Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
 
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
 
Street to Home, Brooklyn
 
Breaking Ground’s Street to Home program makes regular contact with more than 2,000 street homeless individuals and connects more than 500 individuals to housing, medical and mental health services, substance abuse counseling, and other essential supports each year. The caring, individually tailored attention clients receive at each stage of their journey from street to home ensures that more than 95% of people who Breaking Ground places remain stably housed.
 
The Housing Advocate works with street homeless conducting street outreach with the objective of engaging individuals to determine their eligibility for Street to Home services and to help secure appropriate housing solutions for all people who are street homeless. If chronic, the Housing Advocate will build a relationship with clients utilizing motivational interviewing to encourage the client to work towards permanent housing. If not chronic, Housing Advocate will utilize available resources to secure other housing placements. The Housing Advocate will always work with a partner and will spend most of the shift in an automobile conducting street outreach. The incumbent will respond to 311 calls and community concerns; participate in Code Blue and Code Red alerts; identify new hotspots/encampments and work to eradicate existing hot spots/encampments; conduct a bi-annual vulnerability index on all clients in their catchment area; and enter all required data into outreach database(s).
 This position has a 35-hour work week.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:

<li>Create and maintain active registry of chronic homeless clients; non-chronic included in registry</li>
<li>Conduct vulnerability assessment</li>

MINIMUM QUALIFICATIONS:

<div>The Enhanced Care Management (ECM) Advocate is responsible for the and coordination, improving health outcomes, enhancing satisfaction, and reducing unnecessary healthcare utilization. ECM is a comprehensive, patient-centered approach to healthcare that aims to improve outcomes for high-need populations. This involves the coordination of medical, behavioral, and social services to address the full range of patient needs, focusing on those with complex and chronic conditions. ECM programs are designed to optimize care delivery, reduce unnecessary hospitalizations, and improve the overall quality of life for. The intention of our services is to help people who have not been able to get the needed help in terms of Housing, Mental Health, Substance Abuse treatment, and linkages to community resources to help minimize recidivism. The role of the ECM Advocate is to help create a supportive network of services and resources for the client to prevent reincarceration and support long term permanent housing goals.</div>

SAFE Alliance seeks a Community Shelter Advocate for the Residential &amp; Support Services program in the Family &amp; Community Shelter department. The Advocate provides crisis intervention, safety planning, and emotional support for clients, individually and/or through group formats. They assist clients in accessing services including, but not limited, as shelter, legal advocacy, counseling, education, and housing support.

The SAFE Alliance exists to stop abuse for everyone by serving the survivors of child abuse, sexual assault and exploitation, and domestic violence. We are dedicated to ending violence through prevention, advocacy, and comprehensive services for individuals, families, and communities that have been affected by abuse.
In the past two years (23-24), our community outreach and education has provided over 12,000 adults and youth violence prevention training. The number of forensic exams provided at Eloise House numbered over 300. Our various housing and shelter programs provided over 700 survivors per day with safe housing and shelter. Over 120 survivors fleeing sex trafficking received services through our CARES program and over 27,000 callers/chats/texts, walk-in advocacy, crisis intervention and face-to-face emotional support has been provided through our 24/7 confidential SAFEline.

Position Details:

We are looking for one person to work on a full-time, non-exempt basis for an annual salary of $47,684 to $52,900 dependent upon experience and an additional annual language differential of $3,600 for those who are English/Spanish bilingual. The work location will be based at our new Community Shelter Campus in the Pecan Springs-Springdale area. This position will include some travel in the community or between campuses with no ability for remote/hybrid work.
