Bebashi Transition to Hope is seeking a dedicated and experienced Community Health Worker to join our team. This role will report to the Chief of Strategic Initiatives.
POSITION SUMMARY 
A Community Health Worker bridges the gap between health care systems and the community, ensuring access to care and programs that can help them lead healthier, safer, and more secure lives.
CHWs conduct community outreach activities, working directly with individuals and families with low-incomes, unmet needs, and/or who are eligible for Medicaid benefits in Philadelphia, PA.
Community outreach activities aim to support adoption of healthy behaviors by assessing community health needs and providing information on available resources, social supports, and navigation of resources.
CHWs work collaboratively with individuals and families to advocate for and support their access and understanding of available public health and community services and provide: outreach; screening/assessments; referral to services and follow-up; information on relevant health topics; advocacy and supporting increased health literacy; and enhanced social supports including support networks.
CHWs represent the diversity of cultures, backgrounds, generations, social identities, lived experiences, and neighborhoods in Philadelphia, PA and further develop relationships in a culturally sensitive manner. 
 
KEY RESPONSIBILITIES<div><ul><li> 
Plan and coordinate outreach, supervise individual and family assessments, assess training needs, provide health education, home visiting, and service coordination performed by CHWs.

<ul><li>Develop an overall work plan, including outreach strategies for the in the targeted geographic area.

<ul><li>As a member of an integrated care team, works closely with other providers across the organization and partners to ensure progress and implementation of the workplan, including leading structured meetings and identifying/resolving issues. 
</li></ul></div><div> 
<ul><li>Prepare and present reports &amp; evaluations defining project progress, problems, and solutions. 
</li></ul></div><div> 
<ul><li>Establish relationships, promote collaboration and coordination with other community health and human service providers, and diverse community partners.

<ul><li>Conduct community and public education sessions to promote initiative or program including, primary prevention education, chronic conditions/disease management, screening, and testing.
</li></ul></div><ul><li>Participate in and/or lead a community action board and engage community members through community and civic engagement activities.
</li><li>Provide or ensure clients have access to education, resources, and enhanced social supports (e.g., affordable and stable housing; food insecurity, or utility services threat(s)).

<ul><li>Ensure that clients are engaged with providers to support access to systems of healthcare to support primary and specialty care, social services, public benefits, and other community resources.

<ul><li>Collaborate with clinical care team (e.g., Nurse Practitioner, Physician Assistant, Peer Professional, or Medical Case Manager, Behavioral Health Practitioner) to monitor client services, referrals, and appropriate follow-up.

<ul><li>Design and implement client surveys to evaluate client satisfaction with CHW services.

<ul><li>Coordinate with agency staff to develop documents (educational materials, forms, signage, etc.) and procedures that support a health literate environment and enhance client understanding of health information.

Street Outreach WorkerABOUT HOMEWARD PIKES PEAK (HPP)The mission of HPP is to work with individuals and families in our community to achieve their full potential through supportive housing and recovery services, to rebuild dignity &amp; self-sufficiency, and permanently exit homelessness. We believe that these values infuse the work of HPP and are a guidepost to which our staff and partners should aspire.SUMMARYThe Street Outreach Worker seeks to support individuals experiencing homelessness and/or living with a mental illness, substance abuse or all three. Outreach Workers perform within a harm reduction and person-centered model. All duties are performed with a multidisciplinary and multiagency team approach for those sheltered in cars, parks, abandoned buildings, encampments and on the streets. Street outreach utilizes a person-centered approach, focused on the individual&rsquo;s strengths and resources, and never makes assumptions about what a person might need or want. The Outreach Team collaborates with city, fire and police, and other community-based agencies; this position will focus primarily on the Downtown Pilot &ldquo;Clean &amp; Safe&rdquo; program. Our Street Outreach team does not require individuals to enter emergency shelter or transitional housing as an interim step or prerequisite to accessing stable housing. However, street outreach does make immediate connections to emergency shelter or temporary housing to provide safe options while individuals and families are on a pathway toward stability.This is a full-time position that reports to the Lead Street Outreach Worker. This position is authorized to work 40 hours per week with Saturday shifts required. Adjustments in daily assignments and schedules will be made with the Street Outreach Program Director.DUTIES AND RESPONSIBILITIES<ul><li>Support individuals experiencing homelessness and living in the community through activities such as:</li></ul>-- one-on-one contact with individuals on the streets and in emergency shelters;-- advocating on behalf of clients and/or supporting them to advocate for themselves;-- collaboration and coordination of services with appropriate community agencies.<ul><li>Provide resources and referrals to individuals experiencing homelessness and living in the community by liaising with community agencies and consulting with the Clinical Services and Homeward Pikes Peak housing team, and potentially with other community organizations as a part of the Continuum of Care.</li><li>Conduct the housing survey/VI-SPDAT as needed and record all encounters of people engaged in referrals, resource navigation and general information sharing.</li><li>Work with the CSPD HOT team (Colorado Springs Police Department Homeless Outreach Team) to support team outreach in community-based resource centers such as the Springs Rescue Mission, Catholic Charities, Pikes Peak Library District, and in public spaces.</li><li>Support the Lead Street Outreach worker with managing and maintaining an application interface that assists with outreach efforts, and effectively triage reported activities and requests. Work with community partners to develop and increase utilization efficiency.</li><li>Work with the Pikes Peak Coordinated Entry system to connect individuals selected for housing and the referring agency. Assist these clients to complete paperwork, obtain identification and prepare for housing applications.</li><li>Assist individuals seeking housing in acquiring resources, identification, completing applications, navigate housing options, connect with case managers and transition successfully into permanent housing. <div>We are looking for a dedicated candidate join our team as a Community Health Worker (CHW) to service our patients in the Mt. Pleasant area. The Community Health Worker (CHW) serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality-of-service delivery. This role includes visiting patients in their homes and Assisted Living Facilities (ALFs) to facilitate visits with providers and specialty services. Additionally, the CHW is responsible for delivering

Community Health workers perform visits in homes and facilities (ALF and ILF's) in their designated service area. You must have reliable transportation as travel is required daily. This is a full time, salary-based working 12-hour shifts (7:00AM-7:00PM)
