<b>Position Description</b>:<br>
The Community Liaison works under the direction of the Community Coordinator to implement, coordinate, and sustain community engagement activities in assigned communities. The Community Liaison advances the agency&rsquo;s mission by building and sustaining trusting relationships with community stakeholders, facilitating communication and coordination across partners, and connecting community members to resources within the child welfare continuum of care.<br>

<b>Primary Responsibilities</b>:<ul><li>
Build strong, trust-based relationships with individuals, families, and local leaders within assigned communities.</li><li>
Regularly attend community events, meetings, and gatherings to maintain visibility and foster collaboration.</li><li>
Identify and document existing community supports and services related to child and family well-being.</li><li>
Support advocacy efforts for additional services or resources where gaps exist, in collaboration with community members and partner organizations.</li><li>
Share community observations, feedback, and identified gaps with the Community Lead to inform planning and decision-making.</li><li>
Serve as a point of contact between the community and service providers, ensuring timely communication and alignment with Community Lead priorities.</li><li>
Facilitate connections between residents, organizations, and systems to strengthen natural support systems.</li><li>
Assist in strengthening informal and formal community networks by supporting meetings, collaborations, and partnership activities.</li></ul><b>
Required Qualifications</b>:<ul><li>
Bachelor&rsquo;s degree in Social Work, Human Services, Community Development, Public Administration, or a related field; or equivalent experience.</li><li>
Two (2) years&rsquo; experience in community engagement, social services, grassroots organizing, or a related field.</li></ul><b>

Overview</b></p>
<p>
The Account Liaison is responsible for building and maintaining relationships with referral sources to promote awareness of Arc Hospice &amp; Palliative Care services. This field-based role generates new business opportunities, supports referral conversion, and serves as a key representative of Arc within the community. The Liaison collaborates closely with operational and clinical leaders to ensure alignment between referral needs and service capabilities. This is a performance-driven role focused on consistent outreach, education, and engagement across the assigned territory.</p>
<p><b>
Key Responsibilities</b></p>
<ul>
<li>Represent Arc Hospice &amp; Palliative Care in the community with professionalism and compassion.</li>
<li>Develop and maintain strong relationships with physicians, hospitals, nursing homes, assisted living facilities, senior communities, and other referral partners.</li>
<li>Meet with patients and families to review hospice philosophy, explain services, answer questions, and obtain signed consents for care.</li>
<li>Educate healthcare providers and community members on hospice and palliative care services.</li>
<li>Conduct in-services, presentations, and community outreach events to raise awareness.</li>
<li>Support referral generation and facilitate timely communication with the intake and clinical teams.</li>
<li>Identify opportunities for partnership or collaboration to enhance access to care.</li>
<li>Track outreach activities and maintain accurate records in accordance with company documentation standards.</li>
<li>Maintain awareness of competitor activity and market trends in the assigned territory.</li>
<li>Participate in team meetings and contribute to ongoing strategy and planning discussions.</li>
<li>Support the company's mission to deliver high-quality, person-centered care.</li>
<li>Maintain confidentiality and adhere to all HIPAA regulations.</li>
<li>Perform other duties as assigned.</li>
</ul>
<p><b>Qualifications</b></p>
<ul>
<li>Bachelor's degree preferred.</li>
<li>Minimum of two (2) years of experience in healthcare sales, business development, or marketing; hospice or home health experience strongly preferred.</li>
<li>Strong interpersonal, communication, and relationship-building skills.</li>
<li>Ability to educate diverse audiences about hospice services with clarity and empathy.</li>
<li>Highly organized, self-directed, and goal-oriented.</li>
<li>Proficient with Microsoft Office and electronic medical record systems.</li>
<li>Valid driver's license and reliable transportation required.</li>
<li>Demonstrated empathy, sensitivity, and ability to support patients and families from diverse backgrounds.</li>
