Qualifications:

 The Guia is responsible for a panel of patients and, in collaboration with other members of a multidisciplinary primary care team, helps patients meet their preventive, chronic, and acute care needs. The Guia engages patients and encourages them to take an active role in their health by providing the tools necessary to make healthy lifestyle choices and adopt lifelong healthy behaviors. This individual&rsquo;s primary responsibilities center around establishing trusting, supportive, collaborative relationships with patients and their families and assisting patients in meeting their social needs. The Guia builds relationships with patients in a clinical setting and in the community by working alongside medical providers, nurses, medical assistants, and a multidisciplinary team in a collaborative and empathetic team approach to improve patient outcomes. Tranquil Minds Behavioral Health

Responsibilities:1. The CHW will provide psychosocial rehabilitation training that assists patients with life skills, coping skills, independent living skills, housing and home maintenance, personal management skills, vocational skills, employment skills, and education on medication management. Classroom Responsibilities:

Develop and implement instruction for each class and laboratory period, consistent with the official course outline and District guidelines.</li><li>
For online and hybrid courses, maintain online course components, including asynchronous or synchronous content delivery, as required by the teaching assignment.</li><li>
For online and hybrid courses, provide regular student-student and student-instructor contact.</li><li>
During the first week of class, develop and distribute a syllabus for each course to communicate to students the course description, objectives, grading, testing, attendance, and other requirements, in accordance with District guidelines on syllabus content.</li><li>

ROLE OVERVIEW and PURPOSE:

The Community Health Worker (CHW) is responsible for working effectively with Enhanced Care Management (ECM) members to improve their whole health through outreach and engagement activities, which are primarily field based. This position works collaboratively as part of the Enhanced Care Management team, members and families, and other professionals and works with the designated primary care provider (PCP) care team.

**This is a hybrid position; on site expectations are dependent on performance. **

RESPONSIBILITIES:
<ul>
<li>Assists members in navigating the healthcare system by finding and following up by phone and in-person with eligible ECM members</li>
<li>Helps members participate in their medical and/or behavioral healthcare by overcoming barriers to care</li>
<li>Documents and shares member barriers with ECM team and PCP to improve care and outcomes</li>
<li>Utilizes evidence-based approaches when engaging with patients to increase their sense of control over their health, such as motivational interviewing to promote collaboration between patients and their medical/behavioral care team</li>
<li>Communicates and delivers easy to read information about health and wellness to the community and provides information on Home Health Plan (HHP) member benefits and services</li>
<li>Advocates on behalf of ECM members in the home, community, and provider organizations</li>
<li>Builds and maintains a positive working relationship with providers, including interactions within provider offices, phone, email, fax, and other approved communication methods</li>
<li>Assists the ECM team and providers to understand the culture, norms, beliefs, and preferences about patients and their communities by representing the community's voice</li>
<li>Helps create messages and materials that fit community culture and delivers messages to fit the culture of members</li>
<li>Works collaboratively within the ECM team to discuss responsibilities, task sharing, conflict resolution, and member case reviews</li>
<li>Collaborates on member care issues with other ECM team members</li>
<li>Participates in weekly case reviews and consults with the member's PCP before taking any clinical action</li>
<li>Ensures documentation is accurate, helpful, and compliant with regulatory requirements and accreditation standards</li>
<li>Participates in required staff meetings or other activities</li>
</ul>
EDUCATION/EXPERIENCE:
<ul>
<li>High school/GED required</li>
<li>Two years of healthcare, behavioral health, or social services industry experience required</li>
<li>Valid CA Driver&rsquo;s License and proof of auto insurance required</li>
<li>Experience working in outreach and customer service within the medical office and behavioral health environments with individuals who struggle with substance use disorder and mental health issues preferred</li>
<li>Bilingual (English/Spanish) preferred</li>
</ul>

Pay range: $24.95 - $34.93 per hour, depending on experience

GENERAL DESCRIPTION OF DUTIES
The purpose of this position is to support the Agency&rsquo;s needs by providing support based on CRT&rsquo;s Steps to Success model, the Case Manager &ndash; Certified Community Health Worker (CHW) is responsible for providing a wide-range of services; including, but not limited to, outreach, engagement, education, coaching, informal counseling, social support, advocacy, care coordination, and basic screening and assessments of any risks associated with social determinants of Health. Daily monitoring of customer contacts and activities within the Community is a primary responsibility. The Case Manager &ndash; Certified CHW conducts assessments of customer needs and coordinates referrals to Community Renewal Team programs and initiatives, Funder services, entitlement programs, as well as, to other Community resources that conduct Case Management services.
SPECIFIC DUTIES AND RESPONSIBILITIES
ESSENTIAL JOB FUNCTIONS

<ul><li>Identify/Provide outreach and engagement functions to customers in the Community.</li><li>
Identify customers in need of Case Management and/or other Outreach Engagement services; linking them with available services.</li><li>
Interview customers on and/or off site as required.</li><li>
Complete Universal Screening to determine Program eligibility.</li><li>
Assist customers in their Homes, Community, or Clinical setting.</li><li>
Establish positive and supportive relationships with the customers.</li><li>
Complete an initial interview and assessment of individual needs.</li><li>
Assess and evaluate customer needs based on the Outcome Scale Matrix (OSM).</li><li>
Develop Individualized Service Plans (ISP) for each customer, and incorporate all special needs.</li><li>
Facilitate referrals to Clinics and Community Support services.</li><li>
Facilitate communication/coordinate services between providers and customers.</li><li>
Coordinate/Monitor services to include comprehensive tracking of customers compliance in relation to ISP goals and objectives.</li><li>
Provide support, advocacy, and mediation services to assure customer&rsquo;s overall needs and referrals are being conveyed.</li><li>
Follow-up with both customer and provider regarding ISPs.</li><li>
Document all encounters/interactions made with and/or on behalf of the customer; including all activities, ISPs, and outcomes achieved by the customers in a detailed and timely manner.</li><li>
Conduct monthly home visits with customers as needed; documenting all visits in the customer&rsquo;s file.</li><li>
Conduct bi-weekly conversations with the customer either by phone or in-person; documenting all conversations in the customer&rsquo;s file.</li><li>
Maintain client records in accordance with documentation requirements and standards.</li><li>
Assist clients in utilizing Community Services, including; scheduling appointments with Social Services agencies, and assisting with the completion of applications for Programs based on eligibility.</li><li>
Assist in the preparation of demographic, and other Funder reports relative to Client Services.</li><li>
Participate in Case Management team meetings.</li><li>
Provide reports to referring agencies including; but not limited to, progress reports, discharge summaries, aftercare plans, and special reports.</li><li>
Ensure all Intake information is obtained based on program guidelines, and accurately entered into STEPS and/or HMIS systems.</li><li>
Collects demographic data, conducts pre-screening, and assesses potential eligibility for CRT, Department of Housing, or other Community-based resources.</li><li>
Facilitates information and referral services, and documents outcomes resulting from such referrals.</li><li>
Facilitates customer access to Community resources, including; but not limited to food, housing, clothing, school programs, vocational opportunities or services, life skills training, and relevant Mental Health services.</li><li>
Educate customer on the proper use of the Emergency Room, and provide information for alternatives.</li><li>
Maintains a caseload of customers in need of intensive Case Management or multiple service needs.</li><li>
Complete and submit Critical Incident reports to Funder when applicable.</li><li>
Continuously expand knowledge and understanding of Community resources and services.</li><li>

MINIMUM TRAINING EXPERIENCE AND HEALTH CERTIFICATION
Education: Bachelor&rsquo;s degree from an accredited college or university required; Education may be substituted with experience on a year-by-year basis.
Minimum Years of Experience: Two (2) year of experience providing Direct Client services to population experiencing Substance Abuse, Homelessness, and/or Mental Illness required.
Certifications: Must have a CT Community Health Worker certificate or be able to obtain the certificate within six months of taking the position required.
Knowledge of: Working with the chronically homeless population preferred; Housing First and Harm Reduction Intervention strategies, Motivational Interviewing techniques required.
Demonstrate skills in: Ability to work with individuals from diverse racial/ethnic and economic backgrounds; Solid understanding of the dynamics of Homelessness, and being at risk of Homelessness; Ability to offer strength-based Case Management; Ability to communicate effectively with Customers, Funders, and Vendors; Ability to function independently in a Community setting; Ability to interact with the Homeless, and develop trusting relationships; Ability to set therapeutic boundaries with clients; Ability to establish good working relationships with staff and Community agencies; Ability to work with a multi-disciplinary team; Ability to decide what presenting conditions are of priority; Ability to plan and organize assigned duties; Computer proficiency in Microsoft Outlook, Excel, Word, PowerPoint; Financial Literacy Skills are all required.
Driving Requirements: Maintain a valid State of Connecticut Driver&rsquo;s License in good standing, and have the ability to take your personal vehicle into the Community required.
