Job Summary and Qualifications</b></h5><p>
The Clinical Pharmacy Specialist is responsible for providing all levels of pharmacy clinical services. This includes monitoring patient medication regimens and providing appropriate interventions as well as supporting all aspects of pharmacy operations. Support the hospital&rsquo;s formulary decision process by working with medical staff to promote rational drug therapy and make improvements identified by drug utilization evaluations.</li><li>
Provide accurate and timely medication information to healthcare staff and patients.</li><li>
Actively participate in pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.</li><li>
Participate in antimicrobial management and other clinical programs.</li><li>
Document all clinical activities and interventions accurately and completely. Responsible for mentoring and education as well as ongoing analysis of interventions to ensure accurate and appropriate documentation and provide summary reports (if assigned)</li><li>
Support and participate in the goals of the organization&rsquo;s quality improvement, risk management, and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.</li><li>
Complete notification reports regarding medication errors and adverse drug reactions</li><li>
Participate in the data collection of drug utilization evaluations, adverse drug reactions, and pharmacy quality initiatives.</li><li>
Participate in the training of new pharmacists, pharmacy students, residents, technicians, nurses, and physicians.</li><li>
Serve as preceptor and resource for pharmacy students and residents.</li><li>
Maintain professional competency sufficiently to meet current and future needs of the facility.</li><li>
Has knowledge of the patient populations served and apply that knowledge in the care of those patients</li><li>
Provide basic technical support for automated dispensing machines and other pharmacy computer systems.</li><li>
Effectively supervises pharmacy operation activities and provides support to the whole pharmacy team.</li><li>
Compliant with facility pharmacy schedule to work physically at patient care areas to interact with medical staff, nurses, and other members of the healthcare team.</li><li>
Effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions.</li><li>
Train and participate with Disaster Team drills and real-event response as assigned.</li><li>
Train and participate with Code Teams as assigned. Obtain and maintain BLS-ACLS or PALS current certification as appropriate.</li><li>
Maintain appropriate controlled substance records. Work with the Pharmacy Director and appropriate manager to identify processes to meet or exceed all controlled substance diversion prevention and monitoring standards.</li><li>
Comply with all regulatory standards. Work with the Pharmacy Director to identify and develop processes to meet or exceed all regulatory standards.</li><li>
Prioritize work and lead others to comply with facility medication turn-around time policy and procedure.</li><li>
Identify areas for improved efficiency.</li><li>
Supervise and assist pharmacy technicians and other support staff effectively if/when assigned.</li><li>
Participate in establishing pharmacy goals and objectives and assist in their implementation.</li><li>
Active participation in at least one nursing, medical staff, quality, or pharmacy committee. Job Summary: </b>The Clinical Pharmacy Specialist serves as a leader in clinical pharmacy practice in a specific clinical area, or various clinical areas, as determined by hospital needs. Specialists provide comprehensive clinical and distributive medication management services and will utilize their advanced clinical skills in the provision of evidenced-based, safe, and cost-effective drug therapy to achieve optimal medication therapy outcomes. They serve as a drug information and educational resource for the pharmacy staff, patients, residents/students, medical staff, nursing staff, and other healthcare professionals. The Clinical Pharmacy Specialist will participate in the development of clinical guidelines/protocols, new programs/services, and facilitate professional development activities in their area of expertise. This position will be working Monday to Friday, working the day shift.</p>
<p><b>
Essential Responsibilities:
</b></p><p></p><p>Practices pharmacy in accordance with laws, rules and regulation of the Massachusetts Board of Registration in Pharmacy, Department of Public Health, U.S. Food and Drug Administration, and Drug Enforcement Administration and in accordance with established departmental policies and procedures. Maintains credentials in accordance with the Massachusetts Board of Registration in Pharmacy.</p>
<p>
Monitors patients in a specific unit/specialty service, including collecting patient-specific data, interpreting drug-related laboratory data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy.</p>
<p>
Takes part in the development, administration, and improvement of all ambulatory pharmacy programs and services. Independent development of drug use evaluations, ADR reporting, clinic development and management, and creation of business plans</p>
<p>
Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, physical assessments as related to drug monitoring and adverse effect management, review of medication profiles and the patient medical records.</p>
<p>
Documents clinical interventions in the electronic health record in a timely and professional manner.</p>
<p>
Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing.</p>
<p>
Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADR's, eliminating unnecessary drug use and duplication.</p>
<p>
Coordinates continuing education activities for medical and nursing staff; orients new employees on pharmacy practices and procedures. Includes submissions to the pharmacy newsletters, participating in journal clubs, and providing in-services to professional staff.</p>
<p>
Precepts PGY1 and PGY2 pharmacy residents, pharmacy students, and contributes to staff development in area of specialty practice. Works collaboratively with other clinical pharmacy specialists to develop the pharmacy skills of pharmacy staff members by teaching and exemplifying those skills to the team members.</p>
<p><b>
Required Qualifications:</b></p>
<p>
Bachelor's degree in Pharmacy required. Doctoral degree in Pharmacy preferred.</p>
<p>
Registration Massachusetts Pharmacist required.</p>
<p>
1-3 years related work experience required.</p>
<p>
Completion of PGY1 accredited pharmacy residency or 3+ years of Clinical Pharmacy experience in hospital setting.</p>
<p>
Must obtain one or more certification (Certified Specialty Pharmacist (CSP), Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP)), within 18 months of hire.</p>
<p>
Understanding of insurance and adjudication processes.</p>
<p>
Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.</p>
<p><b>
Preferred Qualifications:</b></p>
<p>
Pharmacy residency PGY2 specialty experience/fellowship or equivalent experience or 5+ years of clinical pharmacy experience.</p>
<p>
Knowledge of complex/chronic diseases states and therapies used within specialty pharmacy such as but not limited to: Primary Care, Oncology, Transplant, Infectious Disease (including HIV), Multiple Sclerosis, Rheumatoid Arthritis, Gastroenterology, Hepatology, Dermatology, Cardiology, Pulmonary, Allergy and/or various inflammatory conditions preferred.</p>
