Hale Kipa is looking for a Wraparound Facilitator/Care Coordinator to facilitate and coordinate wrapround meetings and services. The Wraparound Facilitator/Care Coordinator is responsible for overseeing the development of the wraparound team and orienting team members to the wraparound process. Under the direction of the Clinical/Cognitive Behavioral Therapy (CBT) Coordinator, the Programs Facilitator administers a library of evidence-based cognitive behavioral programs and ensures those programs are delivered in the intended in-person and/or virtual format. This position is not covered as defined by The Board of Commissioners of the County of Allen Employee Handbook.
<br><br>
<b>ESSENTIAL FUNCTIONS:</b><br>
<ul>
<li>Facilitates evidence-based cognitive behavioral classes and ensures the content of the program curriculum is delivered in its intended format and the participants understand and relates to the required material.</li>
<li>Prepares necessary lesson plans, materials, supplies and instructional hardware to facilitate the program and recommends additional materials needed.</li>

<b>PERSONAL WORK RELATIONSHIPS:</b><br>
The Programs Facilitator maintains frequent contact with other County employees, Allen County Community Corrections (ACCC) staff, program participants, and other mental health and social service agencies regarding evidence-based cognitive-behavioral therapy programs, including CBT programming conducted through an E-learning platform.

<b>WORKING CONDITIONS:</b><br>
The Programs Facilitator may work remotely and in an office and classroom environment with frequent contact with potentially dangerous, violent, and abusive program offenders convicted of various misdemeanor and felony offenses on a daily basis with the ability to move around freely, but frequently sitting, requiring some bending, kneeling, lifting up to forty pounds, reaching overhead and pushing and/or pulling loads. Very frequent typing, including proofreading and attention to detail, monitoring equipment and detailed inspection is required. <p>The Team-Based model of care program named <b>EMPOWR</b> (EMbracing, Progress and Opportunities, We Rise) aims to support transition aged youth (TAY, 16-24) attain mental health support, housing stability, employment and education gains, and community connections. The program provides community-based therapy, case management and care coordination, and peer and family support through a team-based model. The whole team collaborates on one shared caseload, providing youth with an entire team of support, and allowing staff to share in the successes and challenges of their clients.</p>
<p>
The <b>Intensive Care Coordinator (ICC) </b>provides support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth's natural supports together. This position<b> </b>is responsible for engaging and assisting foster youth and other clients, family members, foster parents, and other support persons in forming a Child and Family Team (CFT). The CFT works with identified support persons from natural supports (friends, extended family, neighbors, community members) and formal system partners (child welfare, education, juvenile probation, etc.) to comprehensively address the child and family's identified goals.</p>
<p>
The position<b> </b>draws on lived experience to provide support and guidance to family and community members who are a source of strength for the youth. They help bring other providers and the youth's natural supports together. Because of their lived experience, they help create a connection with family and other close contacts, and offer a unique perspective to the rest of the team.</p>
<h3 class="jobSectionHeader"><b>
How you'll make an impact:</b></h3>
<ul><li>Outreach to and engagement with youth; being a strong advocate for the youth within the team</li>
<li>Provide community-based trauma-informed services to foster youth or former foster youth transitioning to or struggling with independent living.</li>
<li>Provide intensive skill building with youth, with a focus on independent living.</li>
<li>Provide assessments, crisis intervention, stabilization, and safety plans.</li>
<li>Identify and connect youth &amp; young adults with other agencies to address their needs, including housing, medical, mental health, education, employment, advocacy, placement stability, and enrichment activities.</li>
<li>Work collaboratively in partnership with the team to engage and connect clients to appropriate resources and services.</li>
<li>Assist with initial contact and intake, establishing relationships and building trust in order to help clients navigate systems for access to services.</li>
<li>Provide advocacy, targeted case management and community based rehabilitation services to children and their families.</li>
<li>Provide services and participate in agency meetings in a combination of environments including virtual (zoom / phone) and in-person office or community locations (drop-in center, client homes, school, parks, etc.)</li>
<li>Provide stabilization support services to children and TAY in an in-person milieu environment.</li>
</ul><h3 class="jobSectionHeader"><b>Qualifications:</b></h3>
<ul><li>Direct, lived experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources.</li>
<li>Bachelor's degree in Psychology, Criminal Justice, Social Work or related field (preferred)</li>
<li>Experience providing case management and rehabilitation services.</li>
<li>At least two (2) years experience working with system-involved youth.</li>
<li>Demonstrated ability to work with families who have experienced high levels of trauma and demonstrate high mental health needs.</li>
<li>Ability to work independently and collaboratively as part of a multidisciplinary team.</li>
<li>Proven ability to be organized, manage time well, complete high quality and timely documentation.</li>
<li>Ability to travel by car to meet with clients, generally within a 90 mile radius of the WCC office. <b>Clean CA DMV record, current car insurance and a vehicle; travel is required.</b></li>
