Our mission-driven and member-focused company is seeking an experienced and self-motivated Community Guide who has worked with individuals living with serious mental illness and substance use disorder, and understands the unique needs of this community. In this role, the Community Guide will be working with the multidisciplinary care team to plan and coordinate services for members.

The Outreach Specialist is the first voice a member hears from Reema Health. This telephonic role is responsible for the critical first phase of the member journey: reaching out to eligible members, introducing them to Reema's care model, building initial trust, and completing a warm handoff to a Community Guide for ongoing support. Based out of our Minneapolis, MN office, this role serves members across the United States entirely over the phone.

Member Outreach and Engagement
<ul>
<li>Conduct high-volume telephonic outreach to eligible members across the United States to introduce them to Reema Health's care model and services.</li>
<li>Build rapport quickly and authentically over the phone, meeting members with empathy and a non-judgmental approach.</li>
<li>Educate members on the value of Reema Health services and answer questions to drive enrollment and engagement.</li>
<li>Collect verbal and written consent from eligible members and complete enrollment documentation accurately.</li>
<li>Maintain consistent daily outreach activity, meeting or exceeding call volume, connection, and enrollment targets.</li>
<li>Adapt outreach strategies and messaging based on member demographics, communication preferences, and engagement barriers.</li>
</ul>
Care Team Coordination and Handoff
<ul>
<li>Execute a warm, thorough handoff to an assigned Community Guide once a member is engaged and enrolled.</li>
<li>Communicate pertinent member information, expressed needs, and any urgent concerns to the care team as part of the handoff.</li>
<li>Collaborate with Community Guides and clinical staff to ensure newly enrolled members experience a seamless transition into ongoing care.</li>
</ul>
Documentation and Technology
<ul>
<li>Utilize Reema's technology platform daily to document all member interactions, track outreach activities, and manage your outreach pipeline.</li>
<li>Review pre-populated member demographic information and ensure records are accurate and up to date.</li>
<li>Understand and comply with confidentiality requirements for protected health information and HIPAA.</li>

Education, Experience and Skills:

<li>2 or more years of experience in business development, sales, customer engagement, or a related outreach-focused role.</li>

Client Services Specialists are the initial point of contact for all new clients of Gulfcoast Legal Services. JOB SUMMARY:
The Street Outreach Specialists, individually and collectively, reach people who might not otherwise seek assistance or come to the attention of the homelessness service system. This position works to meet unsheltered people&rsquo;s basic needs while supporting them along the path toward housing stability. Street Outreach Specialists embrace a person-centered approach that inherently requires flexibility, patience, strong engagement techniques, and empathy. Due to the independent nature of frequently working solo and directly in the community, Street Outreach Specialists must be skilled in handling encounters and interactions that require problem-solving and independent judgement in situations such as individuals needing immediate housing, medical care or psychiatric hospitalization.
JOB RESPONSIBILITIES:<ul><li>
Provide targeted street outreach, assuring appropriate geographic coverage, to communities within Housing Forward&rsquo;s service area to identify unsheltered homeless persons living on the streets or other places not meant for human habitation.</li><li>
Establish rapport and reduce harm by providing critical resources such as food, water, clothing, blankets, and other necessities.</li><li>
Determine the person&rsquo;s immediate safety needs.</li><li>
Using a trauma-informed approach, provide crisis intervention, and other skills and strategies as needed for engagement.</li><li>
Perform assessments and prioritize for assistance as sheltered person assessed through the coordinated entry process. Refer unsheltered families to the Family Support Specialist and unsheltered homeless youth to the System Navigator.</li><li>
Perform intake and complete VISPDATs as necessary.</li><li>
Work as a team with the other Street Outreach Specialists.</li></ul>
Service and Housing Coordination responsibilities:<ul><li>
Make immediate connections to emergency shelters or temporary housing to provide safe options while individuals and families are on a pathway toward stability.</li><li>
Make connections to stable housing with tailored services and supports of their choice, such as health and behavioral health care, transportation, access to benefits, and more.</li><li>
Perform warm handoffs to Coordinated Entry or to shelter, housing, and service providers (e.g., outreach staff may offer to physically accompany the individual to appointments to provide support).</li><li>
Collect and maintain required program documentation and data in input into the HMIS system and agency SharePoint site.</li></ul>
Partnership Relationships and Stewardship:<ul><li>
Establish working relationships with community stakeholders - law enforcement, libraries, first responders, hospitals, health and behavioral providers, townships, homeless education liaisons, faith-based organizations, and other community-based providers.</li><li>
Conduct joint outreach with Loyola&rsquo;s Street Medicine and Live4Lalli teams to identify individuals that may be appropriate for harm reduction interventions.</li><li>
Provide access to primary medical care, both onsite and offsite, though warm handoff to Loyola Health clinicians.</li><li>
Communicate and cooperate with staff from other programs as needed to ensure seamless delivery of service to clients.</li><li>
Create linkage to medically assisted treatment and harm reduction counseling enrollment.</li><li>
Provide evidence-based training to community organizations regarding the challenges and issues confronted people living on the streets and programs and strategies including, services offered by the street outreach team that are designed to help individuals on the street.</li><li>
Participate in regular program, administrative and planning meetings with the partners in the project.</li></ul>
Other:<ul><li>
Log activities and time as required by the agency and its funders.</li><li>
Attend team and agency meetings and trainings.</li><li>
Provide technical assistance to other Outreach Specialists with data entry and client files.</li><li>

Requirements:

QUALIFICATIONS:
Education: BA, B.S. Advanced degree or credential or certification in Social Work, Family Services, Human Services, Counseling or in related field; with a minimum of one year experience. Individuals with lived experience and two years&rsquo; working in related fields will also be considered.

THE OPPORTUNITY
Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court along with community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant&rsquo;s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling.
SIJC seeks a Participant Engagement Specialist for the borough&rsquo;s Supervised Release Program (SRP) which provides supervision, case management, and social service connection to thousands of court-mandated participants each year. Reporting to the Clinical Intake Coordinator, the Participant Engagement Specialist will leverage interpersonal and data-management skills to support participants&rsquo; successful engagement with the program. The Participant Engagement Specialist will provide outreach to program participants who missed program orientation, intake, and those who have been disengaged for 30 days or more. Additionally, this role will effectively communicate and collaborate with intake, case management, and compliance teams to ensure accurate court reporting and continued program engagement.
Responsibilities include but are not limited to:<ul><li>
Complete outreach to all disengaged participants mandated to SRP in Staten Island to re-engage them in the program;<ul><li>
Engage in weekly outreach efforts by phone and email to participants and their collateral contacts and defense attorneys to support program engagement;</li></ul></li><li>
Monitor all participants simultaneously assigned bail and SRP, collaborating with the clinical intake team as appropriate;</li><li>
Maintain accurate and timely data entry and case documentation aligned with the program model, court reporting obligations, and best practices;</li><li>
Collaborate with the Clinical Intake Coordinator and the intake, case management, and compliance teams to ensure outreach to all participants whose orientations or intakes are incomplete or who have been disengaged from the program for 30 days or more;</li><li>
Contribute to consistent and effective cross-team communication;<ul><li>
Participate in team and inter-team meetings;</li></ul></li><li>
Participate in individual supervision and regular staff trainings to develop program expertise and related skill sets;</li><li>
Participate in all-staff and departmental meetings to build team cohesion, communication, and morale;</li><li>
Attend project events, community events, and meetings after hours, as needed; and</li><li>
Additional relevant tasks, as necessary.</li></ul>
Qualifications:<ul><li>
High school diploma or equivalent required;</li><li>
Minimum 1 year direct practice or customer service experience required;</li><li>
Excellent written and verbal communication skills required;</li><li>
Comprehensive knowledge of and/or experience with the criminal legal and related systems is valued but not required;</li><li>
Must be detail-oriented with strong organizational and time management skills;</li><li>
Must be able to work effectively and independently and as part of a highly interdependent, multi-disciplinary team within a fast-paced and dynamic work environment;</li><li>
Ability to work collaboratively and effectively with a variety of program stakeholders, including judges, prosecutors, defense attorneys, court staff, and site partner agencies; and</li><li>
Bilingual (English-Spanish) strongly preferred.</li></ul>
Position Type: Full-time in-person work required; weekend and evening hours required, as needed.

Compensation: The compensation range for this position is $53,500 - $61,800 and is commensurate with experience.
