Job SummaryThe Community Ambassador Team Leader is responsible for oversight and effective implementation of programs sponsored by Ambassadors 4 Mental Health Access (A4MHA). This role is responsible for coordinating communication and facilitating the sharing of resources and program updates among A4MHA Liaisons. The Team Leader also manages the collection, analysis, and reporting of both qualitative and quantitative data related to program performance and impact.This is a two-year grant funded position, 24 hours a week(potential opportunity to expand to 35 hour a week position in future) Principal Duties and Responsibilities1. Daily oversight of the A4MHA initiative.2. Provide technical assistance, guidance, and support to A4MHA Ambassadors and Liaison.3. Support the coordination of a Community Impact Board.4. Ensure that the principles of a wellness and recovery-based service system are understood and modeled by all staff and promoted to the entire community.5. Additionally, these principles must be incorporated in the delivery of all A4MHA workshops, classes, and activities.6. Must possess an ability to interact with difficult and disruptive service participants in a calm and empathetic manner to ensure the safety of the individual as well as the other group/activities members (customer service/ boundaries/empathy).7. Work with management team to establish policies and procedures that promote wellness principles and embrace diversity and cultural awareness.8. Maintain strong relationships with community, partners and wellness consumer resources.9. Develops and maintains professional relationships with community stakeholders and providers to ensure program visibility and cooperative coordination of service delivery.10. Implement, monitor &amp; maintain A4MHA data. Including monthly technical assistance calls with the funder.11. Coordinate all technical aspects of A4MHA service coordination (Microsoft teams/excel/word/PowerPoint, Canva, Zoom, Survey Monkey, Constant Contact, Go-Mo, Facebook, Instagram, TikTok etc.)12. Possess the ability to collect, input into the computer and maintain pertinent data, develop reports and take an active part in development of quality improvement plans.13. Provide educational opportunities for A4MHA Ambassadors to expand knowledge of all wellness and evidence-based modalities utilized as part of the A4MHA.14. Must be able to follow agency policy and procedures, maintain structure for A4MHA program staff and follow policy and procedure.15. Follow budget guidelines, submit all paperwork and receipts, and adhere to policies in relation to all fiscal matters related to the program.16. These community beacons act as living libraries of local knowledge and serve as trusted guides for their peers. Through training and support, ambassadors are empowered to identify and refer to vulnerable at-risk individuals who live independently but may be silently struggling. These are often marginalized adults with limited or no support systems, whose emotional, mental health or physical challenges place them at serious risk of isolation or losing their independence.The genius of the A4MHA model lies in its simplicity: ambassadors are not expected to act as counselors or social workers, nor are they asked to alter their everyday responsibilities. Instead, they are trained to recognize warning signs&mdash;subtle or visible red flags such as expressed conversations, illness, confusion, poor hygiene, isolation, or sudden changes in behavior&mdash;that may indicate an individual is struggling. Once identified, A4MHA will work with MHA Community Impact Zone (CIZ) liaison and team leader to refer the individual to the appropriate service providers for support, evaluation, treatment and follow-up.Our ambassadors will serve as non-traditional referral sources&mdash;individuals who interact with vulnerable populations during their everyday lives. These may include barbers, postal workers, bodega owners, faith or civic group volunteers, bank tellers, or concerned neighbors. This energetic and adaptable Community Guide is an integral part of Reema, a start-up working to transform how health care and social care work together. Their work is completed daily through an integrated technology platform to call and text members, schedule community visits, as well as document all interactions. The Guide will be responsible for the full lifecycle of their caseload, including initial outreach to engage members in services, ongoing work with members to achieve their wellbeing goals through connection to health care and community resources, and graduation from the program. A primary strength of the Guide should be excellent verbal and written communication, building strong meaningful relationships, problem solving, and a comfortability in navigating the ambiguous nature of a constantly growing and evolving startup.

This position requires significant time in the field, directly interacting with members. This role is most suitable for someone who is confident in interacting directly with many people, many of whom have experienced trauma and face barriers because of their mental, physical and chemical health.

<li>Through cold outreach create and manage a caseload of 60 - 100 members and provide coverage for other Community Guides during periods of absence or increased workload.</li>
<li>Understand overall goals for the month. Independently organize and prioritize workload with effective time and task management skills to achieve monthly goals and outcomes.</li>
<li>Actively listen, empathize, and appreciate the vulnerability of members willing to share their challenges. Community Guides will maintain professional boundaries and confidentiality, seeking consultation and support with complex member situations from the multidisciplinary team.</li>
<li>Visit members in person as appropriate. This may be in the member's home, community, the hospital or other treatment setting to build trust, maintain connection and meet member's needs.</li>
<li>Utilize text, phone, email and face-to-face interactions appropriately to match your communication approach with each members' preference and situation.</li>
<li>Effectively identify and connect members to community and health resources to support member needs. This may include transporting members and accompanying them to access other services. Establish relationships with resources to support Reema's members as needed.</li>
<li>Prepare members for services ending by keeping self-sufficiency and independence top of mind.</li>
</ul>
Complete accurate and thorough documentation and record keeping in a timely manner:

<li>Utilize the Reema App to thoroughly document all work with members in a timely fashion, including keeping a care plan updated.</li>
<li>Understand and comply with confidentiality of protected health information and HIPAA.</li>
<li>Support member records to be thorough and accurate in Reema's technology.</li>
</ul>

Education, Experience and Skills:
<ul>
<li>Bachelor's degree in Social Work, Human Services, Psychology, Health Sciences or other related fields. Years of related experience and subsequent expertise can be substituted for years of completed education towards a bachelor's degree</li>
<li>Three or more years of direct experience with population or community to be served and knowledge of community</li>
<li>Driven to work with individuals who have complex health needs and believe that recovery is possible</li>
<li>Demonstrated experience with crisis planning, case management, boundaries and personal safety in community work</li>
<li>Proficiencies in critical thinking, time management, strategizing, and multitasking</li>
<li>Strong propensity towards building and maintaining relationships through initiating conversations and creating connections</li>
<li>Ability to develop, adapt, and execute outreach and care plans</li>
<li>Demonstrated proficiency working with technology, including basic knowledge of all related computer and software applications such as Microsoft Word, Outlook and Excel, and Video Conferencing required; G-Suite preferred</li>
</ul>

<li>18 years of age or older</li>
<li>Qualified applicants must have flexible and reliable access to transportation, a valid Driver's License, Vehicle Insurance, and a satisfactory driving record</li>
<li>Offers are contingent upon passing a pre-employment drug screening and background check</li>
<li>Comply with federal, state and local regulations regarding patient confidentiality HIPAA</li>
<li>Use technology-based tools and systems daily</li>
<li>Lift and carry up to 25 pounds occasionally.</li>
