The Outreach Worker works with the CPEP Mobile Crisis Teams (MCT) and directly reports to the Manager, Program Coordinator, and Medical Director of CPEP. As an integral member of MCT, the Outreach Community Coordinator is expected to perform ancillary functions as designated by the team&rsquo;s clinician for example, Social Worker, Nurse Practitioner, or CPEP Manager/Coordinator. The primary purpose of this position is to transport the CPEP staff to patient homes for clinical evaluation, arrange for the initial scheduling for referrals received surrounding psychiatric assessment of patients in the community, billing and insurance authorization, using EPIC, maintain statistical data on a weekly basis, which is inclusive of, but not limited to the gathering of patient demographics for performance improvement projects, completing MAVEN, performance of light clerical support, for example, faxing and copying patient data, arranging for the maintenance and upkeep of the programs vehicles, and assisting the programs clinician in emergent situations which require 911 intervention.</div>
Responsibilities:
<ul><li>Assist the CPEP clinicians in calling for police/EMS back up as needed.
</li><li>Participate with clinicians in violence de-escalation/containment in a team approach, utilizing good communication and social skills within the patient's home.
</li><li>Provides support clerical services (maintains case records) and follows up with appropriate referrals, under the direction of the designated program clinicians.</li><li>
Responsible for adequate maintenance of programs vehicles, including ensuring those vehicles are in conformity with registration and inspections.</li><li>
Completes billing in EPIC in a timely manner and works with insurance companies to obtain the necessary authorizations.</li><li>
Completes MAVEN database information for DOHMH.</li><li>
Works weekend, evenings, and holidays.</li><li>
Attends trainings including PMCS.</li><li>
Completes all hospital requirements including OHS, yearly reorientation, Corporate Compliance, Flu shot or declination form, and other requirements.</li><li>

LSF is seeking a talented SNAP Case Manager who wants to make an impact in the lives of others. This Case Manager will be working directly with youth ages 12-17 who are dually involved or at risk of being dually involved in the DCF and DJJ Systems. The Case Manager will run groups for youth ages 12-17.
</p><p><b>Purpose and Impact:</b></p>
<p>
Responsible for the day-to-day case management and system navigation of SNAP&reg;.</p>
<p>
Staff in the dually served program will be working with youth ages 12-17 who are dually involved or at risk of being dually involved in the DCF and DJJ systems. This case manager will be primarily working directly with youth and running groups for youth ages 12-17. This role additionally requires that someone be tech savvy as you will guide youth through the SNAP program using a tablet.
</p><p><b>Responsibilities:</b></p>
<ul>
<li>Completes staff training as required by SNAP&reg; and agency;</li>
<li>Completes intake assessments for SNAP&reg; including all required pre and post assessments.</li>
<li>Responsible for the day-to-day case management and system navigation of SNAP&reg;</li>
<li>Follows up on incoming referrals and schedules clients for intakes appointments.</li>
<li>Monitors youth and family's progress through weekly compliance calls;</li>
<li>Facilitates SNAP&reg; curriculum within the fidelity of the model;</li>
<li>Liaises with relevant community outreach responsibilities including, but not limited to community relations with: agencies including schools, community-based agencies, local DCF and DJJ community partners, state attorneys&rsquo; offices, and Police Departments to enhance outreach;</li>
<li>Completes data reporting requirements for intake assessments and ongoing weekly data reporting requirements to all relevant systems in a timely manner (examples include: NETMIS, JJIS, Dropbox, and ASEBA, etc.)</li>
<li>Ensures weekly materials are prepared and ready for SNAP&reg; groups including handouts, manuals, and all other required items;</li>
<li>Supports the SNAP&reg; Coordinator with organization of food arrangements as needed;</li>
<li>Participates in pre-briefing and de-briefing meetings weekly with other SNAP&reg; Team members;</li>
<li>Participates in weekly consultation calls with the Florida Network;</li>
<li>Willingness to work nontraditional business hours including evenings when Groups are scheduled.</li>
<li>Prepares technology that is required for weekly group delivery;</li>
<li>Utilize a strength-based perspective to help youth and families strengthen and manage the quality of their lives by engaging the family, advocating on their behalf, initiating change agent activities, helping them access supports in the community, teaching problem solving skills, modeling productive behaviors, and assisting in the successful completion of youth and family developmental milestones;</li>
<li>Works under the supervision of the SNAP&reg; Coordinator and participates in staff meetings as requested.</li>
</ul>
