Care Practices, Physicians, and staff (&ldquo;Patient Care team&rdquo;) in various quality driven projects and act as an extension of the Patient Care team to help MMG achieve quality driven goals. The Care Coordinator participates in planning, providing, and evaluating patient care at the appropriate skill level. The position has a duty to provide a standard of care that meets or exceeds that of a reasonably competent and knowledgeable Medical Assistant. The Care Coordinator works under the direction of the MMG Care Coordination Clinical Manager and in coordination with the Patient Care Team to outreach to patients whose clinical measures need to be addressed. This individual will partner with MMG Primary Care physician offices to help achieve benchmarks for patient care and maintain/improve quality of care delivery across multiple payers and patient populations.

Your Job Responsibilities: 
 
<ul><li>Educate and train Patient Care team on standardized operational workflows, developed in coordination with Care Coordination Clinical Manager, to close gaps in care and ultimately optimize quality metrics as measured by various value-based care programs</li><li>Partner with the Patient Care team to assist patients in completing health screens and address preventive care measures that are not yet completed</li><li>Support the Patient Care team as needed by outreaching to Patients with clinical measures needing to be addressed, to either address clinical measures over the telephone or scheduling an office visit for the Patients to address the clinical measures</li><li>Update preventive health screens and clinical measures that have been determined as completed in the EMR, as needed</li><li>Communicate regularly with interdisciplinary team members to coordinate/update a patient&rsquo;s care</li><li>Maintain accurate tracking log of patients who have been outreached and status of their preventive health screens and clinical measures</li><li>Participate in various quality driven initiatives within MMG and collaborate together to achieve high level patient outcomes</li><li>Submit data to external agencies as directed by Care Coordination Clinical Manager</li><li>Explain the importance of preventive health screens and clinical measures to patients and families</li><li>Perform as needed all duties at the expert level within the scope of a Medical Assistant&rsquo;s scope of practice</li><li>Record patient care documentation in the medical record accurately and in a timely manner</li><li>Coordinate patient care as directed by provider, and policies/procedures</li><li>Rotate clinic to clinic within MMG locations as assigned by Care Coordination Clinical Manager</li></ul></div>

JOB SUMMARY: The Clinical Care Coordinator provides assessment of consumers&rsquo; functioning and leads development and documentation of individualized rehabilitation plans based on the Recovery Model for adults who have mental illnesses.
ESSENTIAL DUTIES and RESPONSIBILITIES:<ul><li>
Provide services to consumers in accordance with the principles and practices of the ACT Fidelity Model.</li></ul>
 a. Lead the process of developing, updating, and guiding implementation of consumers&rsquo; Individual Recovery Plan (IRP), in collaboration with consumer, Team Leader and other significant parties of the ACT team.
 b. Coordinate treatment planning and discharge meetings at area hospitals to ensure a smooth transition to the community.
 c. Develop and maintain a professional and clinical relationship with area hospital staff to ensure a smooth transition for agency consumers to and from the hospital.
 d. Educate and assist consumers with learning or re-learning activities of daily living (ADL) skills necessary to improve the quality of life including acquisition of consumer health care benefits.
 e. Update Consumer&rsquo;s IRP, Functional Psychosocial Assessment (FPSA), Level of Care Utilization System (LOCUS), Global Appraisal of Individual Needs (GAINS) and Crisis Plan every 180 days.
 f. Provide an average of 4.25 hours of services per day to ACT consumers.
 g. Establish constructive relationships with consumers to promote acquisition of rehabilitative goals.
 h. Maintain adherence to all policies and procedures consistent with Dept. of Mental health (DMH) and the District of Columbia board of social work.<ul><li>
Participate in treatment team meetings and conduct follows ups as indicated in the following areas.</li></ul>
 a. Crisis assessment and intervention.
 b. Symptom assessment, management and individual supportive therapy.
 c. Assistance in the treatment of substance abuse for consumers with co-occurring addictive disorders.
 d. Psychosocial rehabilitation and skill development.
 e. Recommend and sometimes make decisions about the best course of action for a particular service user.
 f. Provide support services to ACT team and consumers.
 g. Develop IRP and other care plans as indicated. Conduct needs and risk assessment.
 i. Provide resolution and referrals services for consumers.
 j. Conduct psychosocial assessments using the Functional Psychosocial Assessment (FPSA) every 180 days for consumers served by the ACT team
 k. Education, support and consultation to consumers&rsquo; families and/or their support system, which is directed exclusively to the well-being and benefit of the consumer.<ul><li>
Develop and implement quality improvement (QI) activities for individual clinical staff members and the clinical aspects of the ACT team. Attend QI meetings as appropriate;</li><li>

SUMMARY OF POSITION
The Clinical Care Coordinator is responsible for managing a caseload of clients, documentation and charting in electronic health record, communicating with multidisciplinary teams, educating community on available programming, and ensuring adequate patient care, with a main focus on veterans, military and their families receiving quality outpatient counseling services. Responsibilities
<ul>
<li>Participate in the QOL, QAPI meetings conducted in the community.</li>
<li>Follow systems, nursing standards and guidelines.</li>
<li>Coordinate and oversee the Medicare Advantage procedures for the facility.</li>
<li>Timely and accurate completion of residents&rsquo; care plans/assessments/PASRR and take follow-up action as necessary.</li>
<li>Participate in the pre-admission process to determine potential residents&rsquo; level of care and participate in resident level of care determination upon admission.</li>
<li>Support the billing schedules and procedures.</li>
<li>Be familiar with residents&rsquo; condition and care needs and follow and educate community associates on the review process and provide recommendations to address potential areas of concern/opportunity, utilizing programs and reports on various state and company generated reports.</li>
<li>Provide education to community associates on POC documentation, change of condition alerts; late and mid-loss ADL&rsquo;s to support MDS and QM data elements.</li>
<li>Provides information to regional operations, clinical and community management team on the status of clinical and financial reimbursement.</li>

<li>Participate in clinical assignments for comprehensive reviews as part of community IDT.</li>
<li>Conduct audits of each communities EHR/documentation systems.</li>

Qualifications:
<ul>
<li>Current/valid Texas RN or LVN nursing license or a current/valid RN/LVN/LPN license from a Compact Party State. (The RN/LVN may continue to practice under the former home state license and multistate licensure privilege for a period not to exceed thirty days.)</li>
<li>Preferred Skilled Nursing Facility Experience a Management level nurse Traits/Skills/Other Requirements</li>

Position Summary

The Clinical Care Coordinator &ndash; NP will be responsible for working in the dynamic and innovative academic practice at NYP/Columbia University Medical Center. Rapidly growing department offers collaborative practice opportunities to establish an ambulatory care practice and participate in the development of new clinical services in the Department of Orthopedic Surgery.

Responsibilities
<ul>
<li>Work independently or collaboratively with the Attendings, Fellows, and Resident staff as a well as the nurses, therapists, social worker&rsquo;s and other support personnel who take part in caring for orthopedic patients.</li>
<li>Care coordination for patients needing non-operative treatment</li>
<li>Assist in managing pre-operative care across other practitioners to optimize patients for surgery</li>
<li>Obtain relevant consultations from other services and follow up on the information.</li>
<li>Peer-to-peer consults to obtain surgical pre-authorization for testing or surgeries for all spine surgical patients</li>
<li>Rounds independently and/or with attendings on all orthopedic inpatients.</li>
<li>Chart preparation according to protocol and orders appropriate x-rays for office hours using the Electronic Medical Record.
</li><li>Complies with Meaningful Use Measures.</li>
<li>Evaluates and diagnoses patients</li>
<li>Orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs.</li>
<li>Administers injections/Prescribes appropriate pharmacological and non-pharmacological treatment modalities.</li>
<li>Monitors effectiveness of prescribed intervention.</li>
<li>Remove/apply dressings, sutures, staples, slings and/or braces</li>
<li>See post-op patients for wound checks and post-op care and notify attending of findings if abnormal</li>
<li>Reviews post-operative discharge instructions with patients including regarding wound care and pain management.</li>
<li>Returns patient phone messages at designated times throughout the day; documents action taken in the Electronic Medical Record (EPIC).</li>
<li>Triage incoming telephone calls; instructs patient and family regarding treatment instruction, then documents action taken in the Electronic Medical Record (EPIC).</li>
<li>Oversees appointment bookings and ensures emergency situations are scheduled appropriately</li>
<li>Sends patient prescription requests to pharmacy and documents action in the Electronic Medical Record (EPIC).</li>
<li>Manage and oversee all spine attending in-basket tasks in EPIC
</li><li>Fill out necessary forms, i.e. disability forms, workman's comp, handicap parking as needed; Assist with patient correspondence as needed, i.e. letters of medical necessity, school notes, insurance inquiries, etc.</li>
<li>Assist patients in meeting health needs by identifying potential or existing problems, exploring available alternatives and providing support to the patient and family members to make an informed decision regarding health care.</li>
<li>Facilitates the patient's transition within and between healthcare settings, e.g. admitting, transferring, discharging and home care including visiting nurse referrals.</li>
<li>Formulate patient care protocols and critical pathways.</li>
<li>Assist with development; participate in design and implementation of clinical research trials.</li>
<li>Participate in accordance with hospital policies.

</ul>Minimum Qualifications
<ul>
<li>Bachelor's degree or equivalent in education and experience plus five years of experience</li>
<li>New York RN licensure, New York Adult Nurse Practitioner (ANP), or New York Nurse Practitioner (NP) licensure and/or New Jersey Nurse Practitioner (NP) licensure</li>
<li>National Certification &amp; DEA required</li>
</ul>
Preferred Qualifications
<ul>
<li>MS in Nursing preferred; other master's degrees considered.</li>
<li>Clinical doctorate</li>
<li>Prior experience in management of adults with orthopedic diseases in an Orthopedic Hip &amp; Knee practice, physical medicine and rehabilitation practice or pain management practice</li>
<li>CPR/AED certification</li>
<li>Bilingual (Spanish/English)</li>
