Program Overview:
Intercept&reg;, developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children. 
 
This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family&rsquo;s home and community&ndash;Intercept&reg; is not an office-based program. 
 
Intercept provides intensive, strength-based support tailored to each family&rsquo;s unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.
Essential Duties and Responsibilities:
The PRN Community-Based Counselor:<ul><li>
Carries a small case loas as assigned by supervisory on an as-needed basis</li><li>
Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families</li><li>
Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective</li><li>
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills</li><li>
Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required)</li><li>
Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan</li><li>
Provides on-call crisis support to the youth and family (schedules vary by location)</li><li>
Completes accurate and timely documentation in an electronic medical record system (EMR)</li><li>

 The Guia is responsible for a panel of patients and, in collaboration with other members of a multidisciplinary primary care team, helps patients meet their preventive, chronic, and acute care needs. WORKING TITLE: Counselor
This Master's Level Counselor will be serving the Oneonta City School System. This counselor will provide assessment, individual therapy and group therapy, conflict resolution and other mental health services to identified students in the Oneonta City School system with severe emotional and/or behavioral problems that are interfering with their functioning at school and/or home. This counselor will also provide mental health care coordination for the Oneonta City School System.

Division: Children's Services
Normal Working Hours/Days: Monday-Friday 8-5 with flexibility to meet client/family need.
Primary Job Functions
Responsible for interviewing, evaluating and assessing for mental health and other appropriate needs of children/families.
Responsible for properly assessing the unmet needs of the target population and devising an individualized service plan for each client.
Collaborates closely with the staff of the Oneonta City School System, as well as other community resources, in order to provide or access appropriate services to meet the needs of the target population (mental health services, school/special education service, etc.)
Responsible for individual group and family therapy with emphasis on behavior modification, Strategic Family Therapy, parent education and other therapeutic approaches as appropriate to treatment needs.
Provides accurate documentation of charts on a daily basis in an electronic medical record, including clinical assessments, case plans, treatment plans, referrals, data collections, case reviews and follow up tracking.
Responsible for providing and presenting clinical assessments and treatment goals at weekly staffings with supervisor.
Provides transportation to children and families when appropriate.
Performs other administrative duties as required.
Minimum Qualifications
Master's degree in psychology, social work or counseling with an emphasis on clinical service for children and adolescents. Licensure preferred.
Two years&rsquo; experience working with children/adolescents with a serious emotional disturbance and their families in a community setting with an emphasis on clinical services is preferred.
Valid Alabama driver's license and driving record acceptable to agency automobile insurance carrier for inclusion on agency policy. Suitable automobile to permit the required travel. Must show and maintain car insurance equal to or in excess of the State of Alabama standards for automobile insurance coverage.

This role involves providing expert clinical counseling, conducting assessments, and facilitating recovery programs for individuals battling substance use disorders.
