Job Summary: 
The Home and Community‑Based Services (HCBS) 1915(i) program provides long‑term, community‑based supports 
for individuals with significant behavioral health, cognitive, or functional needs so they can remain safely in their homes 
and communities. HCBS includes person‑centered planning, skills training, personal care, supported housing and 
employment, and coordination across medical, behavioral health, and social service systems. Case Managers ensure 
members receive the right services at the right time, reduce barriers to care, and support stability in the least restrictive 
setting.

HCBS Case Managers are responsible for delivering trauma‑informed, person‑centered care coordination that meets 
OHA and CMS requirements. Core responsibilities include: 
Responsibilities:

These responsibilities require strong clinical judgment, excellent communication skills, and the ability to coordinate

Qualifications:

Responsibilities:<ul><li>
Completes all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards. Assists in preparing reports as required.</li><li>
Conducts initial and on-going assessment of client's health and/or support service needs. Sets level of client need.</li><li>
Creates formalized case plans and goals with clients; clients are high-need and require more intense services. Develops a written care-plan with the client, identifying problems and needs. Plan includes areas assessed, intended interventions, and expected results in measurable terms, with short and long term goals. Updates plan as client's needs change.</li><li>
Provides therapeutic interventions to clients. Provides intensive individual support to high need clients. Support may include providing therapeutic interventions, providing internal and community services referrals, and more intensive support may include accompanying clients to housing services, appointments, social services, etc. Focus on medical adherence, support, and appointments designed to maintaining necessary care.</li><li>
Performs other duties as assigned.</li></ul>
Requirements:<ul><li>
3 years of work experience providing human services to high risk, medically underserved, disabled populations, and/or relevant community health populations required.</li><li>
Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required. Master's degree in relevant field preferred.</li><li>
Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.</li><li>
For the AOD Services Department (159) Only: Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training required.</li><li>

Responsibilities:
<ul><li>Provide direct case management services (on-site and in the field). Case management services will include but not limited to mental health and substance use assessments, developing and updating service plans, assistance with obtaining public assistance, Social Security benefits/entitlements and other publicly available resources that may be beneficial to the tenant.</li>

We are seeking a community-based Case Manager to join our team in Colmar, PA serving clients in Montgomery County.
The Case Manager is responsible for developing, coordinating, and implementing services for adult consumers as identified in the goal-oriented service plan. This role includes providing case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life.

The Recovery Coach will:
<ul><li>Link consumers with a mental health or behavioral health diagnosis to resources and encourage, teach, and guide them to advocate for themselves.</li>
<li>Case Manager will complete comprehensive assessments, service plans, and environmental matrixes to determine level of care and frequency of contact with consumers on their caseload.</li>
<li>Complete other paperwork requirements from OMHSAS, Magellan Behavioral Health, Montgomery County, and Merakey and will be monitored by Case Manager and completed timely.</li>
<li>Have appointments with consumers in their homes, community, and hospitals and should be familiar with Montgomery County, including being able to access resources to meet client's needs.</li>
<li>Carry a caseload between 25-30 clients and Case Manager will meet with clients according to matrix score, either every 14 or 30 days while maintaining Merakey's productivity standard.</li>
<li>Attend supervision meeting with Manager monthly and team meetings weekly.</li>
<li>Work typically Monday-Friday: 9:00am - 5:00pm but shift may vary at times based on client's needs. This role includes optional on-call.</li>
</ul>
This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
Earn $22.29 per hour.

This role will directly report to the Case Manager Supervisor.Key Duties and Responsibilities● Provides direct and comprehensive culturally responsive case management, including responding to referrals, conducting intakes and a thorough needs assessment, and collaborating with the survivor to prioritize and meet service needs that may include safety planning, basic necessities, access to crime victim benefits, language access, housing, health, and medical, education and employment, among others.● Responds to crisis calls from clients during work hours.● Data entry and case notes per TFP protocols.● Identifies key resource gaps and reports these to the Program Manager and Community Coordinator.● Provides support to participants through the program structure and goals in a timely and sustainable manner.● Works to strengthen partnerships and community collaborations.● Serves as an advocate for survivors within systems that often create barriers for survivors of crime ensuring that survivors have access to crime victim benefits.● Participates in professional development as required and/or agreed upon by supervisor● Record and submit detailed documentation in a timely, thorough, and accurate manner.● Provide ongoing feedback to your supervisor and the CEO on how we can best serve the mission.● Performs other duties and assumes other responsibilities as assigned by the supervisor.Required Experience and Skills● At least 1 year of experience in Social work or human services.● Fierce care for empowering others and a strong passion for access to justice.● An understanding of the provision of trauma-informed services.● A team-oriented attitude.● Knowledge of Microsoft Office (including Excel, Word, and PowerPoint), Adobe Acrobat Pro, and Zoom.● Comfortable working in a dynamic, growing environment.Preferred Experience and Skills:● Bi-lingual Spanish required.You will thrive in the role if you are:● Able to independently initiate project and initiative ideas; managing projects and initiatives through to completion.● Highly creative in generating new ideas and creating novel solutions.● Detail-oriented, with the ability to keep track of many small moving parts across a project.● Highly flexible and able to gracefully adapt to new changes.● Comfortable taking the lead on projects and determining priorities in order to meet deadlines.Schedule:● The typical working hours are Monday - Friday, 32 hours a week, between the hours of 8:00 am - 6:00 pm.● Work is in person, with some remote options depending on the supervisor&rsquo;s request and the client's needs.● Periodic on-call may be necessary due to the crisis nature of this work.Note: This job description in no way states or implies that these are the only duties to be performed by the employee incumbent in this position.
