<p><b>Company Overview</b><br/>True North Evolution is a licensed residential treatment center located in the beautiful Green Mountains of Central Vermont, one hour south of Burlington, Vermont. True North is a small, independently owned program, providing personalized therapeutic interventions and transition support for young adult clients with an emphasis on assessment, wellness, community engagement and family participation. This is an excellent opportunity to work for a nationally recognized therapeutic program, be part of a dynamic, supportive team and live and work in a fantastic community.</p><p><b>Job Summary</b></p><p>The Clinical Therapist is responsible for overseeing comprehensive client care within a therapeutic program, managing a caseload through individual and group therapy, treatment planning, and ongoing progress monitoring. This role involves close collaboration with families, referring professionals, and multidisciplinary team members to ensure coordinated, client-centered care. The therapist facilitates communication, supports client transitions, and integrates supplemental therapeutic services as needed.</p><p>In addition to direct clinical work, the therapist contributes to the overall treatment environment by supporting staff, guiding client care strategies, and fostering a positive community culture. Responsibilities also include documentation, participation in program development, client enrollment support, and involvement in on-call rotations and professional outreach.</p><p><b>Responsibilities</b></p><p>Oversees client care for each client on the caseload:</p><ul><li>Conducts a minimum of 2 individual sessions (in-person) with each client per week.</li><li>Oversees the completion of the TNE curriculum.</li><li>Conduct group therapy sessions.</li><li>Communicates by phone and email with parents/sponsors of each client on their caseload a minimum of one scheduled hour per week as well as any required follow up communication related to client or family concerns, logistical needs, and transition support.</li><li>Supports appropriate correspondence between the parents/sponsors and their clients. This includes facilitation of written correspondence as well as scheduling telehealth and zoom sessions between clients and family members, both with and without therapist facilitation.</li><li>Communicates by phone and email with educational consultants and other referring and consulting professionals to coordinate care, gather information, and provide updates and recommendations.</li><li>Collaborates regularly (at least once per week) with the Parent Program Therapists who are working with the clients on their caseload.</li><li>Maintain awareness of client medication needs and communicate as needed with the nurse, medical director and other treatment team members, parents/sponsors and other relevant professionals.</li><li>Coordinates psychiatric care.</li><li>Facilitates a collaborative transition planning experience for clients together with their families and referring/consulting professionals.</li><li>Assesses need/interest in supplemental therapies and creates treatment plans that may include referrals for EMDR, somatic breathing, infrared sauna/cold plunge therapy, acupuncture, chiropractic, physical therapy, and personalized support with diet and nutrition, exercise plans, and sleep hygiene.</li><li>Assesses psychoeducational testing needs and coordinates testing with appropriate professionals.</li><li>Participates in client graduations.</li><li>Completes individual treatment plans, phone logs, case notes, final reports, and other paperwork in a professional and timely fashion.</li></ul><p>Oversees the emotional and physical well-being of their clients and contributes to a positive, supportive and connected culture within the TNE community as a whole:</p><ul><li>Provides guidance and support to guides working directly with clients.</li><li>Offers perspective by detailing treatment outcomes for clients and for the group.</li><li>Coordinates with direct care and operations staff to ensure appropriate activities and interventions are appropriate and aligned with individual client treatment outcomes.</li><li>Provides feedback for guides and supports their ongoing growth and professional development.</li></ul><p>Contributes to the Treatment team:</p><ul><li>Available to facilitate client enrollments. Clinical Therapists are present for the first 15-30 minutes of a client&rsquo;s enrollment to offer immediate clinical assessment as well as support and a thorough and sensitive introduction to all clients and their families.</li><li>Represent the program at conferences and other professional events.</li><li>Participate in program and curriculum development.</li><li>Participate in the regular on-call rotation. This requires clinical therapists to be available by cell phone for one week at a time for as needed clinical assessment or support. The African American Healing Center is working with clients to prevent and reduce violence in our communities. Services are designed to meet the needs of people with all backgrounds, while proactively serving communities of color with programs that honor African American history and that are sensitive to the cultural context of each client.

The Clinical Therapist works collaboratively with a team consisting of Substance Use Disorder (SUD) Counselors to provide valuable insight, direction, and oversight of Drug Medi-Cal (DMC) services. Under the DMC model, they are qualified to establish medical necessity and approve SUD treatment plans. Maintains an assigned caseload of clients seeking substance use treatment and mental health treatment. In this position an individual would conduct face-to-face assessments, individual counseling sessions, and progress reviews with each client. Additionally, they are responsible for providing safe, supportive, and confidential counseling; maintaining a culturally competent stance; and creating and maintaining a trauma informed program environment at all times.

<li>In collaboration with supervisor, develop and maintain a self-care wellness plan (strategies to maintain physical, psychological/mental, emotional/relational, and spiritual health) to prevent compassion fatigue.</li>

<b>Documentation Responsibilities:</b><br>

<li>Daily maintenance of monitoring of EMR report, to prevent and clear any compliance errors in records.</li>
<li>Conducts assessments, develops treatment plans, completes release and consent forms, billing and reporting paperwork in hard copy and online in various electronic systems.</li>
<li>In addition, completes assigned Drug Medi-Cal, Medicare and Medi-Cal documentation and billing.</li>
<li>Documents individual and group services delivered in various electronic systems in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.</li>
</ul>
<b>Other responsibilities:</b><br>

<li>Schedules weekly team meetings and creates the agenda in coordination with the administrative assistant or other assigned individual.</li>
<li>Ensuring timely response to required compliance/documentation issues.</li>
<li>Ensuring completion of all action/follow-up items and overall clinical consultation.</li>
<li>Responsible for reviewing and signing initial treatment plans and medical necessity forms and adhering to strict DMC timelines.</li>
<li>Works as part of an integrated, multi-disciplinary treatment team.</li>
<li>Maintains an assigned caseload of clients seeking substance use and mental health treatment.</li>
<li>Conducts face-to-face assessments, individual sessions, and progress reviews with each client.</li>
<li>Assesses, manages, and deescalates client mental health crises.</li>
<li>Facilitates group sessions focused on substance use recovery, trauma recovery, co-occurring mental health symptoms, and other personal change topics.</li>
<li>Utilizes principles of trauma-informed care and harm reduction to attend to client milieu, including crisis assessment and management.</li>
<li>Proactively links clients with available internal and external resources to develop/maintain treatment plans, transition/discharge plans, progress notes, and appropriate updates in support of the health and recovery needs of each caseload client.</li>
<li>Attends regular group and individual supervision, trainings, meetings, and events related to furthering professional development.</li>
<li>Responsible for documenting all individual and group counseling sessions.</li>
<li>Maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.</li>

QUALIFICATIONS

Education, Certification, and Experience
<br><ul>
<li>BBS Registered with Board of Behavioral or Board of Behavioral Sciences (AMFT, APCC, ACSW).</li>
<li>Some experience in a substance abuse treatment or community mental health setting.</li>
<li>First Aid and CPR certification.</li>
<li>A valid California driver’s license</li>
<li>Experience with Drug Medi-Cal operations preferred.</li>
</ul>
Knowledge

<li>Experience with mental health assessment and diagnosis.</li>
<li>Experience establishing supportive relationships and working with clients from culturally and linguistically diverse backgrounds.</li>

<li>Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.</li>

What you will do:</div><ul><li>
Conducts psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus.</li><li>
Contacts appropriate previous and current tertiary caregivers to obtain relevant collateral clinical data that may be needed for the patient&rsquo;s coordination of care, treatment planning and discharge planning.</li><li>
Develops, implements and monitors progress of the patient&rsquo;s individualized treatment plan.</li><li>
Conducts daily group and individual assessments to identify issues for treatment focus by interviewing the patient one-on-one and transcribing relevant information.</li><li>
Provides individual, group and family process-oriented counseling sessions. Conducts educational, after care support and other didactic groups for patients and family members using various professional treatment modalities.</li><li>
Participates in regular clinical team meetings/staffing and provides input on the psychosocial, disease management and case management needs of the patient.</li><li>
Demonstrates an understanding, and adheres to all procedures and legislation, which apply to patients cared for in a Mental Health and Substance Abuse treatment facility.</li><li>
Demonstrates a comprehensive knowledge of community resources (12 step support meetings, recovery homes and residential treatment facilities) and medical allied staff expertise to assure an appropriate discharge liaison is implemented.</li><li>
Reports to community/state agencies any patient issues that are required by hospital policy and state or federal law. Serves as the facilities representative in legal proceedings resulting from said reports.</li><li>
Maintains accurate and timely medical records including psycho-social assessments, family session and daily progress notes to include school personnel and other community collateral involvement. Provides descriptions of therapeutic interventions and patient response for discharge planning process.</li><li>
Schedules, attends and documents court hearings as required, such as involuntary admission, guardianship, etc.</li><li>
May provide backup services for incoming assessment and referral calls.</li><li>
Participates in activities that enhance professional growth and development. May serve as a spokesperson before professional or community groups.</li><li>
