Murtha Cancer Center located at the Walter Reed National Military Medical Center, Bethesda, Maryland is a modern, patient-centric, tri-service military healthcare facility. Job Description:
The Clinical Data Abstractor II will be responsible for reviewing, abstracting, and analyzing clinical data from various sources, ensuring accuracy and completeness in accordance with regulatory standards and organizational protocols. Supports quality improvement, clinical programs, and patient care initiatives through chart abstraction, data collection, and other administrative tasks.
Key Responsibilities and Tasks:<ul><li>
Compare medical chart notes with HEDIS/PQA/IHA measure specifications and guidelines to determine if documentation meets requirements for quality measures.</li><li>
Extract and input clinical data from electronic health records, patient charts, and other relevant sources into quality reporting and analytics platform for measure compliance.</li><li>
Conduct supplemental data reviews according to quality program priorities.</li><li>
Ensure data accuracy, integrity, and compliance with established guidelines and standards.</li><li>

The purpose of this position of Registrar is to maintain the integrity of the Trauma Registry (inpatient, outpatient, and other established criteria), which contains patient care data from the following phases of care: prehospital, emergency department, hospital course, and final outcomes. The incumbent is responsible for assuring the accuracy of all components of the database, producing reports, and participating in complex database management processes.
The incumbent serves as a liaison to Patient Financial Services to support the billing of facility fees for the Trauma service, per UCDH, Federal, State, County, and/or third-party payer policies and guidelines as needed. The Trauma Program also has the responsibility for providing public and professional trauma education.
The incumbent is responsible for coordinating and facilitating the American College of Surgeons Advanced Trauma Life Support Courses offered at UC Davis Health.

Provide Trauma or Orthopedic Nurse Analyst / Abstractor Support</li><li>
Review source data from handwritten medical records as well as electronic sources such as MHS Genesis, Armed Forces Health Longitudinal Technology Application (ALTHA), Critical Care Air Transport Team (CCATT) records, ESSENTRIS, Joint Legacy Viewer (JLV), Patient Movement Records (PMR), Tactical Combat Casualty Cards (TC3), Theater Data Medical Store (TMDS), Web Interface Scanned Patient Records (WISPR) and other source data as provided by the Government and enter same information into the Department of Defense Trauma Registry (DoDTR) and/or the integrated clinical modules of the DoDTR in accordance with government provided business rules</li><li>
Collect comprehensive, accurate data relating to trauma patients by abstracting all trajectories of care</li><li>
Verify and update all information compiled, and enter data into the application in a timely manner</li><li>
Code injuries, procedures, and causes of injuries using standard medical record references, specifically International Classification of Diseases (ICD), Ninth and Tenth Revision (9/10) codes, assign diagnoses Abbreviated Injury Scale (AIS) and ICD-9/10-CM (Clinical Modification) codes, ICD-10-PCS and CPT codes, and complete Injury Severity Scale (ISS) and other calculations for an average of one hundred eighty (180) levels of trauma care per week or three hundred sixty (360) orthopedic procedures per week or 120 Infectious disease records per week with at least 90% average accuracy.</li><li>
Correct audited inaccuracies within 7 days of audit utilizing the government provided abstracting business rules</li><li>
Complete abstraction time and medical record documentation in JTSM daily</li><li>
Serve as a trauma and clinical subject matter expert to non-clinical personnel</li><li>

REQUIRED DEGREE/EDUCATION/CERTIFICATION:<ul><li>

<div>JOB SUMMARY: The primary purpose of this position is to ensure the accuracy, completeness, and integrity of the electronic medical record (EMR). This role is responsible for coordinating information between providers and internal departments to ensure patient documentation is thorough and up to date and abstracting relevant information from outside medical records into the EMR. The position requires accurate transcription of provider dictation, routing completed documentation to appropriate providers, and troubleshooting documentation or record issues as they arise.
