<b>General Description:</b> Responsible for participating in an interdisciplinary assessment regarding individuals seeking or residing in nursing home level of care to determine the least restrictive living environment to meet their overall needs.
</p><p><b>Responsibilities and Duties:</b> Assesses OBRA/PASARR referrals assigned by OBRA Coordinator and/or designee. Provides education as needed on the OBRA/PASARR process to community partners, consumers, and families. Completes timely assessments for consumers seeking nursing facility placement or already in nursing facility placements. Ensures consumer placed in appropriate and least restrictive living environment to meet their mental and physical health needs. Provides back-up coverage for OBRA therapists in completing Level II OBRA screens for team leave requests. Ensure Specialized Mental Health services are being provided as authorized within our six-county catchment area. Has an acute knowledge of the Level of Care Determination (LOCD) requirements for nursing facility placements. Ensures compliance with MDHHS-OBRA regulations concerning timelines for all assessments. Completes reviews and delivery of Level II evaluations to the individual, legal guardian, hospital, and nursing facility as required by MDHHS-OBRA. Attends OBRA related team meetings and/or trainings related to professional development and or improving practices. Participates in peer reviews or interdisciplinary meetings prior to submission of evaluations.</p>

The respite homes are community based residential programs that provide a therapeutic environment for adults experiencing a mental health crisis. The program provides Peer Support Services utilizing individuals with a history of lived experience with mental health challenges and a willingness to share their story as well as Crisis Support Staff who have extensive experience in the field. The staff provides services including but not limited to health and wellness coaching, reoccurrence prevention planning, wellness activities, family support, conflict resolution, and referrals and linkage services. The goal is to stabilize crisis to support a guest’s safe return to their community and to provide preventive measures for reducing un-necessary emergency department visits and hospitalizations.

<b>Peer Specialist Job Responsibilities:</b><br>

<li>Provide required services to guests enrolled in the Supportive Crisis Residence as outlined in 14 NYCRR XIII Part 589, including: assistance in personal care and activities of daily living, peer support, engagement, safety planning, integration of direct care and support services, case management, medication management, and medication monitoring.</li>
<li>Provide peer support in fidelity with the Peer and Recovery-Oriented models of support.</li>
<li>Provide any needed crisis intervention, teach individuals to look at crisis differently and utilize wellness tools to prevent future crises or to decrease the intensity of personal crisis.</li>
<li>Work closely with Health Homes, hospitals and treatment providers through a person centered/self-directed approach to develop an integrated plan of care including mental health, physical health, substance use services, and community supports necessary for individuals to succeed in the community.</li>
<li>Provide advocacy, peer support, referral and linkage services for program guests as needed.</li>
<li>Provide coverage for the program as reflected in staffing plan, including overnights as required.</li>
<li>Completes all required documentation, such as Individual Service Plan, Social Determinants of Health Assessment, Strengths and Needs Assessment, Safety Planning, Daily Progress Notes, and Discharge Summary in timelines in accordance with program standards.</li>
</ul>
<b>Peer Specialist Qualifications:</b><br>
<ul>
<li>Work experience in a behavioral health or related setting, preferably using the recovery model with one to two years of personal recovery experience is preferred.</li>
<li>Have personal experience as a recipient of mental health services with a willingness to share personal, practical experience, knowledge, and first-hand insight to benefit program guests.</li>
<li>Certified by the Office of Mental Health (OMH) as a Certified Peer Specialist or willing to complete the certification process within six months of employment.</li>
<li>Minimum of high school diploma or equivalent (GED).</li>
<li>A respect for, compassion and high level of comfort around people experiencing psychiatric crisis.</li>
<li>Ability to utilize critical thinking, problem-solving and de-escalation skills to assist with creating and maintaining an environment that promotes recovery.</li>

Who We Are Looking For:</b></p><p>
The 988/Helpline Outbound Specialist supports NAMI Chicago&rsquo;s mission by providing support to individuals and families who have contacted the NAMI Chicago Helpline and 988 call center through follow-up calls and supportive, scheduled outreach to identified contacts. The NAMI Chicago 988 call center provides anyone immediate crisis response, while the Helpline provides information, resource referrals and emotional support to people living with a mental health condition, their family members and caregivers, mental health providers and the public. Through empathic, active listening, this specialist gains an understanding of concerns that led to caller engagement with 988 or the Helpline and explores subsequent needs. The 988/Helpline Outbound Specialist often supports individuals and families with ongoing support, linkage connection and safety planning, while projecting a non-judgmental attitude that allows those requiring assistance to feel heard and supported as they navigate the healthcare system.</p><p>
The ideal candidate has a Bachelor&rsquo;s degree in a social services field, strong customer service skills, strong verbal and written communication skills and prior experience with the mental health system of care including crisis response.</p><p><b>
Essential Functions &amp;</b> <b>Responsibilities:</b></p><ul><li>
Provides outbound telephonic, text and chat support, education, information, assessment of need and referrals to individuals and families specific to mental and emotional health needs.</li><li>
Engages in de-escalation, safety planning, risk assessment, and emotional support for those needing crisis intervention.</li><li>
Maintains contact notes, agency resource records, and other documentation using NAMI Chicago&rsquo;s database system.</li><li>
Assists contacts in problem-solving, goal setting and communication skills building.</li><li>
Coaches people through navigating service access, specifically the mental health and social service resources and support available to people throughout Chicago and Cook County.</li><li>
Provides limited case management support as callers link to resources in the community, or engages in regular connection with callers seeking accountability, who are building social networks, or are needing support between community resource appointments.</li><li>
Ability to multitask during a call e.g., remaining engaged in conversation, documenting information, assessing needs, and identifying responses to the contacts needs.</li><li>
Exemplify NAMI Chicago&rsquo;s team approach by participating in case review, supporting coworkers, and attending scheduled trainings.</li><li>
Acts as a back-up 988 Crisis Counselor or Helpline Specialist as needed.</li><li>
Perform other duties as assigned by the Helpline or 988 Leadership Team.</li></ul><p><b>

Experience with and able to demonstrate ability to serve anyone in the greater Cook County area who contacts 988 or the Helpline. The NAMI Chicago 988 call center and Helpline are no-barriers access points to support, education and referrals for individuals and families living with mental health challenges, mental illnesses or other emotional stressors. This is especially true for our underrepresented and vulnerable populations, including social and racial/ethnic minorities, youth and young adults, and those with severe mental illness.</li><li>
Ability to make real-time rapid assessments of caller, dexterity in judgement and navigation of a call, recognition of appropriate utilization of leadership support and self-regulation of response to callers.</li><li>
Excellent verbal and written communication skills, with the ability to effectively communicate with those in crisis or the aftermath of crisis, their families and community stakeholders.</li><li>
Exhibits sensitivity and empathy regarding human service issues and projects a non-judgmental attitude towards those requiring assistance.</li><li>
Ability to maintain confidentiality of sensitive information and show professionalism, reliability, and responsibility.</li><li>
Familiar with community resources and how to work with law enforcement and other community organizations (i.e., Hospitals, outpatient clinics, etc.).</li><li>
