Essential Responsibilities:<ul><li>
Provides professional patient care for adults, prenatal and pediatric patients to the full scope of their license as indicated in a Federally Qualified Health Center (FQHC). Serves as clinical resource for site support staff.</li><li>
Prioritizes and collaborates with the Operations Manager to direct support staff workflow.</li><li>
Monitors and maintains undocumented vaccine, medication, lab and procedure reports.</li><li>
Initiates and maintains daily huddles with staff and documents agenda, participants and action item and maintains huddle book or other form of communication for all to review.</li><li>
Along with Operations Manager and lead provider, Clinical Lead assures that all quality measures identified by PrimaryOne Health are being met.</li><li>
Review patient charts daily for accuracy and thoroughness (meds and procedures documented).</li><li>
Under guidance and direction from Clinical Manager, educate and train MAs and LPNs on an on-going basis to improve knowledge.</li><li>
Maintains communication with patients concerning treatment progress and diagnostic reports. Coordinates care with other health services teams as appropriate.</li><li>
Oversees immunization inventory and accountability and ensure all expired medications are disposed of according to guidelines at the discretion of the Clinical Process Supervisor</li><li>
Participates in clinical evaluations for new and established employees.</li><li>
Identifies and coordinates referrals to community resources regarding patient needs.</li><li>
Documents all professional activities, contacts and observations promptly and appropriately.</li><li>
Attends internal and community meetings as assigned and participates in Quality Improvement activities as assigned.</li><li>
Provides oversight of EHR inbox and documents weekly report of inbox status with detailed explanation of any messages/documents falling outside of the guidelines.</li><li>
Will collaborate with the Operations Manager to complete inventory and stock, manage the ordering list and initiate orders for clinical supplies.</li><li>
At the direction of the Clinical Manager and Operations Manager will oversee the monitoring, tracking and documentation of CLIA waived quality assurance testing and sterilization monitoring (ex: urinalysis, A1C, autoclave, etc.).</li><li>
Oversee and maintain crash cart monthly quality and inventory monitoring.</li><li>
Occasional travel required.</li><li>
Other duties or special projects as assigned.</li></ul>
Successful candidates must have a High School Diploma or equivalent; current licensure as a Practical Nurse in the State of Ohio or current documentation of completion of a Medical Assistant program or its equivalent. Minimum two years' experience in ambulatory care and public health preferred; two years' experience working with Electronic Health Records system required.

Coordinates all inductions and c-sections for the obstetrics patients.</li><li>
Coordinates with HR with for recruiting, hiring, retention, staff training and continuing education, performance development, performance management, and teammate relations (measured by eNPS scores).</li><li>
Support the PM/HR in achieving NPS benchmarks by monitoring Google and Podium reviews; take swift action to resolve patient issues that arise in the practice in partnership with the clinical leadership team.</li><li>
Create, monitor, and optimize provider EMR templates.</li><li>
Physical site maintenance responsibilities including building maintenance issue reporting, inspections and maintaining cleanliness standards that meet healthcare guidelines. Complete the required CLIA with provider assistance for a review three times annually.</li><li>
As needed triage patients for visit with physicians, prepare patients for and assist with examinations or treatments. Administer and documents in the chart any immunizations or minor treatments, assist physicians with patient examinations, diagnostic tests and/or procedures.</li><li>
Uses a team approach to develop and achieve daily patient-centered individualized care goals.</li><li>
Oversees the triage department, this department is fully self-sufficient and implements telephone triage, patient education, physician-based care calls, pharmacy calls, portal communication with patients, and all task assignments from physicians and care teams through EMR system.</li><li>
Leads the team in cleaning and stocking all patient rooms, oversees medical supplies needed and reports for ordering, cleans and prepares instruments for procedures, orders lab tests, counsels patients concerning lifestyle modifications, medication regimens, side effects, and interactions using an appropriate communication.</li><li>
Coordinates ordering test/procedures with other clinical staff and oversees that all diagnostic tests results are reported to physician.</li><li>
Support PM/HR with channels of communication within the practice such as daily huddles, monthly team member meetings, 1:1 coaching sessions and assisting with townhall sessions.

The person in this position must be able to remain in a stationary position when checking in patients or when operating the register</li><li>
The person in this position must be able to observe and assess patient entry via cameras and inspect patient IDs upon entry</li><li>
Frequently communicates with other staff via Walkie Talkie or Headset</li><li>
Constantly positions self throughout sales transaction by maintaining the computer/POS, collecting product and retail bags</li><li>
Constantly moves and transports dispensary products/totes up to 30 lbs. throughout the dispensary</li></ul><div></div>
